The features that constitute an "ideal" functional occlusion have not been conclusively established. Orthodontic treatment has the capacity to change static and functional occlusal relationships fundamentally. In this article, we present the evidence on which features of the occlusion are reported to be detrimental to the teeth and masticatory system Deficiencies in this research area are highlighted, together with the need for prospective longitudinal trials to clarify the requirements of an ideal functional occlusion Based on the existing evidence this paper suggests which occlusal features may be significant in producing an "ideal" functional occlusion As no long-term studies exist to measure the impact of non-ideal occlusal relationships on the dentition, it is debatable whether orthodontic treatment should be prolonged in order to ensure that "ideal" occlusal contacts are achieved As the occlusion tends to "settle" in the period following appliance removal, we propose that it may be more appropriate to examine the functional occlusal relationships after retention has ceased rather than prolong active orthodontic treatment to achieve "ideal" functional occlusal goals.  (+info)

Regulation of mandibular postures: mechanisms and clinical implications. (2/35)

This review argues that (1) the habitual mandibular position is constantly variable and so cannot be considered as a craniomandibular reference point, (2) there is no unique centric relation, (3) mandibular posture greatly depends on head posture, (4) clinical evaluation of the occlusal vertical dimension is mostly empirical, and (5) neither the vertical dimension at rest nor the centric relation can be determined by means of existing instrument-based clinical methods. Orthodontic side-effects of mandibular advancement devices during treatment of snoring and sleep apnoea. (3/35)

The aims of this study were to investigate possible orthodontic side-effects following the use of mandibular advancement devices (MAD) in adults with snoring and sleep apnoea. A second objective was to analyse the effect of the appliance design. Seventy-five patients treated with MAD and 17 reference patients were studied at follow-up after 2.5 +/- 0.5 years. In the test group, 47 patients were provided with soft elastomeric devices, while the remaining 28 patients received hard acrylic devices. The treatment induced a change in overjet of -0.4 +/- 0.8 mm (mean +/- SD) and a change in overbite of -0.4 +/- 0.7 mm (mean +/- SD). These changes were larger than those found in the reference group (P < 0.01). The odds ratio (OR) for the largest quartile of reduction in overjet was 3.8 in patients using hard acrylic devices compared with those using soft elastomeric devices (P < 0.05). A large reduction in overjet in patients using the hard acrylic devices was unrelated to the degree of mandibular protrusion by the device. The OR for a large reduction in overjet in patients using the soft elastomeric devices with a protrusion of 6 mm or above was 6.8 compared with smaller mandibular protrusions (P < 0.05). The results indicate that the orthodontic side-effects are small during the treatment of adult subjects with MAD for snoring and sleep apnoea, especially in patients using soft elastomeric devices with mandibular protrusions of less than 6 mm. The follow-up of patients treated with MAD is recommended, as individual patients may experience marked orthodontic side-effects.  (+info)

The aim of this series of papers is to explore the role of occlusion in dental practice. The range of opinion in the dental profession as to the importance of occlusion is enormous. It is very important that the profession in general and practising dentists in particular have a balanced view of occlusion. This is more important than every patient having a balanced occlusion. The fact that the study of occlusion is characterised by extremes makes it confusing and possibly difficult for individual dentists to find a philosophy which is in line with contemporary good practice supported by evidence from practice-based research.  (+info)

In this case, the condyle is approximately centered in the glenoid fossa. More specifically, the condyle is slightly closer to the top and the back of the glenoid fossa. This relaxed, centralized position of the condyle within the glenoid fossa is called centric relation. If the TM Joints are in a state of health, they tend to approximate this position whenever the teeth are slightly separated and the muscles of mastication are relaxed. This study investigates changes following bimaxillary osteotomy for correction of Class II malocclusion. The records of 15 patients who had simultaneous maxillary impaction and sagittal split ramus osteotomy with rigid fixation are evaluated. Traditional cephalometric analysis as well as finite element and Euclidean distance matrix analyses are used to assess the changes. The maxilla was relatively more stable than the mandible; the maxilla stayed within 1 mm of its immediate postoperative position, whereas the mandible was 2 mm from the achieved surgical changes. The mandible rotated in a clockwise direction during the first 6 months after surgery. The mandibular plane angle increased by 2.9 degrees. The aim of this study was to evaluate the degree of abrasion in posterior teeth of subjects with unilateral posterior crossbite in the deciduous dentition. A group of 54 untreated subjects in deciduous dentition (test group, TG) was selected from a parent sample of 1500 patients from the files of the Department of Orthodontics of the University of Florence. A sample of 20 subjects with normal occlusion in the deciduous dentition was selected as the control group (CG). As experimental units in TG, 54 crossbite sides (CB, consisting of 33 right and 21 left crossbite sides) and 54 noncrossbite sides (NCB) were used. In CG, one randomly chosen single side for each subject (NCBC) was used for comparisons. The findings of the present study indicated that subjects with unilateral crossbite in the deciduous dentition showed a significantly smaller degree of dental abrasion in the crossbite side when compared with both the opposite side and control group sides.
