Neuromonitoring in thyroid surgery: prospective evaluation of intraoperative electrophysiological responses for the prediction of recurrent laryngeal nerve injury. (17/68)

OBJECTIVE: We evaluated the ability of neuromonitoring to predict postoperative outcome in patients undergoing thyroid surgery for different indications. SUMMARY BACKGROUND DATA: Neuromonitoring has been advocated to reduce the risk of vocal cord palsy and to predict postoperative vocal cord function. METHODS: Three hundred twenty-eight patients (502 nerves at risk) were studied prospectively at a single center. Neuromonitoring was performed with the Neurosign 100 device by transligamental placement of the recording electrode into the vocalis muscles. Cumulative distribution of stimulation thresholds was determined by stepwise decreases in current (1 mA to 0.05 mA) for both the vagus and the recurrent nerve. Patients were grouped according to surgical risk (benign and malignant disease, reoperation for benign and for malignant disease). RESULTS: If the electrophysiological response was correlated to postoperative vocal cord function, the sensitivity of neuromonitoring was modest (86% in surgery for benign disease) to low (25% in reoperation for malignant disease); the positive predictive value was modest (overall rate 62%) but acceptable (87%) if corrected for technical problems. Specificity and negative predictive values were high (ie, overall >95%). Stimulation thresholds were not augmented in 11 patients, in whom postoperative palsy developed despite normal intraoperative recordings. Similarly, an electrical field response was elicited in 14 of 21 patients with preoperative vocal cord palsy. Electromyographic recordings did not reveal an abnormal amplitude or a decline in nerve conduction velocity. CONCLUSIONS: Neuromonitoring is useful for identifying the recurrent laryngeal nerve, in particular if the anatomic situation is complicated by prior surgery, large tissue masses, aberrant nerve course. However, neuromonitoring does not reliably predict postoperative outcome.  (+info)

Influence of muscle relaxation on neuromonitoring of the recurrent laryngeal nerve during thyroid surgery. (18/68)

BACKGROUND: The influence of muscle relaxation on the intra-operative neuromonitoring of the recurrent laryngeal nerve during thyroid surgery is unclear. METHODS: In a prospective study involving 200 patients undergoing elective thyroid surgery, the influence of muscle relaxation on neuromonitoring of the recurrent laryngeal nerve was investigated. The patients received balanced anaesthesia with oxygen-nitrous oxide-isoflurane, and rocuronium bromide was used as the non-depolarizing neuromuscular blocking agent. The degree of relaxation was monitored continuously by accelerometry [twitch (% TW)]. Summed action potentials (SAcP) obtained from the vocalis muscle were characterized by the area under the electromyographic curve expressed in millivolt seconds. RESULTS: Evoked potentials were obtainable in all patients and at all time points. With decreasing neuromuscular blockade a significant increase in the potentials evoked at the vocalis muscle was observed. At 0% TW SAcP was 1.27 (SD 1.02) mV s. An increase in TW to 10% was accompanied by an increase in SAcP to 2.68 (2.01) mV s (P<0.01). At a TW of 25%, mean SAcPs of 5.08 mV s were recorded. CONCLUSIONS: There was a significant difference in the degree of relaxation of the adductor pollicis muscle and the vocalis muscle. The laryngeal muscles exhibited a shorter response time than the adductor pollicis and recovered more quickly. These results confirm the feasibility of intra-operative neuromonitoring of the recurrent laryngeal nerve during neuromuscular blockade.  (+info)

Capsaicin-induced activation of pulmonary vagal C fibers produces reflex laryngeal closure in the rat. (19/68)

Our recent studies show that intravenous administration of capsaicin induces enhancement of the intralaryngeal thyroarytenoid (TA) branch but a reduction of the intralaryngeal abducent branch, suggesting that the glottis is likely closed by capsaicin. The aim of the present study was to examine whether the glottis is adducted by intravenous administration of capsaicin. Electromyographic (EMG) activity of the TA muscle, subglottal pressure (SGP), and glottal behavior were evaluated before and after intravenous administration of capsaicin in male Wistar rats that were anesthetized and tracheostomized. Catheters were placed in the femoral artery and vein, as well as in the right jugular vein. Low and high doses of capsaicin (0.625 and 1.25 microg/kg) produced apnea and increases in the amplitude of the TA EMG. This enhancement of the TA EMG was observed during apnea as well as during recovery from apnea. Moreover, the onset of the TA EMG was advanced such that it commenced earlier during inspiration. Concomitantly, the SGP substantially increased. Increases in both the TA EMG and SGP were abolished after bilateral sectioning of the recurrent laryngeal nerve. In some animals, movement of the vocal folds was recorded by taking a motion picture with a digital camera under a surgical microscope. With intravenous administration of capsaicin, a tight glottal closure, decreases in blood pressure, and bradycardia were observed. These results strongly suggest that glottal closure is reflexively induced by intravenous administration of capsaicin and that closure of the glottis is beneficial for the defense of the airway and lungs when an animal is exposed to environmental irritants.  (+info)

A case of Hodgkin's disease presenting with recurrent laryngeal nerve palsy and tracheoesophageal fistula. (20/68)

Malignancy is the most common cause of tracheoesophageal fistulas. The malignancies commonly implicated in the development of tracheoesophageal fistulas are primary bronchial or esophageal carcinomas. Hodgkins disease rarely leads to such a fistula. We report a case of Hodgkin's disease with a tracheoesophageal fistula as well as a left recurrent nerve palsy at presentation. This presentation has no precedent in literature. The patient was treated with systemic chemotherapy and involved field radiotherapy. On follow up 1 year after the completion of treatment, he was clinically and radiologically disease free.  (+info)

Vecuronium neuromuscular blockade at the adductor muscles of the larynx and adductor pollicis. (21/68)

The differences between neuromuscular blockade of the adductor muscles of the vocal cords and the adductor pollicis were examined in 20 adult women anesthetized with fentanyl and propofol. Vecuronium 0.04 or 0.07 mg/kg was given as a single bolus by random allocation. The force of contraction of the adductor pollicis was recorded. Laryngeal response was measured as pressure changes in the cuff of the tracheal tube positioned between the vocal cords. Train-of-four stimulation was applied to the recurrent laryngeal nerve at the notch of the thyroid cartilage and to the ulnar nerve at the wrist. Neuromuscular blockade had a faster onset, was less intense, and recovered more rapidly at the vocal cords. With 0.04 mg/kg, maximum blockade of first twitch (T1) was 55 +/- 8 (mean +/- standard error of the mean [SEM]) and 88 +/- 4% at the vocal cords and the adductor pollicis, respectively (P = 0.006). Onset time was 3.3 +/- 0.1 and 5.7 +/- 0.2 min, respectively (P = 0.000001), and time to 90% T1 recovery was 11.3 +/- 1.6 and 26.1 +/- 1.8 min, respectively (P = 0.001). With 0.07 mg/kg, onset time was unchanged; maximum blockade was more intense, being 88 +/- 4 and 98 +/- 1%, respectively (P = 0.04 between muscles); and time to 90% T1 recovery was 23.3 +/- 1.8 min at the vocal cords versus 40.3 +/- 2.9 min at the adductor pollicis (P = 0.001). Approximately 1.73 times as much vecuronium was required at the larynx compared with the dose required at the adductor pollicis for the same intensity of blockade.(ABSTRACT TRUNCATED AT 250 WORDS)  (+info)

Endoluminal stenting of mycotic saccular aneurysm at the aortic arch. (22/68)

Endovascular grafts have been widely used for the treatment of aneurysms since the early 1990s. They are preferred especially for use in patients in whom conventional surgical methods carry high risks of death and morbidity. Increasing operator experience and technical refinements in endovascular grafting have enabled these procedures to be performed even in critical segments of the aorta, such as the thoracic and arch levels. In this report, we present the case of a patient who was treated successfully with an endovascular graft for a mycotic saccular aneurysm located just below the left subclavian artery.  (+info)

Electrophysiological characterization of vagal afferents relevant to mucosal nociception in the rat upper oesophagus. (23/68)

Emerging evidence indicates a nociceptive role of vagal afferents. A distinct oesophageal innervation in the rat, with muscular and mucosal afferents travelling predominantly in the recurrent (RLN) and superior laryngeal nerve (SLN), respectively, enabled characterization of mucosal afferents with nociceptive properties, using novel isolated oesophagus-nerve preparations. SLN and RLN single-fibre recordings identified 55 and 14 units, respectively, with none conducting faster than 8.7 m s(-1). Mucosal response characteristics in the SLN distinguished mechanosensors (n = 13), mechanosensors with heat sensitivity (18) from those with cold sensitivity (19) and a mechanoinsensitive group (5). The mechanosensitive fibres, all slowly adapting, showed a unimodal distribution of mechanical thresholds (1.4-128 mN, peak approximately 5.7 mN). No difference in response characteristics of C and Adelta fibres was encountered. Mucosal proton stimulation (pH 5.4 for 3 min), mimicking gastro-oesophageal reflux disease (GORD), revealed in 31% of units a desensitizing response that peaked around 20 s and faded within 60 s. Cold stimulation (15 degrees C) was proportionally encoded but the response showed slow adaptation. In contrast, the noxious heat (48 degrees C) response showed no obvious adaptation with discharge rates reflecting the temperature's time course. Polymodal (69%) mucosal units, > 30% proton sensitive, were found in each fibre category and were considered nociceptors; they are tentatively attributed to vagal nerve endings type I, IV and V, previously morphologically described. All receptive fields were mapped and the distribution indicates that the posterior upper oesophagus may serve as a 'cutbank', detecting noxious matters, ingested or regurgitated, and triggering nocifensive reflexes such as bronchoconstriction in GORD.  (+info)

We report our experience of using the laryngeal mask airway (LMA) in 13 consecutive patients undergoing thyroid surgery and discuss the advantages and limitations of the technique.
