Histological and histochemical quantification of root resorption incident to the application of intrusive force to rat molars. (1/207)

This study was conducted to investigate the nature of root resorption resulting from intrusive forces applied to the rat lower molars, by means of histological and histochemical techniques with tartrate resistant acid phosphatase (TRAP). Thirty-eight 13-week-old Wistar strain male rats were used. Intrusive force was created by a fixed appliance which was adjusted to exert an initial force of 50 g for the duration of 1, 2, and 3 weeks. The degree of root resorption and distribution of TRAP positive cells were evaluated. On the root surface, the TRAP positive scores were low in the apical regions. Significant differences in the scores were found in the inter-radicular region of the roots between the experimental and control groups for the 2- and 3-week groups. More active resorption of bone occurred during the experimental period, as denoted by greater TRAP positive scores on the bone than on the root surface. Root resorption scores in the apical root region were larger in the 2- and 3-week groups than in the 1-week group. Significant differences in the root resorption scores were also found between the 1- and 3-week groups in the inter-radicular region, indicating that intrusive force application of a longer duration may lead to a higher frequency of root resorption. It is shown that, irrespective of the level of TRAP positive cells and root resorption scores, the degree of root resorption activity is higher in the apical root region than in the inter-radicular area. These results indicate that cellular cementum may be resorbed more easily because of its richer organic components and low mineralized structure.  (+info)

Longitudinal post-eruptive mandibular tooth movements of males and females. (2/207)

Unbiased estimates of post-eruptive eruption and migration of the mandibular teeth for large representative samples are presently unavailable. The purpose of this study was to evaluate pure tooth movements of untreated children and adolescents longitudinally. Lateral cephalograms of 214 French-Canadians, followed bi-annually between 8 and 15 years of age, were traced, and the positions of the mandibular permanent central incisors and first molars were digitized. Temporal changes in tooth position were evaluated relative to naturally stable mandibular reference structures, using the mandibular reference line for orientation. The statistical analyses included t-tests to assess gender differences and Pearson product-moment correlations to evaluate associations. The results showed that the incisors proclined significantly more for males (6 degrees) than females (3 degrees). The incisor tips displayed early mesial movements that were countered by later distal movements. The incisor apex showed a consistent pattern of distal migration between 8 and 15 years. Mandibular arch length decreased over the 7-year observation period. Rates of mesial molar migration accelerated until 11 years of age and then decelerated. There was no significant change in the mandibular occlusal plane angle between 8 and 15 years of age. Incisor eruption showed the greatest rates during adolescence, attaining peaks at approximately 12 years for females and 14 years for males. The molars erupted approximately 5 mm between 8 and 15 years of age. The greatest gender differences occurred at the older ages, with males showing greater eruption potential than females. It was concluded that the mandibular teeth show significant migration and eruption during childhood and adolescence, with gender differences in the amount, direction, and timing of movement.  (+info)

Blood vessel response to pan-endothelium (RECA-1) antibody in normal and tooth loaded rat periodontal ligament. (3/207)

Immunolabelling of the normal rat molar periodontal ligament (PDL) with RECA-1 antibody, an endothelial cell surface marker, demonstrated the endothelium in the different categories of blood vessels. The intensity of immunolabelling was similar for venous capillaries (VC), post-capillary-sized venules (PCV), and collecting venules (CV). Arterial capillaries (AC) and terminal arterioles (TA) showed a different response, both having a high intensity of endothelium and smooth muscle cell labelling, whether they were located in the PDL or alveolar bone. An experimental, continuous loading of approximately 100 g was applied unilaterally to the mandibular molars for 10 minutes. In the PDL apical compression zone this load resulted in a loss of RECA-1 immunolabelling of the VC, PCV, and CV. Adjacent to the alveolar crest, where shear and tension loads were judged to have occurred, there was enhanced immunoreactivity of VC, PCV, and CV. In the loaded PDL, the AC and TA, irrespective of their location in the ligament or bone, showed strong immunofluorescence of their endothelium and the enveloping smooth muscle layer. Vessel and PDL immunofluorescence were analysed with standardized grey scale densitometry, and the data subjected to ANOVA. Comparison between individual vessel means showed significant differences (P < 0.05). Control teeth showed no immunostaining difference between the coronal and apical region vessels, whereas in the loaded teeth the overall cervical vessel endothelium had a significantly higher value than the apical vessel endothelium (P < 0.001). These findings demonstrate that the endothelium of this microvascular bed can undergo significant immunoreactivity changes when exposed to short-term, continuous, tooth loading.  (+info)

Evaluation of apical root resorption following extraction therapy in subjects with Class I and Class II malocclusions. (4/207)

The purpose of this study was to determine the amount of root resorption during orthodontic treatment, and to examine the relationship between tooth movement and apical root resorption. Twenty-seven Class I and 27 Class II patients treated with edgewise mechanics following first premolar extractions were selected. The following measurements were made on the pre- and post-treatment cephalograms: upper central incisor to palatal plane distance, the inclination of upper central incisor to the FH and AP planes, the perpendicular distances from the incisor tip to the AP and PTV planes, and incisor apex to PTV. The amount of apical root resorption of the maxillary central incisors was determined for each patient by subtracting the post-treatment tooth length from the pre-treatment tooth length measured directly on cephalograms. Intra-group differences were evaluated by the Student's t-test and inter-group differences by the Mann-Whitney U-test. For correlations the Pearson correlation coefficient was used. The results show that there was a mean of approximately 1 mm (P < 0.01) of apical root shortening in Class I patients, but in Class II division I subjects the mean root resorption was more than 2 mm (P < 0.001). The inter-group differences were statistically significant. No significant correlations were found between the amount of apical root resorption and tooth inclination, or the duration of active treatment.  (+info)

Long-term follow-up of maxillary incisors with severe apical root resorption. (5/207)

The purpose of the study was to analyse the mobility of teeth with severe orthodontically induced root resorption, at follow-up several years after active treatment, and to evaluate mobility in relation to root length and alveolar bone support. Seventy-three maxillary incisors were examined in 20 patients, 10-15 years after active treatment in 13 patients (age 24-32 years) and 5-10 years after active treatment in seven patients (age 20-25 years). All had worn fixed or removable retainers; seven still had bonded twistflex retainers. Total root length and intra-alveolar root length were measured on intra-oral radiographs. Tooth mobility was assessed clinically according to Miller's Index (0-4) and the Periotest method. Crestal alveolar bone level, periodontal pocket depth, gingival, and plaque indices, occlusal contacts during occlusion and function, and dental wear were recorded. There was a significant correlation (P < 0.05) between tooth mobility, and total root length and intra-alveolar root length. No correlation was found between tooth mobility and retention with twistflex retainers. None of the variables for assessment of periodontal status, occlusion and function were related to total root length or tooth mobility. It is concluded that there is a risk of tooth mobility in a maxillary incisor that undergoes severe root resorption during orthodontic treatment, if the remaining total root length is < or = 9 mm. The risk is less if the remaining root length is > 9 mm. Follow-up of teeth with severe orthodontically induced root resorption is indicated.  (+info)

Anesthetic efficacy of infiltrations in mandibular anterior teeth. (6/207)

The purpose of this prospective, randomized, double-blind study was to measure the degree of anesthesia obtained with a labial infiltration of either 2% lidocaine with 1:50,000 or 2% lidocaine with 1:100,000 epinephrine in mandibular anterior teeth. Another objective was to measure the degree of anesthesia obtained with a lingual infiltration of 2% lidocaine with 1:100,000 epinephrine in mandibular anterior teeth. Through use of a repeated-measures design, 40 subjects randomly received a labial infiltration at the lateral incisor apex of either 1.8 mL of 2% lidocaine with 1:100,000 epinephrine or 1.8 mL of 2% lidocaine with 1:50,000 epinephrine at 2 separate appointments. An additional 40 subjects received a lingual infiltration at the lateral incisor apex of 1.8 mL of 2% lidocaine with 1:100,000 epinephrine. The mandibular anterior teeth were blindly pulp tested at 4-minute cycles for 60 minutes postinjection. No response from the subject to the maximum output (80 reading) of the pulp tester was used as the criterion for pulpal anesthesia. Anesthesia was considered successful when 2 consecutive 80 readings were obtained. For the 3 infiltrations, success rates for the lateral incisor ranged from 43 to 50%. Adjacent teeth had success rates of 27 to 63%. There was no significant difference (P > 0.05) in success between the labial infiltration of 2% lidocaine with 1:100,000 epinephrine and 2% lidocaine with 1:50,000 epinephrine or the lingual infiltration of 2% lidocaine with 1:100,000 epinephrine when compared with the labial infiltration of 2% lidocaine with 1:100,000 epinephrine. Duration of pulpal anesthesia declined steadily for all solutions over the 60 minutes. In conclusion, the success rate of 43-50% and declining duration of pulpal anesthesia over an hour indicates that a labial infiltration of 1.8 mL of either 2% lidocaine with 1:100,000 epinephrine or 1: 50,000 epinephrine or a lingual infiltration of 2% lidocaine with 1:100,000 epinephrine over the lateral incisor apex cannot be recommended clinically to provide profound pulpal anesthesia.  (+info)

SEM observations of resected root canal ends following apicoectomy. (7/207)

The purpose of this study was to examine the apical foramen of root apices extracted during apicotomies. A total of 25 teeth extracted from 25 patients admitted to the Department of Conservative Dentistry at Tokyo Dental College's Chiba Hospital were used for the study. All patients were between 22 to 56 years of age at the time of the study, and each of the 25 cases was determined clinically on radiographs to be chronic apical suppurative periodontitis. Microsurgery was performed on all cases, and the extracted root apices were then observed using SEM. The results demonstrated a wide opening, greater than 350 microns as measured along the major axis, of the apical foramen in 80% of the cases. Various characteristics indicative of resorption were observed around the apical foramen. These features included those believed to have been caused by overinstrumentation during root canal treatment as well as irregularly shaped areas presumed to be apical lesions that had enlarged and eroded. We observed a high frequency of manifestations of cementum resorption surrounding the root apices of teeth with apical lesions. Furthermore, we concluded that in the majority of cases in the present study, due to the fact that the apical foramen exceeded normal opening dimensions as a result of overinstrumentation during root canal treatment or resorption around the root apex, prolongation of the lesions had occurred in response to direct contact of microbial infectious matter and tissues surrounding the root apex over a large area. The above finding suggested that, in cases in which the apical foramen is destroyed through overinstrumentation larger than #35 or in which the apical foramen opens up to dimensions greater than 350 microns due to pathologic resorption, surgical intervention may be indicated. On the other hand, in 64% of the cases, an accessory canal was observed in the root apical lesion. Based on this observation, the presence of an accessory canal in the root apex may contribute to some degree to the prolongation of the lesion.  (+info)

The effect of Frankel's function regulator type III on the apical base. (8/207)

Dento-alveolar and skeletal effects of orthopaedic treatment have always been the subject of controversial scientific discussions. The objective of this retrospective study was to demonstrate the changes in the dental arch and the apical base of both jaws following therapy with Frankel's function regulator type III (FR III). For this purpose, 42 Class III patients (28 females, 14 males, mean age 7.5 years) were selected according to strict criteria. The control group consisted of 16 patients (eight females, eight males, mean age 8.3 years) with minor malocclusion symptoms. Study models of all patients at the beginning and end of treatment were evaluated using a sophisticated measuring system. Apart from common parameters of model analysis, cusp inclination of the first molars, and apical bases of the maxilla and mandible were recorded to facilitate a comprehensive evaluation of treatment effects (including growth). The FR III stimulated the development of the maxilla, thus resulting in a more physiological growth pattern. Mandibular prognathism, however, was still discernible after therapy in the Class III patients, even though mandibular growth did not differ significantly between the two groups.  (+info)
