Strategies for cutting hospital beds: the impact on patient service. (1/6)

OBJECTIVE: To develop insights on the impact of size, average length of stay, variability, and organization of clinical services on the relationship between occupancy rates and delays for beds. DATA SOURCES: The primary data source was Beth Israel Deaconess Medical Center in Boston. Secondary data were obtained from the United Hospital Fund of New York reflecting data from about 150 hospitals. STUDY DESIGN: Data from Beth Israel Deaconess on discharges and length of stay were analyzed and fit into appropriate queueing models to generate tables and graphs illustrating the relationship between the variables mentioned above and the relationship between occupancy levels and delays. In addition, specific issues of current concern to hospital administrators were analyzed, including the impact of consolidation of clinical services and utilizing hospital beds uniformly across seven days a week rather than five. PRINCIPAL FINDINGS: Using target occupancy levels as the primary determinant of bed capacity is inadequate and may lead to excessive delays for beds. Also, attempts to reduce hospital beds by consolidation of different clinical services into single nursing units may be counterproductive. CONCLUSIONS: More sophisticated methodologies are needed to support decisions that involve bed capacity and organization in order to understand the impact on patient service.  (+info)

Quality of long-term care in nursing homes and swing-bed hospitals. (2/6)

By 1989, more than 1,100 hospitals in rural communities throughout the United States were using hospital beds as swing beds to provide both long-term and acute care. In this study, the quality of long-term care in swing-bed hospitals was compared with the quality of nursing home care, using patient outcomes along with both process and structural measures of quality. Several methodological and conceptual points on measuring and analyzing the quality of long-term care are discussed in this article. Data were analyzed on approximately 2,000 patients in four different primary data samples, three of which were longitudinal involving multiple follow-up points. An analysis of changes in patient status over time, hospitalization rates, rates of discharge to independent living, services provided, and certain structural indicators showed that (1) relative to nursing home care, swing-bed care is more effective in enhancing functional outcomes and discharge to independent living and in reducing hospitalization for long-term care patients, and (2) nursing home care appears more desirable than swing-bed care for long-stay, chronic care patients with no rehabilitation potential. Swing-bed hospitals have gravitated largely to admitting postacute long-term care patients. They do not typically compete directly with community nursing homes for chronic care patients. The greater effectiveness of swing-bed care for patients with near-acute long-term care needs suggests that this approach should be considered in urban communities and that we should scrutinize our current tendency to place in traditional nursing homes many patients who have at least some rehabilitation potential.  (+info)

The long good-bye: the great transformation of the British Columbia hospital system. (3/6)

Much is made of the "threat" an aging population poses to North American health care systems. In this article, we present hospital utilization data from British Columbia over the period 1969-1985, which reinforce our earlier (Barer, Evans, Hertzman, et al. 1987) conclusion: it is not aging per se that poses the threat; rather, it is what we are choosing (through our health care system) to do to and with our elderly. In 1969, British Columbia hospital patients over 65 years of age and staying longer than 60 days accounted for 12.5 percent of all days; by 1985/86, they were accounting for 39 percent. Furthermore, in 1985/86, 1 patient in 200 was using one-quarter of all patient days and dying at the end of the process, and 2 patients in 100 (who stayed over 60 days whether discharged alive or dead) were accounting for almost one-half of all days.  (+info)

Hospital swing-bed care in the United States. (4/6)

As a result of federal legislation implemented in 1982, hospital beds that are used to provide both long-term care and acute care are now proliferating rapidly throughout the country. Termed swing beds, such beds are currently restricted to rural areas. However, due largely to the impacts of Medicare DRG reimbursement, pressure is mounting to expand the swing-bed approach to urban settings. Swing beds appear to fill a significant gap between the relatively intense medical needs of post-acute care patients (now discharged earlier) and the capacity of our current nursing home delivery system to meet such needs. The evolution of swing beds is marked by an unusual blend of experimentation, scientific investigation, and public policy response to community and personal health care needs. This article summarizes that evolution, highlighting research findings and key policy developments. It concludes with the current status of the national swing-bed program and issues pertinent to future directions.  (+info)

Use of general practitioner beds in Leicestershire community hospitals. (5/6)

BACKGROUND: The shift in care from secondary to primary services is likely to place greater demands on community hospitals. Before changes in the provision of community hospitals can occur, baseline data are needed, outlining their current use. AIM: A study was undertaken to obtain baseline data describing the use of general practitioner beds in Leicestershire community hospitals. METHOD: A three-month prospective, observational study was carried out between February and May 1992 using data from a questionnaire completed by nurses and general practitioners and from patient hospital records. Study patients comprised all patients admitted to general practitioner beds in all eight Leicestershire community hospitals. RESULTS: A 100% questionnaire response rate was obtained giving data on 685 hospital admissions. Around 70% of admissions were of patients aged 75 years and over. Of admissions, 35% were for acute care, 31% for respite care, 22% for rehabilitation, 7% for terminal/palliative care and 5% for other reasons. Fifteen per cent of patients had been transferred from a consultant bed. Of those not transferred, 91% were admitted by their usual general practitioner or practice partner and for 96% of these patients this was the general practitioner's first choice for care. There was significant variation in both the age mix and care category mix of patients between individual hospitals. Medical deterioration in an underlying condition and family pressure on the general practitioner or carers' inability to cope each contributed to around half of all admissions. Of all admissions, 38% lived alone, and 18% of carers were disabled. Incontinence was reported for 35% of patients, and 26% of all patients were of a high nursing dependency. There was low utilization of community services before admission and 33% received none. There was variation between individual hospitals in use of local and district general hospital investigations, specialist referral and types of therapy. Of 685 admissions 11% died during their stay. Of those discharged, 76% went to their own or a relative's home, 10% to a residential or nursing home and 9% were transferred to an acute bed. Nine percent of discharges were postponed and 10% were brought forward. On discharge to non-residential care, 26% of patients received no community services. CONCLUSION: Shifting resources from secondary to primary care is a priority for purchasers. Both the introduction of the National Health Service and community care act 1990, and acute units having increasing incentives for earlier discharge, are likely to place greater demands on community hospital beds. Not all general practitioners have the option of community hospital beds. Before access to general practitioner beds can be broadened, existing beds should be used appropriately and shown to be cost-effective. Purchasers therefore require criteria for the appropriateness of admissions to general practitioner beds, and the results of a general practitioner bed cost-benefit analysis.  (+info)

Arranging hospital admission for acutely ill patients: problems encountered by general practitioners. (6/6)

BACKGROUND: Reports in the national press suggest that general practitioners in London are experiencing difficulties in securing hospital admission for their acutely ill patients. AIM: A study was undertaken to investigate the problems encountered by general practitioners in one family health services authority in south east London in arranging acute admissions to hospital. METHOD: A self-report questionnaire was completed by a sample of general practitioners every time an acute hospital admission was attempted. RESULTS: A total of 493 questionnaires were completed by 111 general practitioners over the 47-day study period. Problems during the hospital admission procedure were experienced in 171 (35%) of the cases reported, with 115 of the 537 telephone calls to a hospital (21%) resulting in a refusal to admit the patient to that particular hospital. The main problem reported was that of 'no beds available', an obstacle to admission that was more likely to be encountered if the patient was aged 75 years or over than if the patient was younger. CONCLUSION: In the light of the problems reported, possible changes to the current method of arranging acute admissions to hospital in London are discussed.  (+info)

Vancouver, Canada, Sep 30, 2019 / 05:01 pm (CNA).- After Alan Nichols death by voluntary euthanasia at a British Columbia hospital in July, his family have questioned the imprecision of Canadas requirements for euthanasia and assisted suicide, which include having a grievous and irremediable medical condition.. They doubt Nichols was able to give informed consent to his euthanization, and maintain his natural death was not reasonably foreseeable.. According to a Sept. 24 article in CTV News based on accounts from Nichols family and a neighbor, Nichols was admitted to Chilliwack General Hospital, about 60 miles east of Vancouver, after he was found in poor physical condition at his home in mid-June.. Nichols, who was 61, was euthanized July 26 at the hospital by medical staff who delivered three injections.. He didnt have a life-threatening disease. He was capable of getting around. He was capable of doing almost anything that you had to do to survive, his brother, Gary Nichols, told CTV ...
The University of British Columbia Hospitals urgent-care centre now stocks the take-home naloxone kits being widely used in Vancouvers Downtown Eastside to reverse drug overdoses.. But some student leaders wonder whether those efforts will be in vain because of government policies that subtly discourage students from asking for them.. The urgent-care centre began stocking naloxone kits in late October in response to the ongoing crisis in B.C. with fentanyl overdoses. There is concern around overdoses at the school, as UBC Hospital experienced an increase last year. Eight overdose incidents were recorded, up from the six that occurred in 2015, prompting further prevention methods like more available naloxone kits.. But so far only six kits have been handed out.. Students fear stigma of declaring themselves drug users. The low number of kits handed out is likely because students must declare themselves opioid drug users at the campus health clinic, according to provincial laws. This means ...

The ultrasound exam is performed between 11 weeks 1 day and 13 weeks 6 days with the blood test performed anytime from 9 weeks to 13 weeks 6 days. During the examination we will confirm the gestational date and the measurement of the skin thickness at the back of the fetus neck. Babies with some birth defects (such as Down syndrome or cardiac malformation), tend to have a thicker measurement. We also look for the bone in the baby nose and other difficult views of the flow in the babys cord and even in the babys heart. Not all of these measurements can be obtained in all babies (mostly because of babies unfavorable position or moms weight) but we attempt to obtain as many as possible.. Thats right people - I botched running across the street. This morning on the way to work I figured Id sprint the block and half to the bus stop as my bus is always early and I didnt want to be late to EA. Unfortunately, in the middle of the street my fifth metatarsal bone on my right foot decided to break, sending me into a freefall 180 degree smash into the road. My hands, knee and ankles were all scraped up, but my foot was in agony. After some kind bystanders helped me up off the street, I hobbled home and applied ice to what I thought was a sprained foot. Alas, an hour later my foot was swelling like balloon and the pain was excruciating. So Mum (wonderful angel that she is) came over and off to the University of British Columbia Hospital we went. Good news: took hardly any time to be seen. Bad news: its broken alright. The xray of my foot almost made me puke. The bone is actually displaced. The 1970s and 1980s saw a large increase in the prevalence of asthma and other allergic disorders in children. The reasons remain unclear. To learn more it is essential to carry out long term studies on subjects, preferably from birth. In Europe, several of these since birth studies on asthma and lung diseases have been initiated over the last two decades.. Our project began with the first cohort of children in 1990 and a second in 1998. It totals over 10,000 families of children born in Leicestershire and Rutland. We have tracked their progress from birth to young adulthood by means of repeated questionnaires, breathing tests and allergy tests. Success is measured in the prolific output of research reports. These are available for doctors and researchers worldwide.. This project is by far the largest study devoted to breathing problems in the UK, and one of the largest in Europe. It is based as a random sample of children born to Leicestershire and Rutland families. Leicester is home to a ...

The Health Affairs study also presents a comparison of hospital pricing growth rates as compared to physician pricing growth rates. The study is only a few weeks old and I am surprised I am able to access as much information as I have. While Health Affairs admits the study is a start and more work differentiating other aspects must be done, the study suggests there are significant growth in the bargaining leverage of hospitals as compared to physicians.. If you recall Rusty Tom and I engaged in a number of different conversations on healthcare with one of them being hospital consolidations (2013). It is a power grab, as Rusty pointed out, for more market segment and ...

This is another reason why the House and Senate plans to roll back the Affordable Care Act (and the destruction of Medicaid as part of the deal) are so important to the Indian health system. The Senate plan especially reverses decades of Medicaid growth. Thats no good for patients (as I have reported before here and here) but it also will cost Indian Country jobs.. Medicaid directly accounts for 24 percent of IHS workforce, but even that is short of the kinds of jobs that serve tribal communities. It does not include school-based programs. Or those private companies that bill Medicaid for transportation of patients or other patient services. Same story for jobs that stem from medical coding, accounting and third-party billing services.. Across the country, Medicaid is a major job creator for women, supporting at least 3 million direct jobs. According to a report by the National Womens Law Center: When Medicaid pays for a health service-a visit with a health professional, a laboratory test, a ...

Officials say they have multiple priorities to see to including contact tracing efforts. In this article, the author will define efficiency as a clinical concept and discuss its application to the practice of modern endodontics. Given a specific standard, there are more- and less-efficient ways to achieve such results. Efficiency uses technical systems to achieve the desired standard of care faster. Performing procedures efficiently is not only a matter of office productivity; it is also a matter of responsible patient service. Efficiency in diagnosis, anesthesia, access, irrigation, instrumentation, and obturation is discussed. The simpler and more efficiently this healthy foundation can be achieved, the faster patients can heal and benefit from endodontic service.
