A prospective randomized study of megestrol acetate and ibuprofen in gastrointestinal cancer patients with weight loss. (1/70)

The use of megestrol acetate in the treatment of weight loss in gastrointestinal cancer patients has been disappointing. The aim of the present study was to compare the combination of megestrol acetate and placebo with megestrol acetate and ibuprofen in the treatment of weight loss in such patients. At baseline, 4-6 weeks and 12 weeks, patients underwent measurements of anthropometry, concentrations of albumin and C-reactive protein and assessment of appetite, performance status and quality of life using EuroQol-EQ-5D and EORTC QLQ-C30. Thirty-eight and 35 patients (median weight loss 18%) were randomized to megestrol acetate/placebo or megestrol acetate/ibuprofen, respectively, for 12 weeks. Forty-six (63%) of patients failed to complete the 12-week assessment. Of those evaluable at 12 weeks, there was a decrease in weight (median 2.8 kg) in the megestrol acetate/placebo group compared with an increase (median 2.3 kg) in the megestrol acetate/ibuprofen group (P<0.001). There was also an improvement in the EuroQol-EQ-5D quality of life scores of the latter group (P<0.05). The combination of megestrol acetate/ibuprofen appeared to reverse weight loss and appeared to improve quality of life in patients with advanced gastrointestinal cancer. Further trials of this novel regimen in weight-losing patients with hormone-insensitive cancers are warranted.  (+info)

Long term orexigenic effect of a novel melanocortin 4 receptor selective antagonist. (2/70)

1. We designed and synthesized several novel cyclic MSH analogues and tested their affinities for cells expressing the MC1, MC3, MC4 and MC5 receptors. 2. One of the substances HS028 (cyclic [AcCys11, dichloro-D-phenylalanine14, Cys18, Asp-NH2(22)]-beta-MSH11-22) showed high affinity (Ki of 0.95nM) and high (80 fold) MC4 receptor selectivity over the MC3 receptor. HS028 thus shows both higher affinity and higher selectivity for the MC4 receptor compared to the earlier first described MC4 receptor selective substance HS014. 3. HS028 antagonised a alpha-MSH induced increase in cyclic AMP production in transfected cells expressing the MC3 and MC4 receptors, whereas it seemed to be a partial agonist for the MC1 and MC5 receptors. 4. Chronic intracerebroventricularly (i.c.v.) administration of HS028 by osmotic minipumps significantly increased both food intake and body weight in a dose dependent manner without tachyphylaxis for a period of 7 days. 5. This is the first report demonstrating that an MC4 receptor antagonist can increase food intake and body weight during chronic administration providing further evidence that the MC4 receptor is an important mediator of long term weight homeostasis.  (+info)

Model for the regulation of energy balance and adiposity by the central nervous system. (3/70)

In 1995, we described a new model for adiposity regulation. Since then, data regarding the biology of body weight regulation has accumulated at a remarkable rate and has both modified and strengthened our understanding of this homeostatic system. In this review we integrate new information into a revised model for further understanding this important regulatory process. Our model of energy homeostasis proposes that long-term adiposity-related signals such as insulin and leptin influence the neuronal activity of central effector pathways that serve as controllers of energy balance.  (+info)

SR146131: a new potent, orally active, and selective nonpeptide cholecystokinin subtype 1 receptor agonist. II. In vivo pharmacological characterization. (4/70)

SR146131 is a potent and selective agonist at cholecystokinin subtype 1 (CCK1) receptors in vitro. The present study evaluates the activity of the compound in vivo. SR146131 completely inhibited gastric and gallbladder emptying in mice (ED50 of 66 and 2.7 micrograms/kg p.o., respectively). SR146131 dose dependently reduced food intake in fasted rats (from 0.1 mg/kg p.o.), in nonfasted rats in which food intake had been highly stimulated by the administration of neuropeptide Y (1-36) (from 0.3 mg/kg p.o.), in fasted gerbils (from 0.1 mg/kg p.o.), and in marmosets maintained on a restricted diet (from 3 mg/kg p.o.). SR146131 (10 mg/kg p.o.) also increased the number of Fos-positive cells in the hypothalamic paraventricular nucleus of rats. Locomotor activity of mice was reduced by orally administered SR146131 (from 0.3 mg/kg p.o.). When administered intrastriatally, SR146131 elicited contralateral turning behavior in mice. Furthermore, orally administered SR146131 (0.3-10 mg/kg), also reduced the levels of cerebellar cyclic GMP. Finally, SR146131 (0.1 microgram/kg to 1 mg/kg, p.o.) significantly and dose dependently antagonized fluphenazine-induced mouth movements in rats. The CCK1 antagonist SR27897B prevented all the effects of SR146131. Conversely, SR146131 was unable to elicit any agonist or antagonist effects in a model of CCK2 receptor stimulation in vivo. SR146131 is a very potent and selective nonpeptide CCK1 agonist in vivo. SR146131 is more potent than any other CCK1 agonists reported to date. Because pharmacodynamic studies suggest that SR146131 should have a high absolute bioavailability, it may be a promising drug for the treatment of eating and motor disorders in humans.  (+info)

Neuropeptide Y restores appetite and alters concentrations of GH after central administration to endotoxic sheep. (5/70)

The objective of this study was to determine whether neuropeptide Y (NPY) and recombinant human interleukin-1 receptor antagonist (IL-1ra) would: first, increase food intake; secondly, decrease concentrations of GH; thirdly, reduce GHRH-induced release of GH; and fourthly, reduce changes to concentrations of IGF-I in plasma during experimental endotoxemia in sheep. Six treatments were given to six castrated male sheep in a 6x6 Latin square treatment order. Osmotic mini-pumps were implanted at 0 h and a jugular vein was cannulated. Each sheep was continuously infused with saline (0.9%) or lipopolysaccharide (LPS) (20 micrograms/kg per 24 h, s.c.) at 10 microliters/h for 72 h via the osmotic mini-pumps. Blood samples (3 ml) were collected at 15-min intervals from 24 to 33 h. At 26 h, one of three treatments (artificial cerebrospinal fluid, NPY or IL-1ra) was injected i.c.v. within 30 s (0.3 microgram/kg), then infused i.c.v. from 26 to 33 h (600 microliters/h) at 0.3 microgram/kg per h. GHRH was injected i.v. (0.075 microgram/kg) at 32 h after which blood samples were collected at 5, 10, 15, 30, 45 and 60 min. Feed intake was reduced up to 50% for 48 h in LPS-treated compared with non-LPS-treated sheep. NPY restored feed intake in LPS-treated sheep and induced hyperphagia in non-LPS-treated sheep from 24 to 48 h. In contrast, IL-1ra did not affect appetite. Injection of NPY increased concentrations of GH from 26 to 27 h, while IL-1ra had no effect. Infusion of NPY suppressed GHRH-induced release of GH. However, no treatment altered pulse secretion parameters of GH. Concentrations of IGF-I were 20% higher at 72 h in LPS-treated sheep given NPY than in sheep treated with LPS alone, and this may reflect increased appetite from 24 to 48 h. We concluded that reduced appetite during endotoxemia is due to down-regulation of an NPY-mediated mechanism. Furthermore, NPY stimulates release of GH in healthy sheep, does not reduce pulse secretion parameters of GH, but does suppress GHRH-induced release of GH in endotoxic sheep. Therefore, NPY may be an important neurotransmitter linking appetite with regulation of GH during endotoxemic and healthy states in sheep.  (+info)

Effects of delta 9-tetrahydrocannabinol and diazepam on feeding behavior in mice. (6/70)

The present study examined effects of diazepam (DZP) alone or in combination with delta 9-tetrahydrocannabinol (THC) on feeding behavior as well as body weight in male ddY strain mice at 5 weeks of age. Because we saw no hyperphagic effect of DZP with or without THC in mice, we explored the hyperphagia elicitable by DZP. THC [2 (THC2) or 4 (THC4) mg/kg/day s.c.] was given daily for 7 days. For the last day the mice were starved and injected i.p. with DZP (2 mg/kg) 10 min prior to a food or maze test. Controls received vehicle injections. Feeding behavior was measured after giving food for 2 hr. THC4 significantly reduced body weight gain. DZP, with or without THC, induced hyperphagia. THC4 alone also induced hyperphagia that was not significantly affected by DZP. Time taken to find food was extended by DZP and further with THC. Both DZP and THC can therefore interact on food ingestion but synergize on food seeking in mice through different mechanisms.  (+info)

Geriatric cachexia: the role of cytokines. (7/70)

Weight loss in elderly patients is a common clinical problem. Wasting and cachexia are associated with severe physiologic, psychologic, and immunologic consequences, regardless of the underlying causes. Cachexia has been associated with infections, decubitus ulcers, and even death. Multivariate analyses of risk and prognostic factors in community-acquired pneumonia in the elderly have found that age by itself is not a significant factor related to prognosis. Among the significant risk factors, only nutritional status is amenable to medical intervention. Cachexia in the elderly may have profound consequences: medical, cognitive, and psychiatric disorders may diminish self-reliance in activities of daily living, thus reducing quality of life and increasing the frequency of secondary procedures, hospitalizations, and the need for skilled care. Cachexia is associated with higher-than-normal concentrations of tumor necrosis factor alpha (TNF-alpha), interleukin (IL) 1, IL-6, serotonin, and interferon gamma. The role of these proinflammatory cytokines has been established in the cachexia seen in cancer and AIDS patients. Reduction in the concentrations of these cytokines is associated with weight gain. Drugs that promote appetite stimulation and weight gain, such as progestational agents, cyproheptadines, pentoxifylline, and thalidomide may work by down-regulating these proinflammatory cytokines. An understanding of the relation between cachexia and negative regulatory cytokines may point to effective treatment of geriatric cachexia as well.  (+info)

Evaluation and treatment of weight loss in adults with HIV disease. (8/70)

Weight loss late in the course of human immunodeficiency virus (HIV) disease is common and often multifactorial. Increased energy expenditure in response to opportunistic disease, as well as to HIV infection itself, can lead to protein-calorie malnutrition similar to that observed in starvation. Weight loss of as little as 5 percent in patients with HIV infection is associated with an increased risk of disease progression. Loss of body cell mass carries a particularly poor prognosis, and aggressive measures should be taken to stop such depletion. Patients exhibiting unexpected weight loss should be carefully examined to exclude decreased food intake, malabsorption, occult infection or neoplasm as the etiology of the weight loss. Early aggressive treatment of HIV disease and underlying opportunistic pathology, along with adequate pharmacologic, hormonal, nutritional and physical therapy, can often restore normal weight and body composition.  (+info)

Cyproheptadine is currently clinically used as an appetite stimulant for children with failure to thrive without underlying organic disease. Otherwise, no randomised control trial demonstrates the efficacy of Cyproheptadine on those patients. This is precisely what the investigators intend to demonstrate on this randomised placebo control cross-over trial. Our hypothesis is that Cyproheptadine is more efficient than placebo to improve weight gain and feeding behaviour on 2 to 4 years old children with failure to thrive ...

A growing body of evidence indicates that a number of peptides expressed in the mammalian hypothalamus are involved in the regulation of food intake and energy balance. Among these, melanin-concentrating hormone (MCH) and neuropeptide Y (NPY) are potent appetite stimulants, whereas alpha-melanocyte-stimulating hormone (alpha-MSH), neurotensin, and glucagon-like peptide (GLP)-1(7-36) amide have appetite-suppressing properties. However, the functional interactions between pathways involving these neuropeptides remain incompletely understood. In the current study, we describe the functional interactions between orexigenic (appetite-stimulating: MCH and NPY) and anorectic (appetite-suppressing: alpha-MSH, neurotensin, and GLP-1) peptides after intracerebroventricular (i.c.v.) administration in the rat. The i.c.v. administration of GLP-1 completely prevents the orexigenic effects of both MCH and NPY. However, i.c.v. administration of alpha-MSH prevents only the orexigenic effect of MCH, as we have ...

The study involved using two different groups of rats. One was given yogurt sweetened with sugar and the other given yogurt sweetened with saccharin. Their findings were astonishing. In 14 days, they found that the group of rats that were fed artifical sweeteners had had their appetites stimulated and actually ate more food. Obviously, this increased their weight and body fat. Also, it was found that the core body temperature of the rats fed the artificial sweeteners decreased subsequently slowing down their metabolism. These are significant findings because this would indicate that the same thing is taking place in humans: lowered temperatures will slow down the metabolism thus making it difficult, if not impossible to lose weight. Medical cannabis has several potential beneficial effects.[8][9] Cannabinoids can serve as appetite stimulants, antiemetics, antispasmodics, and have some analgesic effects,[1] may be helpful treating chronic non-cancerous pain, or vomiting and nausea caused by chemotherapy. The drug may also aid in treating symptoms of AIDS patients.[citation needed] The U.S. Food and Drug Administration (FDA) has not approved smoked cannabis for any condition or disease as it deems evidence is lacking concerning safety and efficacy of cannabis for medical use.[10] The FDA issued an 2006 advisory against smoked medical cannabis stating: marijuana has a high potential for abuse, has no currently accepted medical use in treatment in the United States, and has a lack of accepted safety for use under medical supervision.[10] The National Institute on Drug Abuse (NIDA) states that Marijuana itself is an unlikely medication candidate for several reasons: (1) it is an unpurified plant containing numerous chemicals ...

Some evidence suggests zinc may be helpful for age related macular degeneration, as an appetite stimulant, to help with bad breath, helps boils, burns, and cancer sores (indeed many skin conditions), and helps with cancer prevention and chemotherapy side effects. Zinc seems to help anything involving brain function or lack of it, serious diseases like celiac, cystic fibrosis, Downs syndrome, or any disorder affecting the immune system, including HIV, malaria, and parasitic infections. Diabetics have better blood sugar control with adequate zinc intake, and it can reduce nerve pain. Zinc supplementation has shown improved HDL to LDL ratios, improves thyroid function, helps inflammatory bowel disease and can improve kidney and liver function. This sound very familiar to me. I am taking care of my daughter who has had a similar course of treatment and had spells of vomiting more than three weeks after chemo. In addition she had vomiting due to the mucositis caused by the radiation. The doctors were all flummoxed.. Since everybody reacts differently, there is no way to extrapolate from our experience. Nevertheless, it might be useful to hear all the things we tried. She had the same anti-emetics (Reglan, Compasene, Zofran, and Ativan) and they didnt do any good. Kytril (granisetron) did seem more effective than Zofran (ondansetron). They are both 5HT3 serotonin antagonists but many have reported the Kytril works better for delayed nausea. Early in the treatment, marijuana was effective when all else failed. But eventually her throat became too sore to tolerate the smoke. Marinol has the same active ingredient as marijuana but was not effective because it was too strong an appetite stimulant.. The one thing that always stopped the ...
Digger is an older boy I took in as a stray some years ago. His previous life is a mystery, but as I got his health checked, we discovered kidney disease, a heart murmur, and severe dental decay. The veterinarian advised that we get a handle on his kidney disease before exploring options for dental surgery. This began a long process of many different medications and supplements. Kidney disease is complicated. It causes dehydration (which in turn, causes constipation), loss of protein through urine, weight loss, nausea, and other symptoms. He currently takes... Epakitin to reduce phosphate levels. Hilarys Renal Blend for overall kidney support. Daily IV fluids for hydration (shown in photos). A probiotic blend for digestion. Gabapentin for oral pain (until surgery). As well as an anti-nausea and appetite stimulant if he stops eating. Im happy to say that after imploring these medications and therapies, we have his kidney disease under control for now. Unfortunately, kidney disease is ...

Sutherlandia frutescens. Sutherlandia frutescens is a popular traditional medicine and derives its common name, cancer bush from its long established (since 1895) use as a cancer tonic. Van Wyk and Albrecht16 currently are working on this plant.. Other uses of the plant include its use as an appetite stimulant (e.g. in persons with HIV/AIDS) and for the treatment of dysentry, kidney conditions, diabetes, internal cancers, liver conditions, uterine-related conditions, and stress and anxiety. The constituents of the plant are a high level of amino acids, canavanine (a non-protein amino acid), the cyclitol pinitol, a variety of flavonols, and triterpene glycosides (cycloartanes).. The published biological acitvities of these compounds confirm the traditional uses of the plant. Hi Terry,. Im sorry to hear about your daughter. I did carbo/taxol every three weeks for three rounds. I did not find that it was so unbearable, I seemed to tolerate it fairly well. What are your daughters complaints with the side effects? Nausea/vomiting may mean she needs to try a different anti-nausea med. There are also appetite stimulants that can be prescribed. If the bone/joint pain was an issue try L-Glutamine poweder three times a day, movement/walking/stretching, and make sure that her doctor is aware of her discomfort so that he can authorize appropriate pain relief for her.. Carbo/taxol, as far as I know, does not cause damage to hearing or sight. Cisplatin is known to cause hearing loss, which is similar to carboplatin, but carboplatin is much less likely to cause hearing loss. Carboplatin also is less likely to damage kidney than cisplatin though I beleive it causes more myelosuppression. I hope she is feeling better soon. I know she is strong and can make it through the treatment. ...

Question: I was searching your site for more information about the use of cyproheptadine as an appetite stimulant. I recently had an evaluation with a kiddo who is taking this twice a day to increase appetite but mom doesnt think it has helped much. Wondering what your team does for dosage? I have had some experience working with periactin and I remember that it was usually best to be on it for a period and then off again. Does cyproheptadine work the same way?. Answer: Cypropheptadine (or periactin) is an antihistamine (similar to benadryl), which frequently has the side effects of increase in hunger. Typically this medication is used only after a child is very comfortable having no symptoms of reflux ( such as vomiting, gagging, choking), as well has constipation managed ( stooling 1-2 times per day pudding like stools). It is started to help jump start a childs appetite. Often if a childrens GI symptoms are not medically managed there is not much benefit seen from the periactin.. The ...
1. Neuropeptide Y is a potent appetite stimulant and has been found to modulate glucose metabolism when given chronically. The acute effects of neuropeptide on peripheral glucose handling have not been studied in detail. We have studied the acute effects of central nervous system injection of neuropeptide on glucose metabolism in vivo in the rat.. 2. Rats implanted with chronic cannulae in the third cerebral ventricle were injected with either neuropeptide Y or saline and peripheral insulin sensitivity was assessed during a hyperinsulinaemic euglycaemic clamp. The effect of centrally injected neuropeptide Y on post-absorptive glucose metabolism was studied using a constant infusion of [6-3H]glucose.. 3. Infusion of neuropeptide Y resulted in a 18% increase in glucose requirement during the clamp, suggesting increased peripheral tissue responsiveness to insulin. Neuropeptide Y injection in 10 h fasted rats increased plasma glucose (area under curve 9.9 ± 0.2 versus 9.1 ± 0.1 mmol h−1 I−1, P ...

Coriander appears in so many different cuisines that it can be hard to determine where the plant first originated. We know by its Latin name, Coriandrum sativum, that coriander has been cultivated by humans for a long time (sativum means cultivated in Latin). Coriander seed essential oil is commonly used today as a appetite stimulant, energizer, and detoxifying oil for the whole body.. Some of our pigs have been enthusiastic eaters of comfrey, which is supposed to be an appetite stimulant for them as well as a nutritious feed. So have some of our goats and all of our chickens. Weve never offered them comfrey only; its always been part of a wide range of options. Weve offered comfrey to our rabbits, but they usually refuse to eat it. I have read that animals are more willing to eat comfrey after it has wilted for a day. This seems to work for some farmers; our animals either eat it fresh or dont eat it at all. I have also read that comfrey can be dried and fed, but none of our animals have been willing to eat dried comfrey, although theyre happy to tear into dried burdock.. Despite the complexity of the interaction between improved nutrition and body mass in ACS, our primary treatments for ACS have targeted improvement in appetite stimulation. One reason is that weight loss isnt the only manifestation of ACS: patients and family members often find it upsetting for a patient to not be able to participate in meals, and not eating is a regular reminder several times per day of the effects of the cancer.. One of the most commonly used treatments for primary ACS is a hormonal treatment in the class called progestins: typically megestrol acetate, or Megace. There are certainly studies that show weight gain with megace in patients with cancer, but this is more in fat than muscle/lean body mass. Some studies have used daily doses of just 180 mg, which can be enough to increase appetite, oral intake, and weight, its more common to give higher doses in the range of 480-800 mg/day. I often give a concentrated version called megace ES (for extra strength), which is typically ...

Phytochemical Aspects and Therapeutic Perspective of Cannabinoids in Cancer Treatment. By Sanda Vladimir‐Knežević, Biljana Blažeković, Maja Bival Štefan and Marija Kindl. Cannabinoids comprise the plant‐derived compounds and their synthetic derivatives as well as endogenously produced lipophilic mediators. Phytocannabinoids are terpenophenolic secondary metabolites predominantly produced in Cannabissativa L. The principal active constituent is delta‐9‐tetrahydrocannabinol (THC), which binds to endocannabinoid receptors to exert its pharmacological activity, including psychoactive effect. The other important molecule of current interest is non‐psychotropic cannabidiol (CBD). Since 1970s, phytocannabinoids have been known for their palliative effects on some cancer‐associated symptoms such as nausea and vomiting reduction, appetite stimulation and pain relief. More recently, these molecules have gained special attention for their role in cancer cell proliferation and death. A ...
Each season and sport brings its own set of challenges -- from temperature regulation, to adequate nutrition and fluids for peak athletic performance. While major concerns for cold weather exercisers include dressing appropriately to avoid hypothermia and excessive sweating, what you eat and drink before and during exercise can help you perform your best while staying comfortable and safe in extreme temperatures. In fact, proper nutrition helps regulate your core temperature and provide fuel for working muscles.. When exercising in cold temperatures, your metabolism increases slightly to warm the body and keep your internal temperature in the normal range. A slight drop in body temperature stimulates appetite. While this appetite stimulation is typically not enough to substantially increase hunger, you may want to have an extra snack for cold days when practices or games are scheduled outdoors.. Contrary to what you may think, the biggest nutritional challenge for cold weather athletes is ample ...

Amylopectin A-The highly-digestible complex carbohydrate that is no better-worse, in fact-than table sugar.. So why not eat non-wheat grains all you want? If they dont cause appetite stimulation, behavioral outbursts in children with ADHD, addictive consumption of foods, dementia (i.e., gluten encephalopathy), etc., why not just eat them willy nilly?. Because they still increase blood sugar. Conventional wisdom is that these foods trend towards having a lower glycemic index than, say, table sugar, meaning they raise blood glucose less.. Thats true . . . but very misleading. Oats, for instance, with a glycemic index of 55 compared to table sugars 59, still sends blood sugar through the roof. Likewise, quinoa with a glycemic index of 53, will send blood sugar to, say, 150 mg/dl compared to 158 mg/dl for table sugar-yeah, sure, its better, but it still stinks. And thats in non-diabetics. Its worse in diabetics.. Of course, John Q. Internist will tell you that, provided your blood sugars ...
This was a study that compared quality of life in gastrointestinal cancer patients who knew their cancer diagnosis and who did not. The most striking findings of the study were the fact that patients who did not know their cancer diagnosis showed better conditions on a number of quality of life subscales as measured by the EORTC QLQ-C-30. The findings indicated that those who knew their diagnosis showed a significant lower degree of physical, social and emotional functioning. A study of lung cancer patients also showed comparable results [14]. Similarly in Turkey and India it has been demonstrated that psychiatric disorders occurred to lesser extent in patients who were not aware of their cancer diagnosis. The authors concluded that these patients had a more hopeful outlook to the outcome of treatment [8, 15]. However, studies from different cultural backgrounds suggested that awareness of cancer diagnosis and prognosis does not itself cause emotional distress and honest disclosure of truth does ...
