Effect of selective screening for gestational diabetes. (1/222)

OBJECTIVE: To estimate the percentage of pregnant women who would not be screened and the percentage of women with gestational diabetes mellitus (GDM) who would possibly remain undiagnosed if the American Diabetes Association's (ADA's) new selective screening recommendations are used rather than universal screening for GDM. RESEARCH DESIGN AND METHODS: Since 1987, the University of Michigan Health System has performed universal screening for GDM. In 1997, the ADA recommended that women having all four of the following characteristics need not be screened: age < 25 years, not members of an ethnic/racial group with a high prevalence of diabetes, normal body weight, and no family history of diabetes. We studied a random sample of the 25,118 deliveries at the University of Michigan between 1987 and 1997 to determine the prevalence of these four characteristics in our obstetric population. We also studied the prevalence of these four characteristics in 200 women who were diagnosed with GDM in the Endocrine Testing Unit and delivered at the University of Michigan between 1987 and 1997. RESULTS: Approximately 10-11% of women who delivered possessed all four low-risk characteristics and would not have been screened for GDM according to the new ADA recommendations. Only 4% of women (5 of 141) with GDM who delivered and for whom data on all four characteristics were reported possessed all four low-risk characteristics and would not have been screened. CONCLUSIONS: If the new ADA selective screening recommendations are used, few women with GDM will be missed (4%) but approximately 90% of pregnant women will still need to be screened for GDM.  (+info)

Relationships of fasting and postload glucose levels to sex and alcohol consumption. Are American Diabetes Association criteria biased against detection of diabetes in women? (2/222)

OBJECTIVE: To compare, in men and women, the prevalence of undiagnosed type 2 diabetes assessed using criteria from the American Diabetes Association (ADA) and the World Health Organization (WHO) and to investigate risk factors associated with fasting and 2-h postload plasma glucose. RESEARCH DESIGN AND METHODS: Data from two companion surveys of Europeans, South Asians, and Afro-Caribbeans in west London were used. A total of 4,367 men and women aged 40-64 years who were not known to have diabetes underwent an oral glucose tolerance test after an overnight fast. The prevalence of undiagnosed diabetes was estimated using the ADA (fasting plasma glucose > or = 7.0 mmol/l) and WHO (2-h postload glucose > or = 11.1 mmol/l) criteria for epidemiologic studies. The association of body fat and usual alcohol intake with plasma glucose and diabetes prevalence was assessed. RESULTS: Compared with the WHO criterion, the ADA criterion gave a higher prevalence of diabetes in men (6.4 vs. 4.7%) but a lower prevalence in women (3.3 vs. 4.2%). In Afro-Caribbeans, the sex difference in diabetes prevalence was reversed. Women had significantly lower fasting glucose than men despite higher 2-h glucose levels. Alcohol intake was positively associated with fasting glucose in men and women but not with 2-h glucose levels. CONCLUSIONS: The new ADA criterion, based on fasting glucose alone, does not take account of sex differences in metabolic response to fasting or possible artifactual effects on fasting glucose. With the ADA criterion, alcohol intake was a significant risk factor for diabetes in our study population; this was not the case with the WHO criterion.  (+info)

Properties of LTP induction in the CA3 region of the primate hippocampus. (3/222)

Activity-dependent changes in synaptic strength, such as long-term potentiation (LTP), have been proposed to underlie memory storage in the brains of all mammals, including humans. However, most forms of synaptic plasticity, including LTP, are studied almost exclusively in rodents and related species. Thus, the hypothesis that LTP is important in human memory relies on the assumption that LTP is similar in the primate and rodent brains. We have begun to test this hypothesis by studying the properties and mechanisms of LTP induction in area CA3 of hippocampal slices from cynomolgus monkeys. We have found that LTP can be induced reliably at both mossy fiber-CA3 and collateral/associational-CA3 synapses in the primate brain, and that the properties of LTP induction at these synapses are similar to what we and others have observed in experiments using hippocampal slices from rodents. Also, we have investigated the role of opioids in mossy fiber synaptic transmission and LTP and have found no effect of the opioid antagonist naloxone nor the opioid agonist dynorphin on mossy fiber synaptic transmission or potentiation. These data suggest that LTP in the primate and rat brains has a similar induction mechanism and, thus, that the rodent is a useful animal model in which to study synaptic modification such as LTP.  (+info)

Physiologically realistic formation of autoassociative memory in networks with theta/gamma oscillations: role of fast NMDA channels. (4/222)

Recordings from brain regions involved in memory function show dual oscillations in which each cycle of a low-frequency theta oscillation (5-8 Hz) is subdivided into about seven subcycles by high frequency gamma oscillations (20-60 Hz). It has been proposed (Lisman and Idiart 1995) that such networks are a multiplexed short-term memory (STM) buffer that can actively maintain about seven memories, a capability of human STM. A memory is encoded by a subset of principal neurons that fire synchronously in a particular gamma subcycle. Firing is maintained by a membrane process intrinsic to each cell. We now extend this model by incorporating recurrent connections with modifiable synapses to store long-term memory (LTM). The repetition provided by STM gradually modifies synapses in a physiologically realistic way. Because different memories are active in different gamma subcycles, the formation of autoassociative LTM requires that synaptic modification depend on N-methyl-D-aspartate (NMDA) channels having a time constant of deactivation that is of the same order as the duration of a gamma subcycle (15-50 msec). Many types of NMDA channels have longer time constants (150 msec), as for instance those found in the hippocampus, but both fast and slow NMDA channels are present in cortex. This is the first proposal for the special role of these fast NMDA channels. The STM for novel items must depend on activity-dependent changes intrinsic to neurons rather than recurrent connections, which have not developed the required selectivity. Because these intrinsic mechanisms are not error-correcting, STM will become slowly corrupted by noise. This limits the accuracy with which LTM can become encoded after a single presentation. Accurate encoding of items in LTM can be achieved by multiple presentations, provided different memory items are presented in a varied interleaved order. Our results indicate that a limited memory-capacity STM model can be integrated in the same network with a high-capacity LTM model.  (+info)

Nonassociative and associative modification of head-waving produced by aversive tentacular stimuli in Aplysia. (5/222)

Head-waving, a spontaneously occurring exploratory and appetitive behavior of the marine mollusc Aplysia, provides an opportunity to examine mechanisms of learning expressed in a nonreflexive behavior. The present study explores nonassociative and associative forms of learned modification of head-waving produced using an aversive stimulus as reinforcement. Experiments on intact, freely behaving animals demonstrate that training with electric shock as an aversive unconditioned stimulus, delivered unilaterally to the anterior tentacles, produces a learned shift in head-waving behavior away from the side on which shock was applied. This behavioral change is a novel learned behavioral response that is influenced by the topographic location of an aversive stimulus. Furthermore, training with application of tentacle shock reinforcement, contingent upon the animal's head position, produces operant conditioning of head-waving. Thus, anterior tentacle shock is effective as an aversive reinforcer for both nonassociative and operant learning expressed in the head-waving behavior of Aplysia.  (+info)

Parsing executive processes: strategic vs. evaluative functions of the anterior cingulate cortex. (6/222)

Event-related functional MRI and a version of the Stroop color naming task were used to test two conflicting theories of anterior cingulate cortex (ACC) function during executive processes of cognition. A response-related increase in ACC activity was present when strategic processes were less engaged, and conflict high, but not when strategic processes were engaged and conflict reduced. This is inconsistent with the widely held view that the ACC implements strategic processes to reduce cognitive conflicts, such as response competition. Instead, it suggests that the ACC serves an evaluative function, detecting cognitive states such as response competition, which may lead to poor performance, and representing the knowledge that strategic processes need to be engaged.  (+info)

Mediodorsal thalamic function in scene memory in rhesus monkeys. (7/222)

Three monkeys were trained preoperatively in a scene memory task which is analogous, in some ways, to human episodic memory. The same animals were also trained in object-reward association memory. Following bilateral ablations of almost the entire magnocellular division of the mediodorsal thalamic nucleus, the animals were impaired both in scene memory and in object-reward association memory. These results, combined with recent results in object recognition memory from monkeys with mediodorsal thalamic lesions, show that the impairment produced by this lesion is more general, affecting a broader range of memory tasks, than the impairment which is produced in monkeys by lesions restricted to the hippocampus-fornix-mamillary system. It is also more severe than the effect of lesions limited to the medial part of the magnocellular division of the mediodorsal thalamic nucleus. These findings extend the evidence that the magnocellular division of the mediodorsal thalamic nucleus has an important and general role in memory, and they are consistent with the proposal that lesions of the magnocellular division of that nucleus have their effect by disrupting the function of prefrontal cortex.  (+info)

Associative olfactory learning in the moth Manduca sexta. (8/222)

The proboscis extension response conditioning protocol has been used to explore olfactory-based associative learning in an array of insects. We have monitored a different feeding reflex, which involves activation of the cibarial pump, to demonstrate olfactory learning in the moth Manduca sexta. In the first experiment, four different treatment conditions were used to assess associative (Pavlovian) learning. The results indicate that an excitatory cibarial pump response develops and is retained for at least 24 h only when odor is forward-paired with the presentation of sucrose. Three control treatments, backward pairing, air (no odor) pairing and random pairing, failed to increase the cibarial pump response. However, an excitatory cibarial pump response developed when the backward- and air-paired groups received forward pairing of odor and sucrose on the following day. In contrast, moths experiencing random pairing on day 1 displayed a slower rate of acquisition during forward pairing on day 2, which may indicate inhibition. The second experiment investigated discrimination learning. Two odors were randomly presented, one odor being forward-paired with sucrose (+), the other presented alone (-) in a counterbalanced design. Again, only when odor was forward-paired with sucrose did learning occur. We discuss the implication of these findings for a broader comparative analysis of learning in insects.  (+info)

Purpose:The aim of the current study was to examine the processes by which implicit and explicit stress mindsets relate to health- and performance-related outcomes. Specifically, we propose a stress beliefs model in which somatic responses to stress and coping behaviors mediate the effect of stress mindsets on outcomes. Methods:Undergraduate university students (N = 218, n = 144 females) aged 17- 25 years completed measures of stress mindset, physical and psychological wellbeing, perceived stress, perceived somatic responses to stress, proactive behaviors under stress, and an implicit association test assessing an implicit stress mindset. Over the past few years, researches have come to distinguish two broad forms of synaesthesia which render the spatial expansion principle correct for only one of the two forms. Today, synaesthetes are recognized as either lower synaesthetes termed projectors or higher synaesthetes termed associators . The terms higher and lower refer to the level of processing at which the sense-perceptions are crossed while the terms projector and associator refer to where the percept is sensed i.e. out in the world versus in the minds eye. This account aims to furnish a general framework and specific pathways to implement models that are more closely related to biological evidence. The amygdala, which receives input from brain areas encoding internal states, innately relevant stimuli, and innately neutral stimuli, plays a fundamental role in motivational and emotional processes of organisms. University of São Paulo Reasons for Smoking Scale: a new tool for the evaluation of smoking motivation
Objective: To develop a new scale aimed at evaluating smoking motivation by incorporating questions and domains from the 68-item Wisconsin Inventory of Smoking Dependence Motives (WISDM-68) into the Modified Reasons for Smoking Scale (MRSS). Methods: Nine WISDM-68 questions regarding affiliative attachment, cue exposure/associative processes, and weight control were added to the 21 questions of the MRSS. The new scale, together with the Fagerström Test for Nicotine Dependence (FTND), was administered to 311 smokers (214 males; mean age = 37.6 ± 10.8 years; mean number of cigarettes smoked per day = 15.0 ± 9.2), who also provided additional information. We used exploratory factor analysis in order to determine the factor structure of the scale. Suicide is the 15th most common cause of death worldwide. Although relatively uncommon in the general population, suicide rates are much higher in people with mental health problems. Clinicians often have to assess and manage suicide risk. Risk assessment is challenging for several reasons, not least because conventional approaches to risk assessment rely on patient self reporting and suicidal patients may wish to conceal their plans. Accurate methods of predicting suicide therefore remain elusive and are actively being studied. Novel approaches to risk assessment have shown promise, including empirically derived tools and implicit association tests. Service provision for suicidal patients is often substandard, particularly at times of highest need, such as after discharge from hospital or the emergency department. The sensory recruitment model envisages visual working memory (VWM) as an emergent property that is encoded and maintained in sensory (visual) regions. The model implies that enhanced sensory-perceptual functions, as in synaesthesia, entail a dedicated VWM-system, showing reduced visual cortex activity as a result of neural specificity. By contrast, sensory-perceptual decline, as in old age, is expected to show enhanced visual cortex activity as a result of neural broadening. The aim of the present research was to investigate the interpersonal effects of pride and shame expressions amongst opponents and teammates in a soccer penalty scenario. Across a series of experiments using the point-light method, pride and shame expressions exerted strong effects upon observers anticipated emotions, associated cognitions, and performance expectations. Using the Implicit Association Test (IAT) in two pilot studies we demonstrated that the created pride and shame point-light stimuli were implicitly associated with status and performance related attributes. In Experiment 1, observing pride expressions caused opponents to anticipate more negative emotions, cognitions, and lower performance expectancies toward their next performance in comparison with neutral expressions. In contrast, pride expressions led teammates to anticipate more positive emotions (i.e., pride and happiness), cognitions, and performance expectations toward their next performance than neutral expressions (Experiments 2

Learning is often understood as an organisms gradual acquisition of the association between a given sensory stimulus and the correct motor response. Mathematically, this corresponds to regressing a mapping between the set of observations and the set of actions. Recently, however, it has been shown both in cognitive and motor neuroscience that humans are not only able to learn particular stimulus-response mappings, but are also able to extract abstract structural invariants that facilitate generalization to novel tasks. Here we show how such structure learning can enhance facilitation in a sensorimotor association task performed by human subjects. Using regression and reinforcement learning models we show that the observed facilitation cannot be explained by these basic models of learning stimulus-response associations. BACKGROUND: Implicit negative attitudes towards other races are important in certain kinds of prejudicial social behaviour. Emotional mechanisms are thought to be involved in mediating implicit outgroup bias but there is little evidence concerning the underlying neurobiology. The aim of the present study was to examine the role of noradrenergic mechanisms in the generation of implicit racial attitudes. METHODS: Healthy volunteers (n = 36) of white ethnic origin, received a single oral dose of the β-adrenoceptor antagonist, propranolol (40 mg), in a randomised, double-blind, parallel group, placebo-controlled, design. Participants completed an explicit measure of prejudice and the racial implicit association test (IAT), 1-2 h after propranolol administration. RESULTS: Relative to placebo, propranolol significantly lowered heart rate and abolished implicit racial bias, without affecting the measure of explicit racial prejudice. Research has shown that lesbian, gay, and bisexual youth have increased rates of physical and mental health problems primarily due to the chronic stress incurred by the levels of prejudice and isolation experienced in their environment, especially in Southern States. Teachers are the most available and sought after adults by sexual minority students, helping them to mitigate such toxic environments. In this study, the aim was to examine the relationship between implicit and explicit attitudes of homonegativity and the moderating role of motivation to control prejudiced reactions among teachers in Southern states. Additional information was gathered about the teachers school environment in regard to protective factors that were or were not in place for sexual minority students and possible resources that they perceived would be helpful in establishing a more positive environment for these students. Racism and sexism (like other -isms, capitalism, socialism, etc.) are societal structures that have, over our lifetimes, been cultivated in every one of us (try the Harvard Implicit Association Tests if you havent already). Im not convinced that labeling individuals as racists (so that you can then peace out on them) is productive: it implies that racism is something that can be isolated, and exists in some individuals, and not others, and all we have to do to solve racism is to stay away from those bad people. Synesthesia is an ideal condition for genetic analysis for 3 reasons: (1) A battery of perceptual tests allows confident identification of synesthetes, (2) synesthesia clusters in families and appears to be inherited, and (3) synesthetic perception may result from increased cross-talk between neighboring neural areas, which suggests a set of candidate genes. As for the last point, the increased cross-talk may either be anatomical (implicating genes involved in neuronal pruning, arborization or apoptosis) or it may be functional (implicating genes involved in the balance between inhibition and excitation).
