For all 3 techniques, the repeatability of replicate counts or of measurements made at short intervals were high which suggests that all 3 assays were reliable. Within an infection the repeatability of different samples from the same animal decreased as the interval between samples increased. OBJECTIVE AND METHOD: The aim of this study was to evaluate how frequently EB recurs and whether it develops into chronic airway obstruction. Cough severity, FEV1, PC20 methacholine and sputum eosinophil percentages were serially measured in 36 subjects for up to 48 months. RESULT: Five subjects developed recurrent episodes of EB (RG) between 4 to 6 months after the first episode of EB. Asthma developed in the 9th month of the study in one of these subjects and a progressive FEV1 reduction exceeding 20% was observed in two. Nineteen subjects had no recurrence of EB (NRG). BACKGROUND. The link between mucus plugs and airflow obstruction has not been established in chronic severe asthma, and the role of eosinophils and their products in mucus plug formation is unknown. METHODS. In clinical studies, we developed and applied a bronchopulmonary segment-based scoring system to quantify mucus plugs on multidetector computed tomography (MDCT) lung scans from 146 subjects with asthma and 22 controls, and analyzed relationships among mucus plug scores, forced expiratory volume in 1 second (FEV1), and airway eosinophils. Additionally, we used airway mucus gel models to explore whether oxidants generated by eosinophil peroxidase (EPO) oxidize cysteine thiol groups to promote mucus plug formation. RESULTS. Induced sputum cell counts are a non-invasive, reliable method for evaluating the presence, type, and degree of inflammation in the airways of the lungs. Current reference values for induced sputum cell counts in healthy adults do not account for the effects of circadian rhythm, including diurnal variation. The objective of this study was to describe the diurnal variation in induced sputum cell counts, compared between early morning and late afternoon, in healthy adult individuals. 100 healthy adult subjects with no history of lung disease and normal bronchial reactivity proceeded with induced sputum testing at 7 am and 4 pm on different days. The order of testing was randomized. The cytotechnologist preparing and performing the cell counts was blinded to the sample collection time and subject characteristics. 65 subjects were included in the final analyses. Roflumilast is a targeted, oral, once-daily administered phosphodiesterase 4 (PDE4) inhibitor with clinical efficacy in COPD. Results from in vitro studies with roflumilast indicate that roflumilast has anti-inflammatory properties that may be applicable for the treatment of COPD. In this cross-over study, 38 patients with COPD (mean (SD) age 63.1 (7.0) y, post-bronchodilator FEV1 61.0 (12.6) %predicted) received 500 μg roflumilast or placebo once daily for 4 weeks. Introduction. Posaconazole is a triazole antifungal that is used in the treatment of a variety of fungal infections, as well as in the management of mucormycosis (on an off-label basis). Eosinophilia associated with exposure to azole antifungals has been described rarely in the literature. Case presentation. A 31-year-old male on peritoneal dialysis (PD) for end-stage renal disease, secondary to diabetic nephropathy, presented to hospital with abdominal pain after a trip to St Lucia. He was taken to the operating room, where the PD catheter was removed and an abdominal-wall abscess was debrided. R hizopus species was recovered on culture of the abdominal-wall tissue, and the patient was started on amphotericin B deoxycholate. He was subsequently stepped down to posaconazole, for a planned treatment duration of 12 months. Approximately 43 days after the initiation of posaconazole, it was noted that his peripheral eosinophil count started to rise. No other cause for the eosinophilia was identified.

A 60-year-old female from rural area came to pulmonary out patient department with a history of cough and breathlessness since five months. She denied for fever, chest pain and hemoptysis. Her routine blood investigations were normal except total eosinophil count of 850/mm3. Her chest X-ray showed increased bronchovascular marking with curvilinear, coiled and serpentine calcification in the left upper zone suspecting calcified guinea worms (Figure 1). On examination a hard, crackling feeling present over left supra and infraclavicular region. Overlying skin was normal in color and texture. Past history suggestive of guinea worm infection with multiple times thread like worms had been taken out from her lower limbs. She was consuming unsafe drinking water from an open pond. Pulmonary function test revealed mild obstructive pattern. She advised for excisional biopsy of calcified guinea worm but did not give consent, later managed with bronchodilator and symptomatic treatment.

We report a case of a 90-year-old man with hypereosinophilia, lymphadenopathies and skin lesions, namely lichenification and pruritus. An aetiological investigation was performed, and a bone marrow (BM) biopsy and aspirate showed a hypercellular marrow with hypereosinophilia without dysmorphia or abnormal elements, and the BM and inguinal nodes immunophenotyping denied any presence of abnormal lymphoid cell population. The inguinal node biopsy revealed a multinodular proliferation of large cells S100 and CD1a+, and a diagnosis of Langerhans cell histiocytosis was made. The hypereosinophilia and skin lesions were managed with corticotherapy with substantial improvement of cutaneous lesions and lymphadenopathies and normalisation of eosinophil count. Both mitogen stimulated lymphoid cells and antigen stimulated helper T-cells generated potent colony-stimulating activity in conditioned medium. Monocyte/macrophage, neutrophil, eosinophil, basophil/mast cell, neutrophil/monocyte and mixed phenotype colonies developed in stimulated bone marrow cultures in a conditioned medium dose-dependent manner. I first heard of EE disease when I was in my pediatric residency. I worked with a Pediatric GI specialist who seemed to diagnose all of his infant patients with gastroesophageal reflux (GERD) with EE. When I learned about EE I had no idea that my dear husband had the very same problem!. To elucidate the link between the intake of animal fat and asthma, a murine model was developed to examine the effect of dietary cholesterol on pulmonary allergic inflammation. Male C57BL6 mice were fed either a control diet or a diet supplemented with 2% cholesterol. Following sensitization and inhalation exposure to ovalbumin, the bronchoalveolar lavage fluid of mice in the cholesterol group contained higher numbers of eosinophils and elevated levels of IL-5, PGE2, and MCP-1. In addition, dietary cholesterol also resulted in elevated production of IL-4 and IFN-γ by lymphocytes isolated from the lungs. We examined 24 pre-CSS asthmatic patients and 294 non-CSS asthmatic patients for clinical features, percent forced expiratory volume at 1 second (%FEV1), BHR to acetylcholine, and evaluated eosinophils (%) in the peripheral blood at their first hospital visit for asthma treatment. All of the 24 pre-CSS patients had adult-onset asthma. The asthma of 87.5% of pre-CSS patients at the first hospital visit before the onset of CSS was severe and was complicated by sinusitis. Abstract Between 1974 and 1985, 10 patients were studied who presented with either gastrointestinal complaints or unexplained peripheral blood eosinophilia, and had eggs typical for N. salmincola recovered from their stools. Clinical findings in 8 of the 10 included increased frequency of bowel movements or diarrhea (6), peripheral blood eosinophilia (6), abdominal discomfort (5), nausea and vomiting (3), weight loss (2), and fatigue (2). Two were asymptomatic. Eight recalled eating fish prior to the onset of symptoms. Anthelminthic treatment consisting of three 2-g doses of niclosamide (2 patients) or two 50 mg/kg doses of bithionol (1 patient) proved effective.
