This study establishes the utility of protocol biopsies in recipients of living related donor grafts through a randomized controlled trial. Graft function (as reflected by serum creatinine and MDRD eGFR) was superior at 6 months and 1 year amongst patients who underwent protocol biopsies, but there was no difference in the incidence of clinical AR episodes. Protocol biopsies are useful in detecting subclinical rejection (SCR) in early posttransplant period (6-13). In one study, SCR on 14-day protocol biopsy predicted poorer 10-year graft survival (14).
