Comparative study of carbohydrate-protein complexes. II. Determination of hydroxylysine and its glycosides in human skin and scar collagens by an improved method. (1/1171)

A modification of the existing methods for measuring hydroxylysine, galactosylhydroxylysine, and glucosylgalactosylhydroxylysine is described. The method is based on analysis with an automated amino acid analyzer using a conventional separation system for basic amino acids. The prior removal of acidic and neutral amino acids was necessary. This was achieved by passing an alkaline hydrolysate of collagen through a column of Amberlite CG-120, Type II (H+) and washing the column with 8% aqueous pyridine. A basic fraction containing the hydroxylysine compounds was then recovered from the column by elution with 3 M NH4OH. Model experiments showed that hydroxylysine and its glycosides could be analyzed with an hour and that recoveries exceeded 90%. This method was applied to human tissues to investigate whether the dermal scar is different in collagen composition from normal skin. With the limited number of samples analyzed, the data suggested that long-standing scar tissues reverted to a composition similar to that of normal skin. The composition of hydroxylysine-linked carbohydrate units is also discussed on the basis of the age-related change.  (+info)

Cicatricial fibromatosis mimics metastatic medulloblastoma. (2/1171)

Cicatricial fibromatoses usually occur in the anterior abdominal wall or in the extremities, but rarely in the scalp or the soft tissues of the neck. We report a case of desmoid fibromatosis that developed in a 15-year-old boy 8 months after surgery for cerebellar medulloblastoma.  (+info)

Abnormal reaction to central nervous system injury in mice lacking glial fibrillary acidic protein and vimentin. (3/1171)

In response to injury of the central nervous system, astrocytes become reactive and express high levels of the intermediate filament (IF) proteins glial fibrillary acidic protein (GFAP), vimentin, and nestin. We have shown that astrocytes in mice deficient for both GFAP and vimentin (GFAP-/-vim-/-) cannot form IFs even when nestin is expressed and are thus devoid of IFs in their reactive state. Here, we have studied the reaction to injury in the central nervous system in GFAP-/-, vimentin-/-, or GFAP-/-vim-/- mice. Glial scar formation appeared normal after spinal cord or brain lesions in GFAP-/- or vimentin-/- mice, but was impaired in GFAP-/-vim-/- mice that developed less dense scars frequently accompanied by bleeding. These results show that GFAP and vimentin are required for proper glial scar formation in the injured central nervous system and that some degree of functional overlap exists between these IF proteins.  (+info)

Role of elevated plasma transforming growth factor-beta1 levels in wound healing. (4/1171)

Transforming growth factor (TGF)-beta1 plays a central role in wound healing. Wounds treated with neutralizing antibody to TGF-beta1 have a lower inflammatory response, reduced early extracellular matrix deposition, and reduced later cutaneous scarring, indicating the importance of local tissue TGF-beta1. By contrast, increasing the local, tissue levels of TGF-beta1 increases the early extracellular matrix deposition but does not alter scar formation. Increased levels of plasma TGF-beta1 correlate with increased fibrogenesis in the lung, kidneys, and liver. The aim of the present study was to investigate the role of elevated systemic levels of TGF-beta1 on wound healing. We used transgenic mice that express high levels of active TGF-beta1 and have elevated plasma levels of TGF-beta1 and wild-type mice of the same strain as controls. Incisional wounds and subcutaneously implanted polyvinyl alcohol (PVA) sponges were analyzed. Surprisingly, cutaneous wounds in transgenic, TGF-beta1-overexpressing mice healed with reduced scarring accompanied by an increase in the immunostaining for TGF-beta3 and TGF-beta-receptor RII and a decrease in immunostaining for TGF-beta1 compared with wounds in control mice. By contrast, the PVA sponges showed the opposite response, with PVA sponges from transgenic mice demonstrating an enhanced rate of cellular influx and matrix deposition into the sponges accompanied by an increase in the immunostaining for all three TGF-beta isoforms and their receptors compared with PVA sponges from control mice. Together, the data demonstrate that increased circulating levels of TGF-beta1 do not always result in increased expression or activity in selected target tissues such as the skin. The two wound models, subcutaneously implanted PVA sponges and cutaneous incisional wounds, differ significantly in terms of host response patterns. Finally, the data reinforce our previous observations that the relative ratios of the three TGF-beta isoforms is critical for control of scarring.  (+info)

Laparoscopic management of an ectopic pregnancy in a previous Caesarean section scar. (5/1171)

A case of laparoscopic management of an ectopic pregnancy in a previous Caesarean section scar is reported. A 30 year old woman was admitted to our hospital for profuse vaginal bleeding 2 weeks after an abortion had been performed. A urine pregnancy test was positive. Abdominal ultrasound revealed a well-encapsulated bulging mass over the lower anterior uterine wall measuring 7x5 cm. Hysteroscopy revealed retained gestational tissue in the lower corpus despite a normal uterine cavity. An incision was made over the most prominent area of the mass by operative laparoscopy. Dark reddish tissue suggestive of the products of conception was removed using grasping forceps. One-layer of continuous endoscopic sutures along the affected uterine wall was made with 1-0 Prolene. Laparoscopy enabled the successful treatment of an unruptured ectopic pregnancy in a previous Caesarean scar and made it possible to preserve the patient's reproductive capability.  (+info)

Clear cell carcinoma arising in a Cesarean section scar endometriosis: a case report. (6/1171)

Endometriosis of a surgical scar is rare and occurs mainly when a hysterectomy or Cesarean section was performed. We describe a 54-year-old woman with a large suprapubic mass as a definite case of a endomerioid carcinoma developing within the scar endometriosis following Cesarean section. Scar endometriosis, as well as endometriosis at other sites, can turn malignant. Endometrioid carcinoma is the most common histological pattern of malignant tumor arising in endometriosis. But clear cell carcinoma is very unusual. A case of primary clear cell carcinoma in endometriosis of a Cesarean section scar is described. To the best of our knowledge, this is the first documented case of endomerioid carcinoma developing within the scar endometriosis in Korea.  (+info)

Bias due to incomplete follow up in a cohort study. (7/1171)

AIM: To investigate the bias introduced by incomplete follow up in a cohort study of ocular outcome after premature birth. METHODS: A geographically defined cohort of children born before 32 weeks' gestation was prospectively recruited at birth to study the ocular outcome at 2 years. On the basis of attendance at 2 years, the children's families were allocated to one of three groups: group 1 attended for follow up, group 2 were difficult to trace, and group 3 were very reluctant for assessment. All children were examined by a single ophthalmologist, masked to these groupings. RESULTS: 558 children (98.8% of study group) were examined, of whom 505 were in group 1, 20 in group 2, and 33 in group 3. The groups which were more difficult to study (groups 2 and 3) showed a significantly higher prevalence of ocular abnormalities, including strabismus (p=0. 02) and cicatricial retinopathy of prematurity (p=0.002) compared with those attending for follow up. Further, not all of these cases could have been identified by review of the children's previous records. Ocular abnormalities would be underestimated by 16% (11.3% in group 1 compared with 13.4% in the total cohort, p=0.77). CONCLUSIONS: This study suggests that the prevalence of abnormalities would be underestimated by incomplete follow up, as those subjects who were most difficult to obtain for study had a significantly higher prevalence of abnormalities.  (+info)

Ophthalmic abnormalities in patients with cutaneous T-cell lymphoma. (8/1171)

PURPOSE: To determine the frequency of ophthalmic abnormalities in patients with cutaneous T-cell lymphoma (mycosis fungoides and Sezary syndrome) and T-cell lymphoma involving the skin and to describe the clinical course of the disease with selected examples. METHODS: A computerized diagnostic retrieval system was used to identify all patients with T-cell lymphoma involving the skin who were examined at the Mayo Clinic (Rochester, Minnesota) between January 1, 1976 and December 31, 1990. The medical records of affected patients were reviewed. RESULTS: During the 15-year interval from 1976 through 1990, cutaneous T-cell lymphoma was diagnosed in 2,155 patients. Of these 2,155 patients, 42 (1.95%; 26 male and 16 female) had at least 1 ophthalmic abnormality attributable to the disease. The diagnoses in these 42 patients were mycosis fungoides in 19, clinical variants of T-cell lymphoma of the skin (most commonly, peripheral T-cell lymphoma) in 11, and Sezary syndrome in 12. Cicatricial eyelid ectropion was the most common finding, affecting 17 (40.4%) of the 42 patients. Thirty-seven patients had findings that, although probably not a direct consequence of cutaneous T-cell lymphoma, have been cataloged in previous studies. CONCLUSION: Although ophthalmic abnormalities in patients with cutaneous T-cell lymphoma are relatively uncommon, the manifestations of the disease are diverse and frequently difficult to treat.  (+info)

Isthmocele is a pouch-like defect on the anterior wall of the uterus at the isthmus. It appears as a fluid-filled pouch in the anterior uterine wall at the site of a previous cesarean section scar. The flow of blood during menstruation through the cervical region may be hampered by the presence of isthmocele. This is because the blood may accumulate in the pouch-like defect because of the presence of fibrotic tissue. This can also cause pelvic pain in the suprapubic area, infertility and abnormal post menstrual bleeding. The global incidence is somewhere between 6.2% and 36%, with an average rate of 21.1%.[1] These symptoms, taken together, have been closely investigated and are called cesarean scar syndrome.[2] Other complications of cesarean scar defect include complications occurring during subsequent pregnancy such as scar dehiscence, scar ectopic pregnancy, placenta previa and accreta. Complications can occur during gynecological procedures like uterine evacuation, hysteroscopy, or during ...

BACKGROUND AND OBJECTIVES: Non-ablative fractional laser-treatment is evolving for burn scars. The objective of this study was to evaluate clinical and histological long-term outcome of 1,540 nm fractional Erbium: Glass laser, targeting superficial, and deep components of mature burn scars. MATERIALS & METHODS: Side-by-side scar-areas were randomized to untreated control or three monthly non-ablative fractional laser-treatments using superficial and extra-deep handpieces. Patient follow-up were at 1, 3, and 6 months. Primary outcome was improvement in overall scar-appearance on a modified-Patient-and-Observer-Scar-Assessment-Scale (mPOSAS, 1 = normal skin, 10 = worst imaginable scar). Secondary outcomes included histology, patient satisfaction (0-10), patient-assessed improvement, and safety. RESULTS: Study was completed by 17 of 20 randomized patients with normotrophic (n = 11), hypertrophic (n = 5) or atrophic (n = 1) scars. Scar-appearance improved from laser-treatments (P , 0.001 vs. ...
In wound healing and fibrosing processes as well as in urticaria an increase of mast cells (MC) has been observed. MC are key-players in urticaria, and might also contribute to tissue repair. In human cutaneous scar tissue (5-369 days old) and normal skin MC dynamics and MC subtypes were analysed by enzyme- and immunohistochemistry. Moreover, the activity of the MC proteases in extracts of both tissues and their in vitro effect on the mitogenesis of fibroblasts and keratinocytes were assessed. To elucidate mechanisms involved in mast cell accumulation, expression of MC chemotaxins, MC growth factors and their receptors was evaluated comparing cutaneous scar tissue (a), lesional and non-lesional skin of urticaria (b) and normal skin (c): SCF, c-Kit, NGF-R TrkA, NGF-R p75, GM-CSF, GM-CSF-R (a, b, c); NGF, TGF-(, TGF-(-R I, TGF-(-R II (a,c) by immunohistochemistry (a, b, c) and by RT-PCR (a, c). Additionally, expression of proinflammatory cytokines (IL-3, -8, TNF-() was studied (b, c). Tryptase and ...

Results Two hundred and thirty patients with 255 EMR scars were included. The prevalence of recurrent adenoma was 24%. Diagnostic values were high for all modes (negative predictive value (NPV) ≥97%, positive predictive value (PPV) ≥81%, sensitivity ≥90%, specificity ≥93% and accuracy ≥93%). In high-confidence cases, NBI with NF had NPV of 100% (95% CI 98% to 100%) and sensitivity of 100% (95% CI 93% to 100%). Use of clips at initial EMR increased diagnostic inaccuracy (adjusted OR=1.68(95% CI 1.01 to 2.75)). In offline assessment, specificity was high for all imaging modes (mean: ≥93% (range: 55%-100%)), while sensitivity was significantly higher for NBI-NF (82%(72%-93%)%)) compared with WL (69%(38%-86%); p,0.001), WL-NF (68%(55%-83%); p,0.001) and NBI (71%(59%-90%); p,0.001). ...
Glial scar formation (gliosis) is a reactive cellular process involving astrogliosis that occurs after injury to the central nervous system. As with scarring in other organs and tissues, the glial scar is the bodys mechanism to protect and begin the healing process in the nervous system. In the context of neurodegeneration, formation of the glial scar has been shown to have both beneficial and detrimental effects. Particularly, many neuro-developmental inhibitor molecules are secreted by the cells within the scar that prevent complete physical and functional recovery of the central nervous system after injury or disease.[citation needed] On the other hand, absence of the glial scar has been associated with impairments in the repair of the blood brain barrier. The glial scar is composed of several components briefly discussed below. Reactive astrocytes are the main cellular component of the glial scar. After injury, astrocytes undergo morphological changes, extend their processes, and increase ...

Normally healed, inconspicuous scars are usually line-shaped and are flat, pale and soft. In some cases, scarring is associated with pathological processes such as the formation of a bulge. Occlusion, moisturizing, and UV protection of the scar are thought to prevent abnormal scar healing.. To occlude, moisturize, and protect the scar from UV radiation, Vesifact developed a silicone-free VESIsorb® technology-based formulation, which quickly forms an elastic, tack-free, water and rub-off resistant polymer film after application onto the scar. In addition to the polymer providing occlusion, the formulation contains moisturizers (phospholipids, Equol) and UVA/UVB filters.. A clinical study with patients having surgical scars compared the VESIsorb® formulation with one of the market leaders and benchmark product, Kelo-Cote® UV, a silicone-based formulation. For all patients treated with the VESIsorb® formulation, the scars were healed completely or almost completely after three months. ...
I had surgery on both hands in 2005. Then because I only waited 3 months in between the surgeries my right hand (first surgery) was doing all the work while my left hand was healing. I didnt give my right hand time to heal. This caused scar tissue to build up in the right hand making it necessary to have surgery a second time. You ask how you know that scar tissue has built up and the answer is that cts comes back all over again. All the numbness and pain came back. I had an orthopedic doctor do the surgeries the first time (through an HMO) where I used to live. Then my surgery in 2009 on my right hand was done by a hand specialist who gave me so much more advice and was great at fixing my cts. I still am working towards getting strength back into my hands, (Im 54 and I sew a lot). But overall Id say my second surgery was a success. I was told to massage the incision area to keep my right hand from forming scar tissue yet again. I wish that my first surgeries were done by the doctor who did ...

Topicals Dont Work. We know acne scars are troublesome for many patients. Unfortunately, battling acne and cystic acne is not uncommon for teenagers (and adults!), and those breakouts can leave behind battle wounds. Many people feel using topicals will do little to improve the appearance of these stubborn scars. However, with the many technologically advanced, medical grade products on the market, treating those scars starts at home. Using a high quality, growth factor product can encourage collagen stimulation, which is the name of the game in treating acne scarring. SkinMedica TNS Recovery® has the highest concentration of a patented growth factor blend. This potent combination of growth factors and cytokines improves the tone and texture of the skin. A customized at home regimen is an important step in smoothing the skin.. Lasers Are King. Lasers are known for being tough on wrinkles, so we assume they are the perfect choice for acne scarring. However, this isnt always the case. ...
Scars are areas of fibrous tissue that form to repair damaged areas of skin and are a natural physiologic response to trauma or injury - they are how the body heals itself. Immediately after an injury there is a phase of reduced blood flow, called vasoconstriction, when bleeding slows while a scar begins to form. Scars are a byproduct of the body forming new collagen fibers, a naturally occurring protein in the body, to mend the damaged skin. It then goes into a phase of vasodilation, or increased blood flow to the wound; this is why scars may initially look red and sometimes appear firm or raised. Good scar prevention starts well before a wound even occurs. Your genetic makeup, wellness habits and overall health history play the largest part in how you will heal and scar. Medical professionals do their best to help you from there. The gold standard for stitch technique is a running subcuticular or intradermal suture - continuous stitches below the skin with dissolvable sutures. In a ...

Jaxi123, do you think you might be an individual that makes a lot of scar tissue after surgery? I am of mixed ancestry, my PT commented to me after my last surgery that it appeared to her that I make a lot of scar tissue and might be due to some of my African American ethnicity. I recently found out that I have very high levels of Transforming Growth Factor Beta 1 and this is involved in fibrosis formation. I do not know if the level was elevated at the time of my surgeries so cant say if it may have played a role in the amount of scar tissue formation she noticed in my scar. As we have already mentioned, scars are not a bad thing. They are part of the bodys natural response to injury and are formed as the skin tries to heal itself. Breast scar tissue is comprised of collagen. When the skin is injured, for example by the surgeons scalpel, fibroblast cells from the surrounding tissues move into the area of injury and start a buildup of collagen which then manifests itself as the scar. Collagen helps to strengthen the wound and facilitate a natural healing process. Collagen is like the bodys glue as Barry H. Dolich, MD, a plastic surgeon based in Bronx puts it.. The buildup of collagen progresses for about 3 months and thereafter, the collagen starts breaking down and consequently the scar tissue fades away gradually.. ...

Cesarean scar pregnancy treated by curettage and aspiration guided by laparoscopy Shan-rong Shu, Xin Luo, Zhi-xin Wang, Yu-hong Yao Department of Obstetrics and Gynecology, The First Affiliated Hospital of JiNan University, HuangPu Road West, Guangzhou, Peoples Republic of China Abstract: Pregnancy in a cesarean scar is the rarest form of an ectopic pregnancy. The treatment for cesarean scar pregnancy mainly includes systemic methotrexate and uterine artery embolization. Here, we reported a case of cesarean scar pregnancy treated by curettage and aspiration guided by laparoscopy. The treatment plan included two phases. Three days after a combination of methotrexate and mifepristone was administered, the gestational sac was removed under laparoscopy, which enabled a successful treatment for the unruptured ectopic pregnancy in a previous cesarean scar and made it possible to preserve the reproductive capability of the patient. Keywords: cesarean scar pregnancy, laparoscopy, curettage and aspiration
Dear Editor,. Cesarean scar pregnancy refers to implantation within the myometrium of a prior cesarean delivery scar. Its incidence approximates 1 in 2000 normal pregnancies and has increased alongside the cesarean delivery rate (1).. Several treatment options are available, but none of them are standard. Because of the risk of uterine rupture and uncontrollable bleeding, hysterectomy is indicated. However, several types of conservative treatments have been used such as dilation, curettage, and excision of trophoblastic tissue using laparotomy or laparoscopy (2, 3).. Fertility preserving options include systemic or locally injected methotrexate (MTX), either alone or combined with conservative surgery (4).. There are two cases of cesarean scar pregnancies, which were treated completely with locally injected methotrexate (in one case) and potassium hydrochloride (in the other one).. Case 1: She was a 33-year-old woman with a history of one previous C/S presented with spotting per vagina of 9 ...
The success rate of glaucoma filtration surgery is closely related to conjunctival inflammation, and the main mechanism of dry eye disease (DED) is inflammation. The aim of this study was to evaluate the effect of DED on bleb scar formation after rabbit glaucoma filtration surgery. Sixteen New Zealand white rabbits were randomly divided into control and DED groups. A DED model was induced by twice-daily topical administration of 0.1% benzalkonium chloride (BAC) drops for three weeks. Ocular examinations were performed to verify the DED model. Surgical effects were assessed, and histologic assessments were performed on the 28th postoperative day. A scar is the marking that remains after tissue damaged from a cut, burn, or other wound has healed. The most familiar scars appear on the surface of the skin, though they also can develop on tissues located inside the body. Scar formation is a natural process that results from replacement tissue being somewhat structurally and functionally different than the original tissue that was injured. Produced by fibroblasts, scar tissue is predominantly composed of fibrous connective tissue made of collagen and is typically thicker, denser, and paler than surrounding tissues. The poor elasticity of scar tissue can limit movement in areas of the body that were extensively damaged, and the lack of oil glands in the inferior tissue is sometimes associated with itchiness or irritation.
To investigate losartans role in muscle injury regeneration in the context of aging, the Hopkins team worked with 40 mice which, at 21-months old, were considered geriatric. The researchers examined the stained muscle tissue under a microscope at four days and again at 19 days, looking for signs of regeneration: small fibers with larger-than-usual nuclei.. After four days, they saw no difference in the number of regenerating fibers between the losartan-treated mice and those not treated. However, after 19 days, the losartan-treated mice had about 10 to 15 per cent of scar tissue formation compared with 30 to 40 per cent of scar tissue formation in those not treated.. Next, the researchers conducted disuse experiments to find out if losartan, in addition to improving muscle regeneration, might have even broader clinical applications in the protection against immobilization atrophy.. Again, using 21-month-old mice, half treated with losartan and half not, the team this time clipped the hind right ...

Although scars do not affect us physically, they can have psychological effects.. FOR many of us, scars are a real-life concern. There are many types of scars - chicken pox, cuts and grazes, surgical scar, etc.. Of the numerous myths regarding scars and scar healing, one of the most common is consuming dark soy sauce and food like seafood and eggs can predispose one to scarring. The popular view is that dark soy sauce, when taken, can result in darkening and thickening of scars, and that taking prawns or crab meat can result in scarring. There is no medical evidence to indicate that this is the case, and in the latter, additional protein may actually help wound healing in certain patients.. What is a scar and why do we scar?. Following an injury, the body acts as quickly as possible to patch the wound together. There is increased production of collagen at the site of the wound and numerous strands of fibrous material to draw the edges of the wound together, culminating in the formation of a ...
Objectives: Image integration into clinical mapping systems could be used to display a 3D myocardial scar map and its border zone to assist substrate-guided ventricular tachycardia (VT) ablation. This study reports the first clinical experience of simultaneous display of 3D scar embedded into LV anatomy, characterization of scar and border zone using metabolic 18-F fluorodeoxyglucose (FDG) PET images and anatomic substrate evaluation using CT images.. Methods: Ten patients scheduled for VT ablation underwent contrast-enhanced CT and Rubidium-82/FDG PET imaging to create a 3D LV model and 3D FDG scar map. The FDG segment uptake with peak Rb perfusion was used as 100% to normalize FDG data. Using a novel imaging module LV and scar models were co-registered with a 3D mapping system and analyzed with a 17 segment model. Registration errors and alignment of 3D scar from FDG embedded into LV anatomy and voltage map were analyzed. Segments with all voltage values , 0.5mV were defined as scar only; ...

The present invention comprises a modified womans undergarment that includes a peri-pubic light compression panel allowing for compression at the incision site and a treatment dressing means for exposing the wound to a medicament. The combination of light compression and treatment with applicable medicament functions to minimize the formation of scars following cesarean section incisions performed in the lower transverse uterine or peri-pubic area. The undergarment is not limited in style and can include various styles such as a bikini, thong, low-rise, support style, full or high cut undergarment. Considering the long phase of wound healing, this design flexibility allows for the use of the garment under a variety of street clothes. Unlike a girdle or abdominal support device, the crux of the invention does not require heavy or high abdominal support, although for those applications requiring girdle-type support, the invention could also be fabricated in a style that allows abdominal support
