Unusual maxillary first molar with 2 palatal canals within a single root: a case report. (1/305)

A case report is presented regarding a maxillary first molar with 5 canals. The morphology is atypical because it is characterized by a single palatal root with 2 canals with separate orifices joining in the apical third. A literature review pertaining to the morphology of maxillary first molars is discussed. Modifications to the normal access opening and examination of the pulpal floor for additional canals are stressed.  (+info)

Periapical surgery of 29 teeth. A comparison of conventional technique, microsaw and ultrasound. (2/305)

OBJECTIVES: A clinical and radiological study is made of the results obtained with different periapical surgical techniques. MATERIAL AND METHODS: Twenty-nine single-root teeth from 29 patients with periapical pathology following correct endodontic treatment were divided into three groups: (A) 10 patients in which rotary instruments were used to perform osteotomy, apicoectomy and cavity preparation; (B) 10 patients in which rotary instruments were used to perform osteotomy and apical resection, with ultrasound root-end cavity preparation; and (C) 9 patients in which microsaws were used for osteotomy, and ultrasound for cavity preparation. All teeth were filled with IRM. Clinical and radiological controls were made after 10 days, one and 6 months, and one year. RESULTS: Clinical success with the absence of symptoms was achieved in 17 cases (58.6%), while in 16 patients (55.2%) the X-ray images showed complete bony regeneration (complete healing). The clinical success rate after one year was 40%, 70% and 66.7% in groups A, B and C, respectively, while complete radiographic healing was recorded in 30%, 60% and 77.8%. There were three failures in group A, two in group C, and none in group B. CONCLUSION: In conclusion, the best results were obtained when ultrasound was used for root-end cavity preparation.  (+info)

SEM observations of resected root canal ends following apicoectomy. (3/305)

The purpose of this study was to examine the apical foramen of root apices extracted during apicotomies. A total of 25 teeth extracted from 25 patients admitted to the Department of Conservative Dentistry at Tokyo Dental College's Chiba Hospital were used for the study. All patients were between 22 to 56 years of age at the time of the study, and each of the 25 cases was determined clinically on radiographs to be chronic apical suppurative periodontitis. Microsurgery was performed on all cases, and the extracted root apices were then observed using SEM. The results demonstrated a wide opening, greater than 350 microns as measured along the major axis, of the apical foramen in 80% of the cases. Various characteristics indicative of resorption were observed around the apical foramen. These features included those believed to have been caused by overinstrumentation during root canal treatment as well as irregularly shaped areas presumed to be apical lesions that had enlarged and eroded. We observed a high frequency of manifestations of cementum resorption surrounding the root apices of teeth with apical lesions. Furthermore, we concluded that in the majority of cases in the present study, due to the fact that the apical foramen exceeded normal opening dimensions as a result of overinstrumentation during root canal treatment or resorption around the root apex, prolongation of the lesions had occurred in response to direct contact of microbial infectious matter and tissues surrounding the root apex over a large area. The above finding suggested that, in cases in which the apical foramen is destroyed through overinstrumentation larger than #35 or in which the apical foramen opens up to dimensions greater than 350 microns due to pathologic resorption, surgical intervention may be indicated. On the other hand, in 64% of the cases, an accessory canal was observed in the root apical lesion. Based on this observation, the presence of an accessory canal in the root apex may contribute to some degree to the prolongation of the lesion.  (+info)

Cervical shaping in curved root canals: comparison of the efficiency of two endodontic instruments. (4/305)

The aim of this study was to determine the removal of dentin produced by number 25 (0.08) Flare files (Quantec Flare Series, Analytic Endodontics, Glendora, California, USA) and number 1 e 2 Gates-Glidden burs (Dentsply - Maillefer, Ballaigues, Switzerland), in the mesio-buccal and mesio-lingual root canals, respectively, of extracted human permanent inferior molars, by means of measuring the width of dentinal walls prior and after instrumentation. The obtained values were compared. Due to the multiple analyses of data, a nonparametric test was used, and the Kruskal-Wallis test was chosen. There was no significant difference between the instruments as to the removal of dentin in the 1st and 2nd millimeters. However, when comparing the performances of the instruments in the 3rd millimeter, Flare files promoted a greater removal than Gates-Glidden drills (p > 0.05). The analysis revealed no significant differences as to mesial wear, which demonstrates the similar behavior of both instruments. Gates-Glidden drills produced an expressive mesial detour in the 2nd and 3rd millimeters, which was detected trough a statistically significant difference in the wear of this region (p > 0.05). There was no statistically significant difference between mesial and lateral wear when Flare instruments were employed.  (+info)

Apical seal of root canals with gutta-percha points with calcium hydroxide. (5/305)

The objective of this research was to determine if gutta-percha points with calcium hydroxide [Ca(OH)2] improve the apical seal after root canal filling and if the master point does it alone. Human single recently extracted teeth were biomechanically prepared and the root canals filled by the lateral condensation technique with ZOE and gutta-percha points, with or without calcium hydroxide. The teeth were placed into a 2% methylene blue solution in a vacuum environment for 24 h after which they were processed for stereomicroscope evaluation. Better results were observed with the teeth filled with gutta-percha points with calcium hydroxide (p=0.01). We conclude that these new points make a better apical seal and that these results can also be obtained with the calcium hydroxide master point associated with regular ones (p=0.05).  (+info)

Morphometric analysis of the effectiveness of different concentrations of sodium hypochlorite associated with rotary instrumentation for root canal cleaning. (6/305)

This study evaluated the cleaning capacity of Profile.04 instruments with different concentrations of NaOCl solutions on flattened root canals. Twenty central mandibular incisors were divided into 4 groups: Group I: 0.5% NaOCl; Group II: 1% NaOCl; Group III: 5% NaOCl; Group IV: distilled and deionized water. After chemomechanical preparation, the teeth were submitted to histological preparation and morphometric analysis. Morphometric analysis showed that the 5% NaOCl was significantly more efficient in cleaning the root canal than 0.5% NaOCl and distilled water, which had the lowest levels of cleaning of the root canals (p<0.01).  (+info)

Histological study of the effect of some irrigating solutions on bacterial endotoxin in dogs. (7/305)

The aim of this study was to evaluate, histopathologically, the effectiveness of mechanical preparation of root canals using different irrigating solutions in dog teeth filled with LPS after pulpectomy. A total of 120 root canals of 6 mongrel dogs were filled with a solution of LPS after pulpectomy. The irrigating solutions used were saline, 1, 2.5, and 5% sodium hypochlorite, and 2% chlorhexidine. No irrigation was used in the control group. The animals were sacrificed after 60 days and the teeth were fixed and demineralized. Subsequently, serial 6-microm sections were stained with hematoxylin and eosin and Mallory's trichrome for histopathological analysis and Brown-Brenn for verification of bacterial contamination. Analysis showed that the inflammatory infiltrate was statistically less intense in the groups in which the root canals were irrigated with 5% sodium hypochlorite and 2% chlorhexidine. However, none of the irrigating solutions completely inactivated the harmful effects of LPS. Mechanical preparation associated with different irrigating solutions did not completely inactivate LPS.  (+info)

In vitro evaluation of the thermal alterations on the root surface during preparation with different Ni-Ti rotary instruments. (8/305)

The present study evaluated, in vitro, the temperature alterations on the external root surface during instrumentation with four different rotary systems. A total of 20 extracted human maxillary lateral incisors were instrumented using either the ProFile, MicroMega, Quantec or K3 systems and the thermal alterations on the root surface were recorded by means of three thermocouples attached to the coronal, middle and apical portions of the root. Mean temperature increases no higher than 0.4 degrees C +/- 1.0 degrees C (ProFile system) were recorded, which indicates that these instruments are safe for the surrounding periodontal tissues.  (+info)
