Resistance training affects GLUT-4 content in skeletal muscle of humans after 19 days of head-down bed rest. (1/330)

This study assessed the effects of inactivity on GLUT-4 content of human skeletal muscle and evaluated resistance training as a countermeasure to inactivity-related changes in GLUT-4 content in skeletal muscle. Nine young men participated in the study. For 19 days, four control subjects remained in a -6 degrees head-down tilt at all times throughout bed rest, except for showering every other day. Five training group subjects also remained at bed rest, except during resistance training once in the morning. The resistance training consisted of 30 isometric maximal voluntary contractions for 3 s each; leg-press exercise was used to recruit the extensor muscles of the ankle, knee, and hip. Pauses (3 s) were allowed between bouts of maximal contraction. Muscle biopsy samples were obtained from the lateral aspect of vastus lateralis (VL) muscle before and after the bed rest. GLUT-4 content in VL muscle of the control group was significantly decreased after bed rest (473 +/- 48 vs. 398 +/- 66 counts. min-1. microgram membrane protein-1, before and after bed rest, respectively), whereas GLUT-4 significantly increased in the training group with bed rest (510 +/- 158 vs. 663 +/- 189 counts. min-1. microgram membrane protein-1, before and after bed rest, respectively). The present study demonstrated that GLUT-4 in VL muscle decreased by approximately 16% after 19 days of bed rest, and isometric resistance training during bed rest induced a 30% increase above the value of GLUT-4 before bed rest.  (+info)

A five-year assessment of controlled trials of in-patient and out-patient treatment and of plaster-of-Paris jackets for tuberculosis of the spine in children on standard chemotherapy. Studies in Masan and Pusan, Korea. Fifth report of the Medical Research Council Working Party on tuberculosis of the spine. (2/330)

In two centres in Korea 350 patients with a diagnosis of tuberculosis of the thoracic and/or lumbar spine were allocated at random: in Masan to in-patient rest in bed (IP) for six months followed by out-patient treatment or to ambulatory out-patient treatment (OP) from the start; in Pusan to out-patient treatment with a plaster-of-Paris jacket (J) for nine months or to ambulatory treatment without any support (No J). All patients recieved chemotherapy with PAS with isoniazid for eighteen months, either supplemented with streptomycin for the first three months (SPH) or without this supplement (PH), by random allocation. The main analysis of this report concerns 299 patients (eighty-three IP, eighty-three OP, sixty-three J, seventy No J; 143 SPH, 156 PH). Pre-treatment factors were similar in both centres except that the patients in Pusan had, on average, less extensive lesions although in a greater proportion the disease was radiographically active. One patient (J/SPH) died with active spinal disease and three (all No J/SPH) with paraplegia. A fifth patient (IP/PH) who died from cardio respiratory failure also had pulmonary tuberculosis. Twenty-three patients required operation and/or additional chemotherapy for the spinal lesion. A sinus or clinically evident abscess was either present initially or developed during treatment in 41 per cent of patients. Residual lesions persisted in ten patients (four IP, two OP, one J, three No J; six SPH, four PH) at five years. Thirty-two patients had paraparesis on admission or developing later. Complete resolution occurred in twenty on the allocated regimen and in eight after operation or additional chemotherapy or both. Of the remaining four atients, all of whom had operation and additional chemotherapy, three died and one still had paraparesis at five years. Of 295 patients assessed at five years 89 per cent had a favourable status. The proportions of the patients responding favourably were similar in the IP (91 per cent) and OP (89 per cent) series, in the J (90 per cent) and No J (84 per cent) series and in the SPH (86 per cent) and PH (92 per cent) series.  (+info)

Energy expenditure and balance during spaceflight on the space shuttle. (3/330)

The objectives of this study were as follows: 1) to measure human energy expenditure (EE) during spaceflight on a shuttle mission by using the doubly labeled water (DLW) method; 2) to determine whether the astronauts were in negative energy balance during spaceflight; 3) to use the comparison of change in body fat as measured by the intake DLW EE, 18O dilution, and dual energy X-ray absorptiometry (DEXA) to validate the DLW method for spaceflight; and 4) to compare EE during spaceflight against that found with bed rest. Two experiments were conducted: a flight experiment (n = 4) on the 16-day 1996 life and microgravity sciences shuttle mission and a 6 degrees head-down tilt bed rest study with controlled dietary intake (n = 8). The bed rest study was designed to simulate the flight experiment and included exercise. Two EE determinations were done before flight (bed rest), during flight (bed rest), and after flight (recovery). Energy intake and N balance were monitored for the entire period. Results were that body weight, water, fat, and energy balance were unchanged with bed rest. For the flight experiment, decreases in weight (2.6 +/- 0.4 kg, P < 0.05) and N retention (-2. 37 +/- 0.45 g N/day, P < 0.05) were found. Dietary intake for the four astronauts was reduced in flight (3,025 +/- 180 vs. 1,943 +/- 179 kcal/day, P < 0.05). EE in flight was 3,320 +/- 155 kcal/day, resulting in a negative energy balance of 1,355 +/- 80 kcal/day (-15. 7 +/- 1.0 kcal. kg-1. day-1, P < 0.05). This corresponded to a loss of 2.1 +/- 0.4 kg body fat, which was within experimental error of the fat loss determined by 18O dilution (-1.4 +/- 0.5 kg) and DEXA (-2.4 +/- 0.4 kg). All three methods showed no change in body fat with bed rest. In conclusion, 1) the DLW method for measuring EE during spaceflight is valid, 2) the astronauts were in severe negative energy balance and oxidized body fat, and 3) in-flight energy (E) requirements can be predicted from the equation: E = 1.40 x resting metabolic rate + exercise.  (+info)

Low-dose T(3) improves the bed rest model of simulated weightlessness in men and women. (4/330)

This study tested the hypothesis that low-dose 3,5, 3'-triiodothyronine (T(3)) administration during prolonged bed rest improves the ground-based model of spaceflight. Nine men (36.4 +/- 1. 3 yr) and five women (34.2 +/- 2.1 yr) were studied. After a 5-day inpatient baseline period, subjects were placed at total bed rest with 6 degrees head-down tilt for 28 days followed by 5-day recovery. Fifty micrograms per day of T(3) (n = 8) or placebo (n = 6) were given during bed rest. Serum T(3) concentrations increased twofold, whereas thyroid-stimulating hormone was suppressed in treated subjects. T(3)-treated subjects showed significantly greater negative nitrogen balance and lost more weight (P = 0.02) and lean mass (P < 0.0001) than placebo subjects. Protein breakdown (whole body [(13)C]leucine kinetics) increased 31% in the T(3) group but only 8% in the placebo group. T(3)-treated women experienced greater changes in leucine turnover than men, despite equivalent weight loss. Insulin sensitivity fell by 50% during bed rest in all subjects (P = 0.005), but growth hormone release and insulin release were largely unaffected. In conclusion, addition of low-dose T(3) to the bed rest model of muscle unloading improves the ground-based simulation of spaceflight and unmasks several important gender differences.  (+info)

Serum levels of hyaluronan, antigenic keratan sulfate, matrix metalloproteinase 3, and tissue inhibitor of metalloproteinases 1 change predictably in rheumatoid arthritis patients who have begun activity after a night of bed rest. (5/330)

OBJECTIVE: To evaluate whether and how moderate physical activity following a night of rest influences serum levels of matrix metalloproteinase 3 (MMP-3), tissue inhibitor of metalloproteinases 1 (TIMP-1), antigenic keratan sulfate (Ag KS), and hyaluronan (HA) in 10 normal subjects and 38 patients with rheumatoid arthritis (RA). METHODS: Blood was obtained from 20 RA patients before they arose from a night's sleep, and again 1 and 4 hours after they had begun to perform moderate physical activity. Another 18 RA patients remained in bed and blood was sampled at the same time periods. Serum levels of MMP-3, TIMP-1, Ag KS, and HA were measured by enzyme-linked immunosorbent assay. Clinical activity was evaluated by the Lansbury index. RESULTS: Both in normal subjects and in RA patients who did not remain in bed throughout the period of blood sampling, levels of HA, Ag KS, and MMP-3 increased significantly during the first hour after the subjects arose: the increase in HA and Ag KS correlated with the Lansbury index in the RA group. Three hours later, levels of Ag KS had dropped to baseline values in both groups of subjects. Levels of HA remained significantly and moderately elevated in the RA group but not in the control group, while levels of MMP-3 did not drop significantly in either group. In contrast, levels of HA, Ag KS, and MMP-3 did not change significantly in RA patients who had remained in bed. Unlike the other markers, the levels of TIMP-1 remained unchanged at the different time periods in all 3 groups studied. CONCLUSION: Significant changes in serum levels of some metabolic markers occur during the first hour after one arises from a night of sleep, especially in patients with RA. Measurement of the magnitude of these changes at different times in individual patients provides very different information about metabolic changes occurring in joint tissue than does measurement of the level of the markers at a single time point, as is usually currently reported.  (+info)

Contributions of MSNA and stroke volume to orthostatic intolerance following bed rest. (6/330)

We examined whether the altered orthostatic tolerance following 14 days of head-down tilt bed rest (HDBR) was related to inadequate sympathetic outflow or to excessive reductions in cardiac output during a 10- to 15-min head-up tilt (HUT) test. Heart rate, blood pressure (BP, Finapres), muscle sympathetic nerve activity (MSNA, microneurography), and stroke volume blood velocity (SVV, Doppler ultrasound) were assessed during supine 30 degrees (5 min) and 60 degrees (5-10 min) HUT positions in 15 individuals who successfully completed the pre-HDBR test without evidence of orthostatic intolerance. Subjects were classified as being orthostatically tolerant (OT, n = 9) or intolerant (OI, n = 6) following the post-HDBR test. MSNA, BP, and SVV during supine and HUT postures were not altered in the OT group. Hypotension during 60 degrees HUT in the post-bed rest test for the OI group (P < 0.05) was associated with a blunted increase in MSNA (P < 0.05). SVV was reduced following HDBR in the OI group (main effect of HDBR, P < 0.02). The data support the hypothesis that bed rest-induced orthostatic intolerance is related to an inadequate increase in sympathetic discharge that cannot compensate for a greater postural reduction in stroke volume.  (+info)

Increase in epinephrine-induced responsiveness during microgravity simulated by head-down bed rest in humans. (7/330)

The epinephrine (Epi)-induced effects on the sympathetic nervous system (SNS) and metabolic functions were studied in men before and during a decrease in SNS activity achieved through simulated microgravity. Epi was infused at 3 graded rates (0.01, 0.02, and 0. 03 microg. kg(-1). min(-1) for 40 min each) before and on the fifth day of head-down bed rest (HDBR). The effects of Epi on the SNS (assessed by plasma norepinephrine levels and spectral analysis of systolic blood pressure and heart rate variability), on plasma levels of glycerol, nonesterified fatty acids (NEFA), glucose and insulin, and on energy expenditure were evaluated. HDBR decreased urinary norepinephrine excretion (28.1 +/- 4.2 vs. 51.5 +/- 9.1 microg/24 h) and spectral variability of systolic blood pressure in the midfrequency range (16.3 +/- 1.9 vs. 24.5 +/- 0.9 normalized units). Epi increased norepinephrine plasma levels (P < 0.01) and spectral variability of systolic blood pressure (P < 0.009) during, but not before, HDBR. No modification of Epi-induced changes in heart rate and systolic and diastolic blood pressures were observed during HDBR. Epi increased plasma glucose, insulin, and NEFA levels before and during HDBR. During HDBR, the Epi-induced increase in plasma glycerol and lactate levels was more pronounced than before HDBR (P < 0.005 and P < 0.001, respectively). Epi-induced energy expenditure was higher during HDBR (P < 0.02). Our data suggest that the increased effects of Epi during simulated microgravity could be related to both the increased SNS response to Epi infusion and/or to the beta-adrenergic receptor sensitization of end organs, particularly in adipose tissue and skeletal muscle.  (+info)

Myonuclear domain and myosin phenotype in human soleus after bed rest with or without loading. (8/330)

After 2 or 4 mo of bed rest (6 degrees head-down tilt) and 1 mo of ambulation, there was a tendency toward a higher percentage of fibers expressing fast myosin heavy chain (MHC) isoforms and a de novo appearance of fibers coexpressing type I+IIa+IIx and IIa+IIx MHC in human soleus fibers. After 2 and 4 mo of bed rest, the mean size of type I fibers decreased by 12 (P > 0.05) and 39%, respectively. Because myonuclear number/mm of fiber length was unchanged, myonuclear domain was smaller after bed rest than before. The mean size and myonuclear domain of type I fibers were largest after 1 mo of recovery. The effects of wearing an antigravity device (Penguin suit), which had a modest but continuous resistance at the knee and ankle (Penguin-1) or knee resistance without loading on the ankle (Penguin-2), for 10 consecutive h/day were determined during 2 mo of bed rest. Mean fiber sizes in Penguin-1, but not Penguin-2, group were maintained at or above pre-bed-rest levels, whereas neither group showed phenotype changes. Myonuclear domain in type I fibers was larger in Penguin-1 and smaller in Penguin-2 group post- compared with pre-bed rest, indicating that a single daily 10-h bout of modest muscle loading can prevent bed-rest-induced soleus fiber atrophy but has minimal effect on myosin phenotype. The specific adaptive cellular strategies involved may be a function of the duration and magnitude of the adaptive stimulus as well as the immediate activity history of the fiber before the newly changed functional demands.  (+info)

For other patients, it may be appropriate to have strict bed rest until the stability of columna is assessed. The need of strict bed rest is assessed by a physician based on the risk of increased neurological deficits and the degree of pain. When columna is considered stable enough (usually clarified 2 to 4 days after the initiation of radiotherapy), gradually mobilization until pain threshold should quickly get started. Increasing pain or neurological deficits should be observed during mobilization ...
Doctors have often counseled women to go on bed rest following their in vitro fertilization (IVF) procedure, the theory being that it makes the womans body more likely to accept the fertilized egg. This practice may no longer be advised, however, as a new study has found that despite being common practice, [bed rest] did not help women ultimately have a baby.. Too much rest may be harmful. Dr. Jani Jensen, a fertility expert with the Mayo Clinic, said this should be proof that you dont need bed rest in any way, shape, or form in order to conceive. Previously, many doctors offices suggested women remain prone for at least 10 minutes before leaving the clinic after IVF. Other offices took the idea to an extreme, requiring hours or even days of bed rest. Previous studies, however, actually have found that extended bed rest might actually hurt a womans chances of delivering a full-term, healthy baby.. Women without bed rest have higher birth rates. The study looked at 240 women who were ...
In a review published in the journal Lancet, authors evaluated 39 different studies on bed rest prescribed for 15 different conditions, involving a total of 5,777 patients. Results of the analysis revealed that bed rest was not an effective treatment recommendation. In 24 trials investigating bed rest following a medical procedure, no patients improved significantly and eight worsened significantly. And in 15 trials investigating bed rest as a primary treatment, no patients improved significantly, while nine worsened significantly. Healing involves much more than rest, a fact which this study seems to emphasize but which many doctors seem to ignore. Don¹t settle for quick medical advice (pills, bed rest, etc.) that may end up doing more harm than good, or no good at all. Consult with your doctor of chiropractic for information on active, noninvasive approaches to managing a variety of health problems. Reference:. Allen C, Glasziou P, Del Mar C. Bed rest is an inherent part of treatment for acute myocardial infarction (AMI). In clinical practice this intervention is prescribed in different ways and for different lengths of time. Current guidelines recommend at least 12 hours bed rest following AMI. However the basis for these recommendations is unclear. This review found 15 trials which were generally outdated and of moderate to poor methodological quality. Bed rest ranging from 2 to 12 days appears to be as safe as longer periods of bed rest. No evidence was found to support the current bed rest recommendations of not more than 12 to 24 hours. The optimal duration of bed rest after AMI remains undetermined from the available evidence.. ...

In chronic diseases, hypoxia and physical inactivity are associated with atherosclerosis progression. In contrast, a lower mortality from coronary artery disease and stroke is observed in healthy humans residing at high altitude in hypoxic environments. Eleven young, male volunteers completed the following 10-day campaigns in a randomized order: hypoxic ambulatory, hypoxic bed rest and normoxic bed rest. Before intervention, subjects were evaluated in normoxic ambulatory condition. Normobaric hypoxia was achieved in a hypoxic facility simulating 4000 m of altitude. Following hypoxia, either in bed rest or ambulatory condition, markers of cardiometabolic risk shifted toward a more atherogenic pattern consisting of: (a) lower levels of total HDL cholesterol and HDL2 sub-fraction and decreased hepatic lipase; (b) activation of systemic inflammation, as determined by C-reactive protein and serum amyloid A; (c) increased plasma homocysteine; (d) decreased delta-5 desaturase index in cell membrane fatty acids
As part of the 2nd Berlin BedRest Study (BBR2-2), we investigated the pattern of muscle atrophy of the postero-lateral hip and hamstring musculature during prolonged inactivity and the effectiveness of two exercise countermeasures. Twenty-four male subjects underwent 60 days of head-down tilt bed-rest and were assigned to an inactive control (CTR), resistive vibration exercise (RVE) or resistive exercise alone (RE) group. Magnetic resonance imaging (MRI) of the hip and thigh was taken prior to, during and at end bed-rest. Volume of posterolateral hip and hamstring musculature was calculated and the rate of muscle atrophy and effect of countermeasure exercises examined. After 60 days bed-rest, the CTR group showed differential rates of muscle volume loss (F=21.44, ...
I have read a few articles that say that the benefit of bed rest has not been proven with preeclampsia, but obviously your case is a little more complicated with your strong family history of placental abruption. My OB was quick to take me off work at 29 weeks with both pregnancies, and I was on strict bed rest with my first. I do believe that bed rest benefited me with my first pregnancy. In my opinion, the improvement in your condition is relatively good proof that bed rest is a good option for you, but Im obviously not a doctor:) Its amazing how doctors opinions differ so much ...

When youre pregnant, a prescription to stay in bed might seem like a welcome break. In reality, however, restrictions on movement during pregnancy can pose challenges and even certain health risks. Heres what you need to know.. Bed rest during pregnancy is no longer recommended for most conditions. While bed rest increases blood flow to the placenta, there is no evidence that it decreases the risk of premature birth.. In the rare situations when bed rest is recommended, it is prescribed at varying levels of activity restriction. In some cases, it means decreasing your activity level for a period of time. You might be free to move about the house, as long as you avoid lifting children and doing heavy housework. You might even be able to continue working.. In other cases, bed rest guidelines are stricter. You might need to remain in a sitting or reclining position, only getting up to use the toilet or shower. You might not be allowed to work or do even light household chores until the baby is ...

During spaceflight, skeletal unloading results in loss of bone mineral density (BMD). This occurs primarily in the spine and lower body regions. This loss of skeletal mass could prove hazardous to astronauts on flights of long duration. In this study, intense resistance exercise was used to test whether a training regimen would prevent the loss of BMD that accompanies disuse. Nine subjects (5 men, 4 women) participated in a supine maximal resistance exercise training program during 17 wk of horizontal bed rest. These subjects were compared with 18 control subjects (13 men, 5 women) who followed the same bed rest protocol without exercise. Determination of treatment effect was based on measures of BMD, bone metabolism markers, and calcium balance obtained before, during, and after bed rest. Exercisers and controls had significantly (P , 0.05) different means, represented by the respective following percent changes:. lumbar spine BMD, +3% vs. -1 ...
Thats right folks, its that time again. My blood pressure has been a little elevated for a couple of weeks now, but it spiked pretty high today. After seeing the on-call doctor this afternoon, monitoring the baby, and doing lab work, I was told to increase my medication and take it easy. Take it easy translates into bed rest. So for now it isnt strict bed rest. I just have to take it easy, laying around as much as possible...no cooking, cleaning, or shopping. Sounds like strict bed rest to me! Well, she did tell me that if I had to I could pick up and take the girls to school. Actually she said that realistically, complete bed rest would probably be impossible with three children. I appreciated the fact that she took that into consideration. All she kept saying was do as little as possible, call in favors from friends, have my husband help, and so forth. The husband helping isnt a problem. He is really great about helping with the kids, doing laundry, picking up the house, etc. Just so ...

Expectant moms can become impatient during the nine month wait to meet their newborn, but imagine if you spent twenty-six weeks (thats 182 days!) of your pregnancy on bed rest. Thats what happened to Stephanie Johnson in late 2010 when her doctor ordered immediate and prolonged bed rest after complications with her first pregnancy. My life stopped when everyone elses continued, said Johnson in a recent interview with Bethenny Frankel. That left her brainstorming proactive solutions for other pregnant women in similar circumstances.. Enter Bed Rest Concierge. A mere three months after giving birth to her daughter Harlow in March of 2011, Stephanie started her business - a resource for resting moms in need of motivation and support. The focus, she claims, is to help women have a beautiful and joyful pregnancy while preparing for their new arrival.. Johnson and her team offer a variety of pre-delivery services including nursery design, baby shower planning and laundry service. (A mobile ...
Climbing Kilimanjaro or bed rest, which one is harder, a friend asked me recently. Both take mental strength, but I didnt have to run and lift weights for nine months to prepare for bed rest. Assumption: Bed Rest will prevent premature birth. FALSE Although more studies are needed, current findings show no evidence that bed rest prevents premature birth. http://onlinelibrary.wiley.com/doi/10.1002/14651858.CD003581.pub2/abstract My Views: My suspicion is that bed rest per se is not going to prevent premature birth. However, I wonder if stress might contribute to the problem. Some women may…

Four will spend 21 days in bed while engaging in recumbent weight-training and a vibrating exercise contraption. Another group of four will engage in the same, but add whey protein to their diets to see if there is any difference in muscle building. A final four will spend the same amount of time in bed, but lacking any of these attempts at countermeasures ...
To elucidate the molecular mechanisms behind physical inactivity-induced insulin resistance in skeletal muscle, 12 young, healthy male subjects completed 7 days of bed rest with vastus lateralis muscle biopsies obtained before and after. In six of the subjects, muscle biopsies were taken from both legs before and after a 3-h hyperinsulinemic euglycemic clamp performed 3 h after a 45-min, one-legged exercise. Blood samples were obtained from one femoral artery and both femoral veins before and during the clamp. Glucose infusion rate and leg glucose extraction during the clamp were lower after than before bed rest. This bed rest-induced insulin resistance occurred together with reduced muscle GLUT4, hexokinase II, protein kinase B/Akt1, and Akt2 protein level, and a tendency for reduced 3-hydroxyacyl-CoA dehydrogenase activity. The ability of insulin to phosphorylate Akt and activate glycogen synthase (GS) was reduced with normal GS site 3 but abnormal GS site 2+2a phosphorylation after bed rest. ...

A survey from Australia suggests that the common practice of keeping patients in bed may be not only not helpful, but actually harmful. Yesterday I went to see my OB and it has happened....the dreaded bed rest! With roughly 20 days to go she put me on bed rest. YUCK!! I complained to my Mom and she just said Enjoy it now honey because in a month you will wish you could sleep and just hang out! Isnt that the truth! I also had an ultrasound yesterday and the boys are doing great and growing like crazy. Baby A is roughly 4lbs and 6 oz and Baby B is 5lbs even. Its hard to believe that in a few short weeks they will be here and we will be tired! We are excited and ready....I think ...

The present study extends our previous results obtained during a 90-day bed rest in men (9) to women. Extreme physical inactivity, independent of changes in energy balance and macronutrient intake, induces hypertriglyceridemia, a decrease in insulin sensitivity, and a decrease in total lipid oxidation in favor of an increase in total carbohydrate oxidation in both fasted and fed states. As in men, inactivity decreases saturated but not monounsaturated fatty acid oxidation. The present study suggests some mechanisms involved in these metabolic alterations.. Bed rest induces an increase in plasma triglyceride concentrations, which may be due to a greater lipid absorption in enterocytes and/or a lower clearance of triglycerides. The head-down position used to simulate microgravity may induce a larger blood volume in the visceral areas (29), leading to greater macronutrient absorption. Our data suggest that hydrolysis of triglyceride-rich lipoproteins by LPL may be reduced by bed rest, which is ...
I woke up this morning, hoping to be productive since I definitely was not productive yesterday. As soon as I got out of bed to make breakfast my body said Excuse me, but I dont think so. Yes, my body talks to me and it can be quite rude! By the time I dragged my breakfast and myself up the stairs again, I knew I would have to give into my worst enemy. Bed rest. Ugh, the thought of it makes me feel like a lazy, worthless slug. So I flopped myself into bed and resigned myself to the fact that I would be spending the rest of my day under the covers. For most people this sounds like the most delightful day, relaxing in bed, watching TV, and basically doing nothing. Its hard for me because Im trying to get my life back and for every step forward I take, I end up taking two steps back ...
Getting Help. My surgeons did a great job of putting my parts in the right places and sewing me up, but their advice on how to cope with weeks without sitting or bending was along the lines of Youre smart, youll figure it out, and Its tough. However, just because Im good at math doesnt mean that I had any clue about how to get out of bed without bending at the waist. I asked for more help.. A physical therapist and an occupational therapist visited me in the hospital after surgery and made managing the bed rest universe seem plausible. The OT gave me a hip kit (most often used by people whove had hip replacement surgery) with tools for bend-free living: a grabber to pick up things, a lollipop sponge on a stick for bathing, elastic shoe laces and a long shoehorn so I could put on my shoes myself, and a device to help me put on my socks. We went over the basics of dressing, bathing, and using the bathroom. The PT taught me how to get in and out of bed and how to roll over, and we ...
Im 22 weeks 1 day and I have gained 12 lbs this pregnancy. Its my first and I have been on some form of bed rest since 6 weeks and Ive also been on strict bed rest for the past month. Im not back on modified but I cant stand too long or anything. The doctor said something to me today about not drinking any beverages other than water and especially no juice. Ive been eating healthy, I so snack, but I try to eat lots of salads. It seems like everything has too much sugar: strawberries and cherries, everything! So, I guess what my question is: In your opinion, have I gained too much weight so far? My doctor said he wouldnt say that I have gained too much but the past couple of weeks Ive gained two pounds each time. And is a glass of juice or soda really that horrible? I know this is such a silly question and I apologize for asking such a non life threatening question and taking up your time, but I like getting multiple opinions on things pregnancy related. Thank you for your time!
One of my favorite suggestions given by the author is to create pause in our days. Even if we cant or wont change the entire pace of our day, we can build in the little space of time to stop and exhale. Creating a broader margin in our days can take the form of arriving at piano lessons ten minutes early and sitting under a tree to watch the clouds. Or someone suggests tea before bed and the family gathers around the table, lights a candle, and drinks tea in the evening. As I neared the end of my bed rest, I renewed my personal commitment to leave some space around the edges of my days and build the margins to keep my days from being inscribed too densely.. The second big gift I received was from the amazing people around me who displayed how to truly help another person. The most comforting, supportive words that I heard were How can I help? What a different ring that has than Let me know if theres anything I can do. When someone said, How can I help? and then waited for an answer, ...

Gunnar actually left me a Facebook message! this is third he has left me in the time he has been gone..(about four months since the last message left). It was very sweet and good of him. I have been hounding him for months now to write me letter, I am getting desperate for some hubby love- so I resorted to say- just leave me a note of Facebook, or shoot me an e-mail- I sometimes need more than an hour total of my husband per week. Is it bad, though he is very sweet and loving in his message, that I almost take it all with a grain of salt-- ya sure you love me oh so very much, but you cant write me a stupid letter???...even when I am stuck on bed rest??! ah, maybe we will have some issues to work through... well, throw them onto the pile- it is going to be party when he gets back ...
Cue Sunday morning, 2am. Im awakened by severe cramping so bad that I curl up in the fetal position and clutch the sheets. I felt almost nauseous because of the pain. It didnt subside after about a half hour, so I finally woke Jeremy up and we decided to go to the hospital. Id actually made a tiny bit of progress since my OB appointment just five days ago; at that time, I was still closed up, but now they were telling me I was 1 cm dilated and 50% effaced. I was given a shot of Terbutaline to stop the contractions. It slowed them down significantly and after two hours of monitoring, I was sent home. I saw my OB the next day and got my official orders for bed rest ...

Insulin resistance after surgery has been shown to be related to several important derangements in protein and fat metabolism. However, mechanisms of impaired glucose tolerance after surgery remain ill-defined. 2. Insulin sensitivity and glucose (6,62H2-glucose) were studied in seven before and after elective surgery (surgery group), by two step-hyperinsulinaemic (0.3 and 0.8 munits kg-1min-1), normoglycaemic (4.5 mmol/l) clamps. Six healthy subjects were studied, using the same protocol, before and after a similar period of bed rest and hypocaloric nutrition (fast/bed rest group) to delineate the effects of surgery per se. 3. Unless your condition is causing significant problems or is rapidly getting worse, most doctors will begin with nonsurgical treatment.. At first, your doctor may want your low back immobilized. Keeping the back still for a short time can calm inflammation and pain. This might include one or two days of bed rest. Lying on your back can take pressure off sore discs and nerves. However, most doctors advise against strict bed rest and prefer their patients to do ordinary activities using pain to gauge how much is too much. In rare cases in which bed rest is prescribed, it is usually used for a maximum of two days.. A back support belt is sometimes used for patients with lumbar disc herniation. The belt can help lower pressure inside the problem disc. Patients are encouraged to gradually discontinue wearing the support belt over a period of two to four days. Otherwise, their trunk muscles begin to rely on the belt and start to atrophy (shrink).. Doctors prescribe certain types of medication for ...
PURPOSE: In this study we sought to determine whether a Titin peptide fragment can serve as a clinical biomarker for changes in muscle mass. METHODS: Mass spectrometry was used to identify Titin fragment in urine. An antibody against this Titin sequence was raised and used to develop a competitive ELISA assay for measurement in serum. Rat tissue extractions in the presence or absence of a series of proteases of interest were used to identify its enzymatic origin. A rat model of dexamethasone (DEX) induced muscle atrophy and a human 56-day bed rest study with and without vibration therapy were used to assess biological and clinical relevance. RESULTS: A technically robust ELISA measuring the Titin fragment was developed against a Titin peptide fragment identified in human urine. The fragment was shown to be produced primarily by MMP-2 cleavage of Titin. In the rat muscle DEX induced atrophy model, Titin-MMP2 fragment was decreased in the beginning of DEX treatment, and then significantly ...

Postprandial hyperglycemia has been linked to elevated risk of cardiovascular disease. Endothelial dysfunction and/or damage may be one of the mechanisms through which this occurs. In this exploratory study, we determined whether acute glucose ingestion would increase markers of endothelial damage/activation and impair endothelial function before and after a short-term low-carbohydrate high-fat diet (HFD) designed to induce relative glucose intolerance. Nine healthy young males (body mass index 23.2 ± 2 kg/m2) consumed a 75 g glucose drink before and <24 hours after consuming seven days of an iso-energetic HFD consisting of ~70% energy from fat, ~10% energy from carbohydrates, and ~20% energy from protein. CD31+/CD42b- and CD62E+ endothelial microparticles (EMPs) were enumerated at fasting, 1 hour (1 h), and 2 hours (2 h) post-consumption of the glucose drink. Flow-mediated dilation (FMD), arterial stiffness, and diameter, velocity, and flow of the common and internal carotid, and vertebral
Challenge number one begins with the end days of my pregnancy with Caleb. I begin to have blood pressure problems. My blood pressure is raising. Uh oh. Im put on strict bed rest, but that doesnt seem to ease the situation so off to the hospital I go for more tests. At this point, I am 39 weeks pregnant and 3 days. They test further and yes, the blood pressure has become worrisome to them. And in addition to that, I have a low platelet count. They actually tell me I have the beginning stages of a Syndrome called HELLP which stands for Hemolysis (blood pressures issues), Elevated Liver enzymes, and Low Platelet count. I never did have the liver enzymes leak, but they werent going to wait to see if it was going to happen. They recommended I be induced and get this baby out quick as the worry is that if I have severe high blood pressure, which I didnt yet, then I wouldnt be able to have a c-section should I require assistance with the delivery.. Okay, Saturday morning drive to the hospital for ...
On anti-TB treatment and strict bed rest, there was mild improvement of her neurological function in the first week after admission. Surgery for a decompression and fusion revealed metastatic adenocarcinoma. She died in hospital.. Discussion. Bone and joint TB continues to be a major problem in most developing countries, with evidence of resurgence in the developed world. Several Medical Research Council (MRC) trials established that drug therapy is the mainstay of treatment for TB of the spine, and that surgery is only indicated when there is gross destruction, deformity or associated neurological deficit not responding to chemotherapy. A cause of such destruction and deformity is delay in diagnosis. More than 50% of the vertebra has to be destroyed before a lesion can be seen on a plain radiograph, a process ...

The research is part of a long-term effort to understand the factors that contribute to the loss of muscle mass - in particular as a result of immobility.. We know that if you are under a condition of disuse - for example, as a result of long-term bed rest, or the immobilization of a body part in a cast - you lose muscle mass, Boppart said. And even when you come out of that state of immobility and you start moving your muscles again, theres this really long, slow process of recovery.. Older adults might never fully rebuild the lost muscle mass after a period of immobility, she said.. They cant recover, they become disabled, and theres this downward spiral, Boppart said. They may become institutionalized and experience early mortality.. Researchers have long searched for clinical interventions that can help restore lost muscle mass and impaired function as a result of inactivity, Boppart said.. To my knowledge, no one has demonstrated that anything has been effective in improving the ...
Give her permission to rest as much as possible, especially in the first trimester when sitting up or standing can provoke relentless vomiting. Prolonged nausea and vomiting will leave her exhausted and depressed. Help her by making sure she is getting the best care possible by educating yourself on hyperemesis and her treatment options. If she is not improving, take her to a different health care professional. She may be too sick to do this for herself. (Find a new healthcare professional in our Referral Network.). Every effort to eliminate triggers of vomiting and nausea is critical to her physical and emotional health. If she has children at home that she is unable to care for, it is very important that she have help. It is likely that she will be unable to help with house work and other responsibilities for some time. She may feel guilty and stressed and need reassurance, understanding and help from those around her. The stress she feels will increase her cortisol levels, which means the ...

This is a summary of a systematic review on using early ambulation as a treatment for pulmonary embolism (PE) and deep vein thrombosis (DVT).
The 2 week wait was as difficult as always and the extra task of 2 days bed rest was frustrating and tense. I felt emotional and anxious so made sure I did extra sessions of calming Zita West visualizations and meditation to counter this. Two days later a call came which shook our foundation of positivity for the first time, it was the embryologist to report that the other 3 embryos stopped growing so we now had none to freeze. Initially we thought ok theres the back up plan gone and wondered if this meant that the overall quality of the entire batch wasnt so good? This blow felt like the stakes were raised but my internal friend Miss Positivity piped up and reminded me its ok, we only need 1 to work and the strongest 2 are where they need to be, inside me.. During the wait I tried to keep my mind distracted and even had a trip back home to London to look forward to but really the next 2 weeks were just one huge build up until the day I could start doing early tests (approx. 10 days after ...

Today Dr. Foulk came in and did another ultrasound to see the status of my fluid build up. He said it was looking like things were calming down and there shouldnt be anything to stop us from doing the transfer. We talked about the risk of the OHSS coming back and Matt was worried about me going back to Indiana by myself and not being able to take care of myself while I was sick. Dr. Foulk said after a positive pregnancy test we could come in for another ultrasound and if there was more fluid building up then we could extend my FMLA so I could be here with Matt and have family to take care of me for another OHSS flair up. We were again given unbelievably good news about the quantity and quality of our 5-day embryos (actually blastocysts at this point ...
The moment I found out I was pregnant, I never knew the obstacles i had to maneuver through. Single mom, 21 and no clue how to be a mother. Hurting from his father wanting me to end the pregnancy. I chose not to and he left me alone and pregnant. Knowing it would be complicated because I already had pulmonary stenosis and was high risk. At 14 weeks, I was diagnosed with preeclampsia. Yes, at 14 weeks! Before I knew the sex of my little bundle of joy! I saw my doctor every two weeks and each week I would have a home care nurse check on me and my baby. Somedays, I had to go to triage and get medical treatment due to high blood pressure.. 18 weeks, Im having a BOY! I was so excited! The project which has become a bone of contention among the four of us in that its an example of what happens when an engineer/inventor has ADD. And the children with whom hes doing a project have the attention span of, well, children. And said project goes on for YEARS. And there really isnt a good spot to work on it, so it has to be put away each time, thus getting it set UP to work on it sometimes takes 50% of the time available for working on it, thus limiting the motivation for said work. And I just want it DONE. It is currently 90% done. For petes sake FINISH THE THING. But the word is just gone. Cannot come up with it. Day 2 was pretty uneventful. Symptom watch is pretty quiet. I have only had the period coming feeling which I know is from the PIO. I no longer pay attention to it, me the now veteran IVFer. What does it mean to be a veteran IVFer? Is it going forward after even just one failed cycle? I believe a person in the military is considered a veteran after one tour of duty - using that definition then yes. But for me, I dont think I considered myself a veteran until after my 3rd failed cycle. Part of that was because my RE mentioned that in ART if a treatment doesnt work by the 3rd time, then statistically the chances are it wont work. So, my going forward after my 3rd (4th transfer) was like continuing to fight and win a losing battle position ...

Wittink H, et al. The Dangers of Inactivity; Exercise and Inactivity Physiology for the manual therapist. Manual Therapy 2011; 16:209-216.. This paper defines deconditioning as the physiological response of the body to a reduction in metabolic rate; that is, how the body reacts to a reduction in energy use or exercise levels, as in sitting or bed rest. Deconditioning is associated with a host of physiological changes:. 1. Muscle mass decreases with loss of muscle strength.. 2. Muscle capillary density declines along with mitochondrial enzyme activity & ATP production leading to loss of muscle oxidative potential & increased fatigability of muscle.. 3. Decline of cardiac output.. 4. Decrease of aerobic capacity (VO2 max).. 5. Bone strength decreases through increased bone resorption & decreased bone formation in response to unloading. Bone loss occurs most at weight-bearing skeletal sites.. 6. Disuse decreases the collagen turn-over in tendons & muscles, weakens attachments of ligaments to bone & ...
Twisted Thruster with Dumbbells or Sandbag You know the thruster perhaps. Now meet the twisted thruster. This move is a superb full-body blaster that engages your core by stabilizing you as you descend, rise and twist. That extra twist at the top, unlike a strict thruster, recruits your obliques a ton and gives the entire core extra work to maintain good form and control. Keep this move fairly quick. Dont hang out at the bottom of the squat or at the top of the twist. The move, when done with speed but under control, will activate your core like crazy and steer you toward a six pack faster. Do it: Stand with feet slightly wider than hip width, holding a dumbbell in each hand or a sandbag (as shown) with the parallel handles. If holding dumbbells, lift the weight to each shoulder to start. Abs braced. If using a sandbag, clean the weight to your shoulders by pulsing your knees and then using that slight momentum to bend your arms and lift the weight to chest height, then flip your elbows forward ...
Since waking up usually happens during rapid eye movement sleep (REM), the vivid bizarre REM sleep dreams are the most common type of dreams that is remembered. (During REM sleep an electroencephalogram (EEG) shows brain activity that, among sleep states, is most like wakefulness.) During a typical lifespan, a person spends a total of about six years dreaming[43] (which is about two hours each night).[44] Most dreams only last 5 to 20 minutes.[43] It is unknown where in the brain dreams originate, if there is a single origin for dreams or if multiple portions of the brain are involved, or what the purpose of dreaming is for the body or mind. During REM sleep, the release of the neurotransmitters norepinephrine, serotonin and histamine is completely suppressed.[3][45][46] During most dreams, the person dreaming is not aware that they are dreaming, no matter how absurd or eccentric the dream is. The reason for this may be that the prefrontal cortex, the region of the brain responsible for logic ...

By participating in physical therapy, you may learn how to improve your physical skills, move your body in a different way, or use assistive equipment to help compensate for limitations. You will learn exercises that are beneficial for gaining function, reducing stress, relieving discomfort, and preventing disability or future injury ...
I had an IVF procedure and conceived triplets. I researched all the options, looked at the statistics and decided to take my chances. To be faced with selectively reducing a child that you have tried years to conceive is absolutely beyond words. I had a cerclage(cervical stitching) and moderate bedrest at 14 weeks. Despite doing everything I was told, I had premature labor at 21 weeks and was admitted to the hospital. Total bedrest, shower every 3 days, bedpans, the works. It was awful. At 23 weeks trendelenburg- head down in the bed until 28 weeks 5 days and the babies were born. I almost bled to death during the delivery, the babies weighed 2lb 8 oz, 2 lb 2 oz, and 1 lb. 7 oz. Day 5 B baby suddenly died. it was horrible and painful experience to lose a child and to watch the others suffer, feeling like it was my fault. I chose this for them, although, I would not change my decision even losing the one child. I am lucky to have 2 beautiful healthy children, and at least my other one had a ...
You can expect to lose muscle size and strength as you age. Between 40 and 50 years of age, you lose more than eight percent of your muscle size. This loss increases to 15 percent per decade after age 75. The people who lose the most muscle usually are the least active, exercise the least and are the ones who die earliest. Older people who lose the most muscle are four times more likely be disabled, have difficulty walking, and need walkers and other mechanical devices tohelp them walk (Am J Epidemiol, 1998; 147(8):755-763).. Lack of exercise makes muscles smaller. If you inactivate a leg by putting it in a cast for just four days, you lose a significant amount of muscle size. However, contracting those muscles by stimulating them with an electric current delayed some of the loss of muscle size (Nutrition, Acta Physiol (Oxf), March 2014;210(3):628-41). Prolonged periods of inactivity due to bed rest, cut nerves, casting or even decreasing the force of gravity lead to loss of muscle tissue which ...
