Low-dose combination therapy as first-line hypertension treatment for blacks and nonblacks. (1/112)

To assess the efficacy and safety of bisoprolol/6.25-mg hydrochlorothiazide (HCTZ), amlodipine, and enalapril in black and nonblack patients, data from two comparative studies were pooled and subgroup analyses performed. Both studies had similar designs and included all three active treatments. The second study also included a placebo group. Subjects (n = 541) with a sitting diastolic blood pressure of 95-114 mmHg were titrated to achieve a diastolic blood pressure < or = 90 mmHg. The studies included 114 blacks and 427 nonblacks. Results of an intention-to-treat analysis of mean change from baseline after 12 weeks of treatment showed the following: 1) blood pressure was significantly lowered by all three active drugs compared with baseline or placebo; 2) in blacks, bisoprolol/6.25-mg HCTZ resulted in significantly greater reductions of systolic and diastolic blood pressure than enalapril or placebo, but was not significantly different from amlodipine; 3) in nonblacks, bisoprolol/6.25-mg HCTZ resulted in significantly greater reduction of diastolic blood pressure than amlodipine, enalapril, or placebo. The placebo-corrected change in blood pressure was greater for blacks than whites on the bisoprolol/6.25-mg HCTZ combination, but this was not statistically significant. Bisoprolol/6.25-mg HCTZ controlled diastolic blood pressure to < or = 90 mmHg in significantly more patients than enalapril or placebo in blacks and nonblacks. The difference in control rates was not significant versus amlodipine. The incidence of drug-related adverse events was similar between treatments; however, bisoprolol/6.25-mg HCTZ had a lower discontinuation rate due to lack of blood pressure control or adverse experiences in both blacks and nonblacks.  (+info)

Heart rate variability and ischaemia in patients with coronary heart disease and stable angina pectoris; influence of drug therapy and prognostic value. TIBBS Investigators Group. Total Ischemic Burden Bisoprolol Study. (2/112)

AIMS: Determination of the influence of therapy with bisoprolol and nifedipine on the heart rate variability of patients from the Total Ischemic Burden Bisoprolol Study and examination of the prognostic value. METHODS AND RESULTS: Four hundred and twenty-two patients with stable angina were included. The heart rate variability was determined over a period of 24 h. Parameters determined: standard deviation of the mean of all corrected RR intervals, standard deviation of all 5 min mean cycle lengths, square root of the mean of the squared differences of successive corrected RR intervals. Nifedipine reduced the mean values of all heart rate variability parameters tested. Square root of the mean of the square differences of successive corrected RR intervals increased under bisoprolol. Standard deviation of the mean of all corrected RR intervals and standard deviation of all 5 min mean cycle lengths increased from low baseline values and declined from higher baseline values. The increase in heart rate variability under therapy was accompanied by a tendency towards a better prognosis. Patients with an increase in heart rate variability and simultaneous complete suppression of ischaemia under therapy displayed no serious events in the course of one year. CONCLUSIONS: The increase in the heart rate variability, which can be regarded as prognostically favourable, was predominantly observed under bisoprolol. The parameter constellation of an increase in heart rate variability and complete ischaemia suppression on the 48-h Holter ECG was associated with the greatest benefit.  (+info)

In vitro and in vivo characterization of intrinsic sympathomimetic activity in normal and heart failure rats. (3/112)

Clinical studies conducted with carvedilol suggest that beta-adrenoceptor antagonism is an effective therapeutic approach to the treatment of heart failure. However, many beta-adrenoceptor antagonists are weak partial agonists and possess significant intrinsic sympathomimetic activity (ISA), which may be problematic in the treatment of heart failure. In the present study, the ISAs of bucindolol, xamoterol, bisoprolol, and carvedilol were evaluated and compared in normal rats [Sprague-Dawley (SD)], in rats with confirmed heart failure [spontaneously hypertensive heart failure (SHHF)], and in isolated neonatal rat cardiomyocytes. At equieffective beta1-adrenolytic doses, the administration of xamoterol and bucindolol produced a prolonged, equieffective, and dose-related increase in heart rate in both pithed SD rats (ED50 = 5 and 40 microgram/kg, respectively) and SHHF rats (ED50 = 6 and 30 microgram/kg, respectively). The maximum effect of both compounds in SHHF rats was approximately 50% of that observed in SD rats. In contrast, carvedilol and bisoprolol had no significant effect on resting heart rate in the pithed SD or SHHF rat. The maximum increase in heart rate elicited by xamoterol and bucindolol was inhibited by treatment with propranolol, carvedilol, and betaxolol (beta1-adrenoceptor antagonist) but not by ICI 118551 (beta2-adrenoceptor antagonist) in neonatal rat. When the beta-adrenoceptor-mediated cAMP response was examined in cardiomyocytes, an identical partial agonist/antagonist response profile was observed for all compounds, demonstrating a strong correlation with the in vivo results. In contrast, GTP-sensitive ligand binding and tissue adenylate cyclase activity were not sensitive methods for detecting beta-adrenoceptor partial agonist activity in the heart. In summary, xamoterol and bucindolol, but not carvedilol and bisoprolol, exhibited direct beta1-adrenoceptor-mediated ISA in normal and heart failure rats.  (+info)

The effect of bisoprolol on perioperative mortality and myocardial infarction in high-risk patients undergoing vascular surgery. Dutch Echocardiographic Cardiac Risk Evaluation Applying Stress Echocardiography Study Group. (4/112)

BACKGROUND: Cardiovascular complications are the most important causes of perioperative morbidity and mortality among patients undergoing major vascular surgery. METHODS: We performed a randomized, multicenter trial to assess the effect of perioperative blockade of beta-adrenergic receptors on the incidence of death from cardiac causes and nonfatal myocardial infarction within 30 days after major vascular surgery in patients at high risk for these events. High-risk patients were identified by the presence of both clinical risk factors and positive results on dobutamine echocardiography. Eligible patients were randomly assigned to receive standard perioperative care or standard care plus perioperative beta-blockade with bisoprolol. RESULTS: A total of 1351 patients were screened, and 846 were found to have one or more cardiac risk factors. Of these 846 patients, 173 had positive results on dobutamine echocardiography. Fifty-nine patients were randomly assigned to receive bisoprolol, and 53 to receive standard care. Fifty-three patients were excluded from randomization because they were already taking a beta-blocker, and eight were excluded because they had extensive wall-motion abnormalities either at rest or during stress testing. Two patients in the bisoprolol group died of cardiac causes (3.4 percent), as compared with nine patients in the standard-care group (17 percent, P=0.02). Nonfatal myocardial infarction occurred in nine patients given standard care only (17 percent) and in none of those given standard care plus bisoprolol (P<0.001). Thus, the primary study end point of death from cardiac causes or nonfatal myocardial infarction occurred in 2 patients in the bisoprolol group (3.4 percent) and 18 patients in the standard-care group (34 percent, P<0.001). CONCLUSIONS: Bisoprolol reduces the perioperative incidence of death from cardiac causes and nonfatal myocardial infarction in high-risk patients who are undergoing major vascular surgery.  (+info)

Effects of ACE inhibition and beta-receptor blockade on energy metabolism in rats postmyocardial infarction. (5/112)

Chronic treatment with beta-receptor blockers or angiotensin-converting enzyme (ACE) inhibitors in heart failure can reduce mortality and improve left ventricular function, but the mechanisms involved in their beneficial action remain to be fully defined. Our hypothesis was that these agents prevent the derangement of cardiac energy metabolism. Rats were subjected to myocardial infarction (MI) or sham operation. Thereafter, animals were treated with bisoprolol, captopril, or remained untreated. Two months later, cardiac function was measured in the isolated heart by a left ventricular balloon (pressure-volume curves), and energy metabolism of residual intact myocardium was analyzed in terms of total and isoenzyme creatine kinase (CK) activity, steady-state levels (ATP, phosphocreatine), and turnover rates (CK reaction velocity) of high-energy phosphates (31P nuclear magnetic resonance) and total creatine content (HPLC). Bisoprolol and partially captopril prevented post-MI hypertrophy and partially prevented left ventricular contractile dysfunction. Residual intact failing myocardium in untreated, infarcted hearts showed a 25% decrease of the total, a 26% decrease of MM-, and a 37% decrease of the mitochondrial CK activity. Total creatine was reduced by 15%, phosphocreatine by 21%, and CK reaction velocity by 41%. Treatment with bisoprolol or captopril largely prevented all of these changes in infarcted hearts. Thus the favorable functional effects of beta-receptor blockers and ACE inhibitors post-MI are accompanied by substantial beneficial effects on cardiac energy metabolism.  (+info)

The effects of verapamil SR and bisoprolol on reducing the sympathetic nervous system's activity. (6/112)

To assess the response of the sympathetic nervous system (SNS) to the handgrip test in essential hypertensive patients and to evaluate the effects of verapamil SR and bisoprolol on the reduction of the SNS's activity. Seventy eight essential hypertensive patients (50 receiving verapamil SR treatment and 28 receiving bisoprolol treatment) took the handgrip test while the SBP, DBP, and HR were measured on three occasions during the test (before test, 3 min after the patients squeezed the handgrip, and 2 min after the handgrip was released). Before and after the patients received Verapamil SR or Bisoprolol treatment, the plasma concentrations of epinephrine(E), norepinephrine (NE), angiotensin-II (AII), aldosterone (ALD), endothelin-1 (ET-1) and renin activity (RA) were measured post-test. 1) In about 70% of the essential hypertensive patients, SNS activity was above normal. Their HR and BP exceeded 20% when responding to stress. 2) In these patients, the baseline plasma concentrations of E, NE, AII, ET-1, ALD, and RA were higher than those whose SNS's activity was normal. 3) After 6 weeks of treatment, all the patients' BPs decreased remarkably. Verapamil SR could reduce the plasma concentrations of NE, AII, and ET-1 and increase RA. Bisoprolol could reduce E and RA. These two antihypertension drugs can both decrease BP and reduce the activity of SNS through different mechanisms.  (+info)

The importance of beta blockers in the treatment of heart failure. (7/112)

Heart failure, the only cardiovascular disease with an increasing incidence, is associated with significant mortality and poses a considerable economic burden. Traditionally, beta blockers have been considered to be contraindicated in patients with heart failure. Recently, however, several large randomized, controlled mortality trials have been stopped early because of significant improvement in mortality rates in patients with heart failure who were given beta blockers in addition to angiotensin-converting enzyme inhibitors, diuretics and, sometimes, digoxin. Beta blockers should now be considered standard therapy in patients with New York Heart Association class II or class III heart failure who are hemodynamically stable, who do not have dyspnea at rest and who have no other contraindications to the use of these agents.  (+info)

Sex differences in the prognosis of congestive heart failure: results from the Cardiac Insufficiency Bisoprolol Study (CIBIS II). (8/112)

BACKGROUND: Whether female sex is associated with a better prognosis in patients with congestive heart failure (CHF) remains uncertain. The Cardiac Insufficiency Bisoprolol Study (CIBIS) II showed that bisoprolol reduced all-cause mortality and morbidity rates in CHF patients treated with diuretics and ACE inhibitors. We examined whether survival was different in men (n=2132) and women (n=515) enrolled in CIBIS II. METHODS AND RESULTS: Women differed from men with regard to age, NYHA functional classification, primary cause of CHF, and risk factors such as left bundle-branch block. After adjustment for baseline differences, the probability of all-cause mortality was significantly reduced by 36% in women compared with that in men (hazard ratio 0.64, 95% CI 0.47 to 0.86, P:=0.003). Women also had a 39% reduction in cardiovascular deaths (hazard ratio 0.64, 95% CI 0.45 to 0.91, P:=0.01) and a 70% reduction in deaths from pump failure (hazard ratio 0.30, 95% CI 0.13 to 0.70, P:=0.005) compared with men. Kaplan-Meier survival analysis revealed a significant reduction in all-cause mortality among women treated with bisoprolol compared with men (6% versus 12% P:=0.01) but not among women treated with placebo (13% versus 18%, P:=0.10). However, this sex/ss-blocker effect was not significant in multivariate analysis. CONCLUSIONS: These results indicate that regardless of ss-blocker treatment and baseline clinical profile, female sex is a significant independent predictor of survival in patients with CHF.  (+info)

Bisoprolol fumarate is a beta1-selective (cardioselective) adrenoceptor blocking agent without significant membrane stabilizing activity or intrinsic sympathomimetic activity in its therapeutic dosage range. Cardioselectivity is not absolute, however, and at higher doses (≥20 mg) bisoprolol fumarate also inhibits beta2-adrenoceptors, chiefly located in the bronchial and vascular musculature; to retain selectivity it is therefore important to use the lowest effective dose. The most prominent effect of bisoprolol fumarate is the negative chronotropic effect, resulting in a reduction in resting and exercise heart rate. There is a fall in resting and exercise cardiac output with little observed change in stroke volume, and only a small increase in right atrial pressure, or pulmonary capillary wedge pressure at rest or during exercise. Findings in short-term clinical hemodynamics studies with bisoprolol fumarate are similar to those observed with other beta-blocking agents. In normal volunteers, ...
Abstract:. The amalgamation of a unique spectrophotometric technique and principle concept of studying variations in pharmaceutical brands as a contrivance for simultaneous determination of the highly marketed combination of Bisoprolol fumarate and Hydrochlorothiazide is established hereby. The commercial purpose of marketing same drugs or drug combinations by different names creates scope for such analysis. Hence an accurate, precise and sensitive Q-Absorbance ratio UV-spectrophotometric method has been developed and validated for the simultaneous estimation of Bisoprolol Fumarate (BF) and Hydrochlorothiazide (HCT) in combined pharmaceutical dosage form. The analysis was performed using 0.1 N NaOH as solvent. Q-Absorbance ratio method involves formation of Q-absorbance equation at two wavelengths 274 nm (max of HCT) and 238.4 nm (iso-absorptive point). Calibration curves were linear in range of 8-96 μg/mL (r2=0.999, 0.999) and 4-48 μg/mL (r2= 0.998, 0.999) for BF and HCT respectively. The ...

ACE inhibitors and β-blockers constitute standard treatment for CHF.9,10 Although it is essential to combine an ACE inhibitor and a β-blocker, even CHF patients treated by physicians experienced in CHF often do not receive this combination, especially not in adequate doses, and many patients remain on the initial drug for extended periods of time.11,12,28 This is one reason why the order of initiating β-blockers and ACE inhibitors may be important. However, with the exception of 2 smaller surrogate end point studies,22,23 the optimum sequence of initiating these agents has not been examined until now. The CIBIS III trial showed that, in patients with NYHA class II or III CHF and impaired LVEF, there was no difference in terms of efficacy and safety between initiation of treatment with bisoprolol followed by enalapril and initiation of treatment with enalapril followed by bisoprolol. In terms of combined mortality and hospitalization, the 2 strategies led to a similar result, with ...

Bisoprolol is a competitive, cardioselective β1-adrenergic antagonist. Activation of β1-receptors (located mainly in the heart) by epinephrine increases heart rate and the blood pressure causing the heart to consume more oxygen. β1-adrenergic blocking agents such as bisopolol lower the heart rate and blood pressure and may be used to reduce workload on the heart and hence oxygen demands. They are routinely prescribed in patients with ischemic heart disease. In addition, β1-selective blockers prevent the release of renin, a hormone produced by the kidneys causes constriction of blood vessels. Bisoprolol is lipophilic and exhibits no intrinsic sympathomimetic activity (ISA) or membrane-stabilizing activity ...

Results: During two-years long supressive therapy we observed a significant rise in left ventricular mass. In woman group left ventricular mass was increased from 168 ± 39 g to 204 ± 45 g (p < 0.001) and in men from 205 ± 60 to 320 ± 21 g. Likewise, left ventricular mass index was increased in women group from 96 ± 18 g/m² to 116 ± 25 g/m² (p < 0.001) and in men group from 107 ± 37 g/m² to 158 ± 28 g/m². Simultaneous treatment with bisoprolol caused a regression of left myocardial hypertrophy. Already after 6 months of simultaneous treatment with L-tyroxin and bisoprolol, for left ventricular mass was reduced to normal: in woman 165 ± 35 g, and in men to 178 ± 38 g. Paul Anton Cibis, M.D. (26 June 1911 - 30 April 1965) was a clinical ophthalmologist, surgeon and pioneer of modern vitreoretinal surgery. As part of Operation Paperclip Cibis came to the United States and performed research for the U.S. Air Force and studied the effects of atomic weapons testing on the eye. He was an internationally recognized expert in retinal detachment surgery and pioneered the use of liquid silicon for this procedure. Cibis was born in 1911 at Rybnik, Silesia, Germany. In 1921, this became part of Poland. Prior to entry into university, he studied in Rybnik and Racibórz Silesia. Cibis then studied medicine at the University of Breslau and at Munich. He attended University of Berlin for medical school. He interned at Berlin and completed residency at Heidelberg. At Heidelberg he met his future wife Lisa, a medical student who became an ophthalmologist. The couple married in 1939. Lisa held membership in the German Ophthalmological Society. Later, they had a daughter, Andrea ...
Bisoprolol, a beta-blocker, blocks certain nerve impulses to various parts of the body, which accounts for its many effects. For example, it reduces the rate and force of the hearts contractions (which helps to lower blood pressure), decreases the hearts oxygen requirement (which helps prevent angina) and helps stabilize heart rhythm. Hydrochlorothiazide (HCTZ), a diuretic, increases the excretion of salt and water in the urine. By reducing the overall amount of fluid in the body, diuretics reduce pressure within the blood vessels ...

In January 2012, I was diagnosed with mild hypertension and began taking BISOPROLOL-HCTZ 2.5-6.25 MG TB. I suffer no side effects at first and tolerated the medicine well, and my blood pressure stayed within acceptable range. And I felt significantly better taking the med. In July of that same year, I went to the beach for a weekend and discovered that I could not tolerate sitting in the sun for more than thirty minutes without feeling terrible.. This response was strange because I was a sun lover and had tanned comfortably since since childhood. Anyway, I had to sit under an umbrella while my friends were frolicking on the beach. While I didnt make the connection between the Bisoprolol and my sensitivity to the sun then, I later read on one of my pharmacys prescription inserts that the medicine could cause sun sensitivity.. After the first year of taking the med (only taking St. Johns Wort and Tramadol at the same time), I began to perspire horribly and have hot flushes, but I assumed that ...
COMPARATIVE ANALYSIS OF THE EFFICACY OF FIXED-DOSE COMBINATIONS OF AMLODIPINE/LISINOPRIL AND BISOPROLOL/HYDROCHLORTHIAZIDE IN PATIENTS WITH ESSENTIAL ARTERIAL HYPERTENSION COMBINED WITH OBESITY AND OVERWEIGHT
The objective of this study was to develop a simpler and more practical quantitative evaluation method of cold flow (CF) in transdermal drug delivery systems (TDDSs). CF was forcibly induced by loading a weight on a punched-out sample (bisoprolol and tulobuterol tapes). When the extent of CF was analyzed using the area of oozed adhesive as following a previously reported method, the CF profiles were looked different between the samples 12 mm in diameter subjected to a 0.5-kg weight and samples 24 mm in diameter subjected to a 2.0-kg weight despite an equal load per unit area (4.42 g/mm2). The width of oozed ...

Bisoprolol in patients with heart failure and moderate to severe chronic obstructive pulmonary disease: a randomized controlled trial.
How to wean off metoprolol 25 mg - the nurturing environment fosters creative ideas; after all, that is how this company was born.
Hello John,. I tried Digoxin together with Flecainide for AF a few years ago. I didnt like it at all and it seemed to make my AF and heart rate even worse. The heart rate went exceedingly high at times. My EP thought it would hold down my heart rates without dropping my blood pressure too much as might a beta blocker. Digoxin is I believe an old med and quite a heavy weight one and levels of the drug can build in the blood stream causing toxicity if periodic checks of Digoxin levels are not carried out. I had to have frequent checks of my Digoxin levels due to poor kidney clearance.. I am now however on Bisoprolol a beta blocker, low dose, which is preventing my heart rates from going so high. It has proved most effective. My EP felt it was a safer option for me because Bisoprolol is a cardio selective beta blocker (targets mainly heart tissue) whereas some beta blockers will target tissue outside of the heart too, for example the lungs, which would not be helpful for those suffering from lung ...

Dear DoctorMy husband is 64 years old and was diagnosed with Chronic Kidney Disease (CKD) about 2-3 years ago. He lost one kidney due to a rugby injury many years ago. He has had a lot of downward swings in his GFR this year for various reasons but the average has been about 27. It is currently on 30. His BP is difficult to control. At the moment he is averaging about 158/87. He is on four medications (2 x 8mg Cardura XL (Doxazosin - Alpha Blocker), 1 x Zanidip 20mg (Lercanidipine Hydrochloride - Calcium Channel Blocker) 1 x Bilocor 10mg (Bisoprolol Fumarate - Beta Blocker) and 1 x Physiotens 0.4mgs (Moxonodine - Central Acting Hypertensive) and has just had a diuretic added (Daptril 2.5mg (Indapamide). He was previously on a potassium sparing diuretic (Spiractalone) and Ramipril and his Potassium went up to 7.8 and so these medications were withdrawn. My question is, he has, for at least the last year, been very breathless after hardly any activity i.e. after having a shower. He has now started ...
Studies reviewed that provide primary data regarding the efficacy and safety of beta-blocker therapy in noncardiac surgery are summarized in Appendix 3. A more detailed discussion of these studies and of systematic reviews and meta-analyses incorporating these data is provided in the sections that follow. Several randomized trials examined the effect of perioperative beta blockers on cardiac events surrounding surgery. Poldermans et al5 examined the effect of bisoprolol on patients undergoing vascular surgery and in patients at high risk for perioperative cardiac complications who were scheduled for vascular surgery. Of 846 patients with risk factors for cardiac disease, 173 were found to have new regional wall-motion abnormalities with stress on dobutamine stress echocardiography. Of these patients, 61 were excluded from further study owing to large areas (5 or more segments) of regional wall-motion abnormalities on dobutamine stress echocardiography or because they were already taking beta ...

Comment: Histologic changes are consistent with left- and right-sided cardiac insufficiency. Histiologic changes were particularly severe in the right ventricle, although I suspect there were left ventricle lesions that were also present but not represented in the sections based on the changes in the lung. Cardiac insufficiency also accounts for the effusions noted in the abdominal cavity and thoracic cavity. This animal also had advanced nephrosclerosis, particularly for its age. I suspect that previous episodes of hypoxia and infraction contributed significantly to these degenerative disease processes. The changes are not characteristic of congenital renal dysplasia or primary infectious process in the kidney. The renal insufficiency may also have contributed to the effusions. An interesting feature in this case is the parasite migration in one of the lymph nodes. This is a rare lesion for a cavy. Parasites are degenerative and could not be positively identified, although they most closely ...
Metoprolol (Lopresor®);Bisoprolol (Monocor®); Carvedilol (Coreg®); Atenolol (Tenormin®); Acebutolol (Sectral®, Monitan®); Propranolol (Inderal®) What are beta-blockers used for? • To prevent chest pain (angina) • To treat high blood pressure • To treat heart failure • To prevent irregular heart rhythms (arrhythmias) • To prevent heart attacks • To treat some symptoms of anxiety In patients with irregular heart rhythms, beta blockers are used as rate control agents, which prevent the heart from beating too fast. Although beta blockers do not make the heart beat at a regular pace, they may help reduce symptoms patients experience with irregular heartbeats. In heart failure, beta-blockers are used to strengthen the heart muscle and help the heart pump more effectively. Why should I take this medication? How do beta-blockers work? What should I expect? Do β-blockers improve morbidity and mortality in patients with heart failure? 3 large trials have shown the benefits of bisoprolol (1), carvedilol (2), and metoprolol (3) on morbidity and mortality among patients with NYHA class II or III heart failure and have had important implications for how patients with mild-to-moderate heart failure are treated. On the basis of these studies, it has now become standard practice to treat such patients with one of these β-blockers. Whether patients with severe heart failure would also benefit from β-blockade has been unanswered. Patients with severe heart failure have the highest sympathetic outflow and theoretically may benefit most from β-blockade. These patients also have the least inotropic reserve and thus are most susceptible to decompensation when treated with β-blockade. These 2 studies (BEST and COPERNICUS) have provided important data that allow us to assess the benefit of β-blockers in patients with severe heart failure. What is certain is ...

In this article, we will discuss the NYHA (New York Heart Association) functional classification of Heart Failure. So, lets get started.. Classification. Class I (Asymptomatic): No limitation on physical activity. Ordinary physical activity does not cause symptoms of Heart Failure (Patient with cardiac disease but no symptoms).. Class II (Symptomatic with moderate activity): Patients with cardiac disease are comfortable at rest and after mild activity, but moderate activity results in dyspnea, palpitation, and fatigue.. Class III (Symptomatic with mild activity): Patients with cardiac disease have marked limitation of physical activity. Comfortable at rest, but less than normal activity results in fatigue, dyspnea, and palpitation.. Class IV (Symptomatic at rest): Patients have symptoms of cardiac insufficiency at rest, and any physical activity further increases the discomfort. They are unable to carry out any physical activity without discomfort.. ...
Background. Sex differences in LV mechanics exist at rest and during acute physiological stress. Differences in cardiac autonomic and adrenergic control may contribute to sex differences in LV mechanics and LV hemodynamics. Accordingly, this study aimed to investigate sex differences in LV mechanics with altered adrenergic stimulation achieved through post handgrip exercise ischemia (PEI) and ß1-adrenergic receptor (AR) blockade. Methods and Results. 20 males (23±5 yr) and 20 females (22±3 yr) were specifically matched for LV length (males: 8.5±0.5 cm, females: 8.2±0.6 cm, p=0.163), and 2-dimensional speckle-tracking echocardiography was used to assess LV structure and function at baseline, during PEI and following administration of 5mg bisoprolol (ß1-AR antagonist). During PEI, LV end-diastolic volume and stroke volume were increased in both groups (p,0.001), as was end-systolic wall stress (p,0.001). LV twist and apical rotation werent altered from baseline or different between the ...

Cardiovascular research belongs to the most rapidly growing biomedical disciplines as it deals predominantly with diseases that are the leading cause of death in industrial countries. These include, but are not limited to coronary heart disease, cardiomyopathy, hypertension and arterial disease, heart hypertrophy, heart failure, arteriosclerosis, stroke, peripheral arterial disease and varicosis. The focus of my group is on the investigation of biomechanically triggered mechanisms which either promote cardiovascular diseases or at least partially compensate their harmful effects. Therefore, we are especially interested in the analysis of cardiac insufficiency, development of arteriosclerotic lesions, hypertension-induced arterial remodeling, varicose vein formation as well as arteriogenesis, the formation of collateral arteries from preexisting arterioles to compensate the occlusion of a conduit artery. All of these pathophysiological processes are basically associated with the adaptive or ...

To analyze the impact of multimorbidity (MM) on health care costs taking into account data heterogeneity. Data come from a multicenter prospective cohort study of 1,050 randomly selected primary care patients aged 65 to 85 years suffering from MM in Germany. MM was defined as co-occurrence of ≥3 conditions from a list of 29 chronic diseases. A conditional inference tree (CTREE) algorithm was used to detect the underlying structure and most influential variables on costs of inpatient care, outpatient care, medications as well as formal and informal nursing care. Irrespective of the number and combination of co-morbidities, a limited number of factors influential on costs were detected. Parkinsons disease (PD) and cardiac insufficiency (CI) were the most influential variables for total costs. Compared to patients not suffering from any of the two conditions, PD increases predicted mean total costs 3.5-fold to approximately € 11,000 per 6 months, and CI two-fold to approximately € 6,100. Background. Cardiac insufficiency is one of the main causes of morbidity and mortality in the world. Objective. The aim of the study was to determine the content of interleukins in male rats with adrenaline-calcium model of myocardial injury (ACM) and under correction by quercetin. Methods. 143 Wistar male-rats aged 5-6 months were used in the experiments. The changes of interleukins (IL-1beta, IL-2, IL-4, IL-6, IL-10, TNF-alpha) content in blood serum in the development of heart injury by adrenaline and calcium (AC) and use of quercetin for protection were studied. The period of investigation was in 1, 2, 24 hours, 3, 7, 14, 21, 28 days. Results. In ACM model of myocardial injury TNF-alpha increased, IL-2, IL-4 decreased at each period of investigation. IL-1beta increased in 2 and 3 days after the injury, at all other periods (except in 1 hour and 21 days) those indices were decreased. Also we observed similar wave changes of IL-6 and IL-10 content. Injections of quercetin for 7 days caused a
Increased susceptibility to bacterial infection is common in patients with multiple myeloma and infections can be life-threatening if not addressed. Immunodeficiency can be secondary to myelophthisis (which results in leukopenia), decreased production of functional immunoglobulin, and compromised B cell function.. Clinical signs and symptoms may be present for up to 1 year before a definitive diagnosis of multiple myeloma is made. Patients can also present with recurrent infections, non-regenerative anemia, pathologic bone fractures, and/or seizures. Complications secondary to multiple myeloma may include renal failure, infections secondary to immunosuppression, clotting disorders, chronic anemia, cardiac insufficiency, and neurologic dysfunctions such as senility.. A diagnosis of multiple myeloma may be made if there is radiographic evidence of osteolysis, there is a population of greater than 20% plasma cells in bone marrow aspirates or biopsies, a monoclonal gammopathy on serum ...

The material in intelligent wound dressings is a mixture of agarose and synthetic lipid vesicles that contain a self-quenching fluorescent dye. When present at high concentrations inside the vesicles, the dye is quenched and does not fluoresce. The lipid vesicles holding the dye are sensitive to cytotoxic virulence factors produced by major wound pathogens. When virulence factors from bacteria in an infected wound reach clinically relevant levels, the vesicles are lysed and the dye is dispersed. Once released, the dye is no longer at a high enough concentration to self-quench, and a fluorescence color change becomes visible. This color change can be detected through vision alone and thus is a simple yet powerful indication of clinical colonization and the need for immediate treatment. At left, the detergent Triton is used to fully lyse the vesicles, and the activated dye is visible under UV light.. The virulence factors that interact with the wound dressing occur in bacterial biofilms. ...
Hindsight is always 20/20, especially in the field of science. Given what we know now, it seems crazy that people used to think the world was flat. The realm of toxicology is filled with similar stories (see pregnancy-boosting DES and super-insecticide DDT). In the mid-twentieth century America, realization of the harmful effects of chemicals like DES and DDT contributed to rising concern about potential risks to human and environmental health associated with chemicals. Consequently, in efforts to better monitor and understand what chemicals may be harmful to humans and the environment, Congress enacted the Toxic Substance Control Act in 1976, enabling the US Environmental Protection Agency (EPA) to inventory and regulate chemicals being sold or manufactured in the US. As of 2014, 84,000 chemicals are in use in the United States (EPA TSCA).. However, even 39 years later, we know very little about the potentially harmful effects on human health and the environment of a majority of these ...
