Modulation of cellular annexin I in human leukocytes infiltrating DTH skin reactions. (1/380)

Based on our previous studies showing endogenous annexin I being depleted from migrated neutrophils (PMN) in vitro, we have tested whether the levels of this glucocorticoid-regulated protein in PMN and mononuclear cells (PBMC) were modified after adhesion to endothelial monolayers in vitro and extravasation into skin blisters in vivo. In vitro, annexin I levels were depleted more significantly (-70%) in post-adherent PMNs than in monocytes (-25%) and lymphocytes (-50%, only in the positive fraction). In vivo, a significant time-dependent increase (approximately threefold, P < 0.05) in cell-associated annexin I was measured in PBMCs recovered from the blisters, whereas no significant changes were detected in extravasated PMNs. This was associated with annexin I release in the blister fluids (approximately 35 ng/mL), whereas no detectable protein was found in matched-paired plasmas. In conclusion, we report for the first time an activation of the annexin I pathway during an ongoing experimental inflammatory response in humans, which is differently regulated between PMNs and PBMCs.  (+info)

Spontaneous regression of a bulla with the development of adenocarcinoma of the lung. (2/380)

Spontaneous regression of a bulla in the lung is rare. We describe a case of spontaneous regression associated with the development of adenocarcinoma of the lung in a 59-year-old male smoker. The bulla had begun to regress spontaneously at least six months before lung cancer was detected on a chest radiograph. He underwent left upper lobe lobectomy with mediastinal node dissection. The tumor arose within the bulla, extending along the bulla wall. He has been alive for more than eight years with no evidence of recurrence. This case suggests that spontaneous regression of a bulla should be recognized as one of the early radiographic signs of the development of lung cancer in patients with bullous lung disease.  (+info)

Pemphigus vulgaris and pemphigus foliaceus antibodies are pathogenic in plasminogen activator knockout mice. (3/380)

Previous studies have suggested that urokinase plasminogen activator is required for blister formation in pemphigus vulgaris and pemphigus foliaceus. Other studies, however, have shown that downregulation of plasminogen activator does not inhibit blisters induced by pemphigus immunoglobulin G. To eliminate the possibility that small amounts of urokinase plasminogen activator might be sufficient for blister formation, we passively transferred pemphigus immunoglobulin G to urokinase plasminogen activator knockout neonatal mice. Pemphigus foliaceus and pemphigus vulgaris immunoglobulin G caused gross blisters and acantholysis in the superficial and suprabasal epidermis, respectively, to the same degree in knockout and control mice, demonstrating that urokinase plasminogen activator is not absolutely required for antibody-induced blisters. Some studies have shown elevated tissue-type plasminogen activator in pemphigus lesions. Tissue-type plasminogen activator, however, is not necessary for blister formation, because pemphigus foliaceus and pemphigus vulgaris immunoglobulin G caused blisters to the same degree in tissue-type plasminogen activator knockout and control mice. To rule out that one plasminogen activator might compensate for the other in the knockout mice, we bred urokinase plasminogen activator, tissue-type plasminogen activator double knockouts. After passive transfer of pemphigus foliaceus and pemphigus vulgaris immunoglobulin G these mice blistered to the same degree as the single knockout and control mice, and histology indicated blisters at the expected level of the epidermis. These data definitively demonstrate that plasminogen activator is not necessary for pemphigus immunoglobulin G to induce acantholysis in the neonatal mouse model of pemphigus.  (+info)

The functional diversity of epidermal keratins revealed by the partial rescue of the keratin 14 null phenotype by keratin 16. (4/380)

The type I epidermal keratins K14 and K16 are remarkably similar at the primary sequence level. While a structural function has been clearly defined for K14, we have proposed that a function of K16 may be to play a role in the process of keratinocyte activation that occurs after acute injury to stratified epithelia. To compare directly the functions of the two keratins we have targeted the expression of the human K16 cDNA to the progenitor basal layer of the epidermis of K14 null mice. Mice null for K14 blister extensively and die approximately 2 d after birth (Lloyd, C., Q.C. Yu, J. Cheng, K. Turksen, L. Degenstein, E. Hutton, and E. Fuchs. 1995. J. Cell Biol. 129:1329-1344). The skin of mice expressing K16 in the absence of K14 developed normally without evidence of blistering. However, as the mice aged they featured extensive alopecia, chronic epidermal ulcers in areas of frequent physical contact, and alterations in other stratified epithelia. Mice expressing a control K16-C14 cDNA also rescue the blistering phenotype of the K14 null mice with only a small percentage exhibiting minor alopecia. While K16 is capable of rescuing the blistering, phenotypic complementation in the resulting skin is incomplete due to the multiple age dependent anomalies. Despite their high sequence similarity, K16 and K14 are not functionally equivalent in the epidermis and other stratified epithelia and it is primarily the carboxy-terminal approximately 105 amino acids of K16 that define these differences.  (+info)

Penetration of moxifloxacin into peripheral compartments in humans. (5/380)

To characterize the penetration of moxifloxacin (BAY 12-8039) into peripheral target sites, the present study aimed at measuring unbound moxifloxacin concentrations in the interstitial space fluid by means of microdialysis, an innovative clinical sampling technique. In addition, moxifloxacin concentrations were measured in cantharides-induced skin blisters, saliva, and capillary plasma and compared to total- and free-drug concentrations in venous plasma. For this purpose, 12 healthy volunteers received moxifloxacin in an open randomized crossover fashion either as a single oral dose of 400 mg or as a single intravenous infusion of 400 mg over 60 min. An almost-complete equilibration of the free unbound plasma fraction of moxifloxacin with the interstitial space fluid was observed, with mean area under the concentration-time curve (AUC)(interstitial fluid)/AUC(total-plasma) ratios ranging from 0.38 to 0.55 and mean AUC(interstitial fluid)/AUC(free-plasma) ratios ranging from 0.81 to 0.86. The skin blister concentration/plasma concentration ratio reached values above 1.5 after 24 h, indicating a preferential penetration of moxifloxacin into inflamed lesions. The moxifloxacin concentrations in saliva and capillary blood were similar to the corresponding levels in plasma. Our data show that moxifloxacin concentrations attained in the interstitial space fluid in humans and in skin blister fluid following single doses of 400 mg exceed the values for the MIC at which 90% of isolates are inhibited for most clinically relevant bacterial strains, notably including penicillin-resistant Streptococcus pneumoniae. These findings support the use of moxifloxacin for the treatment of soft tissue and respiratory tract infections in humans.  (+info)

Ectosomes released by human neutrophils are specialized functional units. (6/380)

Here we show that human polymorphonuclear leukocytes (PMN) release ectosomes independently of complement attack during their activation both in vitro and at the site of inflammation in vivo. Patterns of biotinylated proteins on the surface of PMN and on PMN-derived ectosomes indicated a specific sorting of cell surface proteins into and out of ectosomes. Ectosomes expressed clusters of complement receptor 1 (CR1), which allowed them to bind efficiently to opsonized bacteria. Myeloperoxidase and human leukocyte elastase, both stored within the azurophilic granules of PMN, were found to colocalize on ectosomes with CR1. Furthermore, myeloperoxidase colocalized with human leukocyte elastase. In contrast, not present on CR1-expressing ectosomes were CD63, a selective marker for the azurophilic granules, and CD14, which is located within the same granules and the secretory vesicles as CR1. Of the other complement regulatory proteins expressed by PMN, only CD59 colocalized with CR1, while CD55 and CD46 were almost absent. Ectosomes released by activated PMN at the site of inflammation may function as a well organized element (ecto-organelle), designed to focus antimicrobial activity onto opsonized surfaces.  (+info)

Targeted inactivation of the type VII collagen gene (Col7a1) in mice results in severe blistering phenotype: a model for recessive dystrophic epidermolysis bullosa. (7/380)

Dystrophic forms of epidermolysis bullosa (DEB) are associated with mutations in the type VII collagen gene (Col7a1) which encodes the major component of anchoring fibrils. To develop a DEB animal model, type VII collagen deficient mice were generated by targeted homologous recombination. The targeting vector replaced exons 46-69 of Col7a1 with the neomycin-resistance gene, in reverse transcriptional orientation, resulting in elimination of most of the collagenous domain 1. Col7a1 heterozygous (+/-) mice were phenotypically normal. Mating of Col7a1 +/- mice revealed that Col7a1 null (-/-) mice, which were born with extensive cutaneous blistering, died during the first two weeks of life probably due to complications arising from the blistering. Transmission electron microscopy revealed subepidermal blistering below the lamina densa and absence of anchoring fibrils. Immunohistochemical staining with anti-human type VII collagen antibody stained the dermal-epidermal junction in control mice, but did not stain the skin of Col7a1 null mice. Collectively, the DEB mice recapitulate the clinical, genetic, immunohistochemical and ultrastructural characteristics of recessive DEB in humans. These mice provide an animal model to study the pathomechanisms of DEB and serve as a system to test therapeutic approaches, including gene replacement, towards the cure of this devastating skin disease.  (+info)

BALB/c mice produce blister-causing antibodies upon immunization with a recombinant human desmoglein 3. (8/380)

Pemphigus vulgaris (PV) is an Ab-mediated autoimmune blistering disease of mucotaneous surfaces. Over 95% of the patients with PV express DR4 or DRw6, and the disease is characterized by the presence of autoantibodies directed against desmoglein 3 (Dsg 3), a protein expressed on keratinocytes. An appropriate animal model is required to understand immunoregulation and to address the role of immunogenetic components in the production of pathogenic Abs that are characteristic of PV. Therefore, we turned to the development of a mouse model. Four strains of female mice (BALB/c, DBA/1, SJL/J, and HRS/J) were screened for their ability to produce pathogenic anti-Dsg 3 Abs. We demonstrated that only BALB/c mice immunized with a full-length Dsg 3 can produce pathogenic Abs capable of causing acantholysis of human foreskin in culture and blistering in neonatal mice. This observation suggested that either H-2d or the BALB background contains the immunogenetic makeup necessary for the production of pathogenic anti-Dsg 3 Abs. No correlation was noted between a given isotype and the pathogenic potential of autoantibodies from different strains of mice. Similarly, the pattern of reactivity of Abs with a panel of 46 synthetic peptides that span the entire Dsg 3 failed to reveal any association between binding specificity and the pathogenic potential, and suggested that pathogenic Abs might recognize conformational epitopes. Moreover, our studies showed that the epitopes recognized by pathogenic Abs are contained within the extracellular Dsg 3.  (+info)

AU - Yancey, Kim B.. PY - 2000. Y1 - 2000. N2 - Patients with one form of cicatricial pemphigoid have IgG antibasement membrane autoantibodies against laminin 5 (α3β3γ2). Although passive transfer of rabbit anti-laminin 5 IgG to neonatal mice has been shown to induce subepidermal blisters that mimic those in patients, it has not been possible to directly assess the pathogenic activity of human autoantibodies in this animal model because the latter do not bind murine skin. To address this question, a disease model in adult mice as well as SCID mice bearing human skin grafts was developed. Adult BALB/C mice challenged with rabbit anti-laminin 5 IgG developed, in a concentration-related fashion, erythema, erosions, and crusts surrounding injection sites, histologic evidence of noninflammatory, subepidermal blisters, ...

A blister is a small pocket of body fluid (lymph, serum, plasma, blood, or pus) within the upper layers of the skin, typically caused by forceful rubbing (friction), burning, freezing, chemical exposure or infection. Most blisters are filled with a clear fluid, either serum or plasma. However, blisters can be filled with blood (known as blood blisters) or with pus (if they become infected). The word blister entered English in the 14th century. It came from the Middle Dutch bluyster and was a modification of the Old French blostre, which meant a leprous nodule-a rise in the skin due to leprosy. In dermatology today, the words vesicle and bulla refer to blisters of smaller or greater size, respectively. To heal properly, a blister should not be popped unless medically necessary. If popped, the excess skin should not be removed because the skin underneath needs that top layer to heal properly. A blister may form when the skin has been damaged by friction or rubbing, heat, cold or chemical ...
Blisters are fluid-filled bubbles that form on the skin of the foot or other parts of the body due to friction or a burn. On the foot, they commonly occur on the heel and toes, areas subjected to friction and rubbing against footwear. Most blisters are filled with clear fluid and do not usually require medical attention; however, larger, more serious, painful blisters may require a visit to the doctor, as they can be filled with pus or blood.. A blister provides a protective barrier so that new skin can form in a clean, sterile environment unexposed to dirt and germs while the skin heals. Since blisters are bodys natural response to protecting the skin, most blisters should not be and do not need to be punctured to release the fluid. To prevent pain and infection, blisters can be covered loosely with an adhesive bandage or special, donut-shaped pad.. If a blister pops or opens, wash it with soap and water only; other solutions could burn and irritate the new skin forming underneath. The ...
BLISTER AGENTS Blister agents or vesicants are likely to be used to produce casualties and force opposition to wear full protective equipment. Blister agents are used to degrade fighting efficiency rather than kill, alhotugh exposure to such agnets can be fatal. More likely they will cause severe blistering and burning of the exposed skin. In either liquid or vapor form, these agents will irritate and blister any part of the body that they touch. blister agents can be effective in small amounts; a drop the size of a pinhead may produce a blister the size of a quarter. These agents are more effective in hot weather than in cold weather. They first affect the moist parts of the body (bends of arms and knees, armpits, and crotch.) People who are sweating are especially subject to severe burns. If you are exposed to blister agents, changes may not occur immediately. One to several hours may pass before your skin starts to turn red. It may be hours or even days later before blisters appear. However, ...

Administration of topical antibiotics led to resolution of the lesions within 3 weeks, and there were no signs of scarring or recurrence.. The clinical presentation and histopathologic findings were consistent with a diagnosis of coma blisters.. Clinically, coma blisters are characterized by tense clear or hemorrhagic blisters that develop on erythematous-violaceous macules or plaques of varying size. The lesions typically appear within 24hours of the intake of drugs and within 48 to 72hours of the loss of consciousness. They primarily develop on pressure points, such as fingers and toes, elbows, knees, ankles, and heels. They are self-limiting and heal within days or weeks, without causing scarring or atrophy. The only treatment indicated thus is topical treatment to prevent secondary infections.2,8. Multiple factors have been implicated in the etiology and pathogenesis of coma blisters, including local pressure or friction, generalized hypoxia and tissue ischemia, direct toxicity due to drugs ...
Most blisters are caused by friction or minor burns and do not require medical attention. New skin will form underneath the affected area and the fluid is simply absorbed. You can soothe ordinary blisters with vitamin E ointment or an aloe-based cream.. Do not puncture a blister unless it is large, painful, or likely to be further irritated. If you have to pop a blister, use a sterilized needle or razor blade. Wash the area thoroughly, then make a small hole and gently squeeze out the clear fluid. Apply a dab of hydrogen peroxide to help protect against infection. Do not remove the skin over a broken blister. The new skin underneath needs this protective cover. Cover the area with a bandage and mild compression.. To avoid blisters, you need to eliminate the irritant. Some simple ways to avoid blisters are to avoid tight clothing, make sure socks and shoes fit properly, and when doing heavy work with your hands, wear work gloves.. ...

One of the reasons that figuring out whether youre suffering from a blister or a hemorrhoid is so important is because the causes and preventions for the two are very different. Hemorrhoid causes and prevention are covered quite exhaustively elsewhere on this site, so we wont go over it again. However, the causes and prevention for blisters in the hemorrhoid area are worth noting.. Friction blisters are by far the most common variety of blister to suffer from anywhere on the body. The basic idea is that forceful rubbing from some source causes enough shear forces within the skin to cause separation and filling. Naturally, there are a couple of possible sources this may come from in the hemorrhoidal area. Now blisters. Always carry a trail-sized first aid kit. Add to it some moleskin and a pair of small scissors. If you sense a sore spot on a heel, dont wait until you have a full-blown blister. Take a break to deal with the sore spot. Pull off your boots and sock and let your feet dry; meanwhile, cut an appropriate-sized piece of moleskin to cover the sore area. Place the moleskin over the spot; then put your socks and boots back on. You dont want your skin to blister. A blister will eventually break, creating an open sore. Given the lax state of personal hygiene on many hikes, this sore can get infected, causing a whole bunch of new problems. Dont prick a blister; instead, cover it with moleskin, and it well eventually dry out on its own. Hand sanitizer is a good antiseptic if you have a blister that does break. It burns a bit since most hand sanitizers contain alcohol. Small, travel-sized containers of hand sanitizer are ideal for hikers and backpackers. Hand sanitizer is good to treat ...

Friction blisters, caused by rubbing against the skin, can be prevented by reducing the friction to a level where blisters will not form.[1][2][3][5] This can be accomplished in a variety of ways.. Blisters on the feet can be prevented by wearing comfortable, well-fitting shoes and clean socks. Inherently ill-fitting or stiffer shoes, such as high heels and dress shoes, present a larger risk of blistering. Blisters are more likely to develop on skin that is moist, so socks that manage moisture or frequent sock changes will aid those with particularly sweaty feet. While exercising or playing sports, special sports socks can help keep feet drier and reduce the chance of blisters.[6] Before going for a long walk, it is also important to ensure that shoes or hiking boots have been properly broken in.. Even before a hot or irritated area on the foot is felt, taping a protective layer of padding or a friction-reducing interface between the affected area and the footwear can prevent the formation of ...
Many women who wear cute shoes, or are active in general, are with blisters. These fluid filled sacs on the skin can be caused by many things, including friction, pressure, poor-fitting shoes, heat (burns), sunburns, athletes foot, contact dermatitis, allergic reactions, cancer and skin swelling. The fluid found in the blisters is generally clear, but it can be bloody. If it hurts or is severely infected or caused by a burn, you might want to see a doctor. Blisters dont necessarily need to be treated because they can heal on their own, but you can try to avoid further injuring the area with the blister and protect it with a bandaid. Dont pop or interfere with the blister because that could cause an infection. If it pops on its own, make sure to wash it, put antibiotic ointment on it and cover it with a bandaid.. ...
As foot blisters are mainly caused by friction, heat and sweating, avoiding these will reduce your chances of getting blisters on feet. Most of these preventive measure help hold back blisters from happening. However, some walkers will try everything, yet still fail to beat these watery fluids from occurring.. To minimize friction, choose shoes that fit you best. Extremely snug or loose shoes can create a problem. Avoid wearing too small or too big socks as they are responsible for friction which leads to blisters. Also try to keep your feet dry and away from sweaty conditions. You may do this with the help of talcum powder or petroleum jelly. Plus, avoid exposure to chemical irritants and sources of allergen (such as curtains and plants) that may trigger blisters.. ...
Blisters are defined as a local swelling of the skin that contains a watery fluid and causes burning or irritation. Since most blisters are either caused from a minor burn or friction, a doctors care is not usually required. Some common areas of the body where a blister might appear are on the hands and feet. According to the size and location of the blister, the bruising can be very painful for some people. In order to heal the blister, proper treatment is needed to minimize infection.
Blisters are small pockets of fluid in an upper layer of the skin caused by friction, burning or freezing. Most blisters are filled with a clear fluid called serum or plasma (blister water). Some blisters, however, are filled with blood; they are called blood blisters. ...
Often caused by friction and heat, blisters are common in people who are very active or who wear ill- fitting shoes. Blisters are most likely to form in hot weather when feet are sweaty and more likely to rub against shoes or between toes from flip flops. Wearing shoes without socks and walking or running for long periods of time can also lead to blisters. They can also appear when the skin reacts to substances like chemicals, detergents and solvents. Having a condition like diabetes where sensation in the feet is reduced can make you more likely to suffer with blisters as you may not feel any rubbing before a blister forms.. ...

Blister packs are commonly used as unit-dose packaging for pharmaceutical tablets, capsules or lozenges. Blister packs can provide barrier protection for shelf life requirements, and a degree of tamper resistance. In the US, blister packs are mainly used for packing physician samples of drug products or for over-the-counter (OTC) products in the pharmacy. In other parts of the world, blister packs are the main packaging type since pharmacy dispensing and re-packaging are not common. A series of blister cavities is sometimes called a blister card or blister strip as well as blister pack. The difference between a strip pack and blister pack is that a strip pack does not have thermo-formed or cold formed cavities; the strip pack is formed around the tablet at a time when it is dropped to the sealing area between sealing moulds. In some parts of the world the pharmaceutical blister pack is known as a push-through pack (PTP), an accurate description of two key properties (i) the lidding foil is ...

Intraocular pressure control improved in both groups. The results in the trabeculectomy group were similar to those of shorter term studies of phaco-trab in low risk patients.10,19-23 It is of interest to compare the results of this study with the larger series of primary glaucoma with additional risk factors drained by implants6 and primary glaucoma drained by trabeculectomy5 previously reported from this institution. In the case of cataract extraction combined with implants, the behaviour of the IOP was similar to that observed in the larger series of primary glaucoma with risk factors, with most cases showing a progressive fall in IOP and reduction in the need for hypotensive medication. In the case of cataract extraction combined with trabeculectomy the long term outcome was less favourable, with a tendency for the mean IOP to rise after one year. This trend consisted of cases in which drainage improved with time, and others where drainage deteriorated. One major difference between the two ...

Another guest blog post by Elizabeth A. Ayello, PhD, RN, ACNS-BC, CWON, MAPWCA, FAAN. Good news! Since the educational training programs held last March and April, CMS has reconsidered its original guidance regarding how to code blister pressure ulcers on the revised Minimum Data Set (MDS) version 3.0. Previously, CMS directed that any pressure ulcer that presents as a blister regardless of what type of fluid seen (serous, sero-anguineous, or blood filled) was to be coded as a stage 2 . This is no longer true, not all pressure ulcer blisters are coded as stage 2 for MDS 3.0.. This July, CMS posted new information on its website that shifts the coding of pressure ulcer blisters to differentiate between those that are stage 2 (M0300B) from those that are unstageable suspected deep tissue injury (M0300G) based on a more comprehensive assessment of the resident and ulcer site. Visual observation of the type of fluid in the blister is not enough; you must now do a complete staging assessment that ...

Rich Schick The practice of injecting Benzoin into blisters was a common practice in the Army for some years. It resulted most times in some relief after the initial pain subsided but in some cases it resulted in severe inflammation and prolonged disability. The practice was banned under threat of legal action for anyone caught practicing it. My own favorite technique is to snip a small hole in the blister with a pair of scissors. This creates a fish mouth shaped opening that does not seal shut as easily. Another very effective treatment if you have access to suture supplies (easily obtained through veternary supply catalogs and stores) is to pass a single strand of suture through the blister, tie a knot on both ends to keep it from pulling out, and snip off the excess. This technique has the advantages of being relatively sterile and not allowing air to get to the interior of the blister which makes it less painful. The suture acts as a wick. It will sometimes quit draining in which case you ...

Cold sores are also known as fever blisters. These lesions are caused by the herpes simplex virus type 1 (HSV-1). It is very contagious and transmitted from one person to other by direct contact with infected skin or saliva. They appear as groups of painful, fluid-filled blisters. Cold sores occur mostly in the region of the lips, underneath the nose, or around the chin. Sometimes the HSV-1 infection can cause blisters inside the mouth on the gums and palate (roof of the mouth) as well that may become ulcers.. Initial infection with HSV causes fever, sore gums, sore throat and swollen or tender glands in the neck and it usually occurs in childhood. The first infection appears like any other viral or bacterial throat infections, and is usually not diagnosed as an HSV-1 infection.. If you are once infected by herpes virus (HSV), it stays in the body forever. In some people the virus remains inactive and in others it can cause recurrent attacks of cold sores.
