Prevention of early childhood caries in North Carolina medical practices: implications for research and practice. (33/364)

Early childhood caries is a significant public health problem in low-income children, with important negative consequences for the child and the family. The purpose of this paper is to describe the development, implementation, and preliminary outcomes of preventive dentistry programs in North Carolina that target low-income children from birth to thirty-five months of age. The focus is on Into the Mouths of Babes, a statewide program in which pediatricians, family physicians, and providers in community health clinics are reimbursed by Medicaid to provide preventive dental services for children (risk assessment, screening, referral, fluoride varnish application) and caregivers (counseling). The provider intervention includes continuing medical education lectures and interactive sessions, practice guidelines for the patient interventions, case-based problems, practical strategies for implementation, a toolkit with resource materials, and follow-up training. In the first two years of the statewide program, 1,595 medical providers have been trained. The number of providers billing for these services has steadily increased, and by the last quarter of 2002, the number of visits in which preventive dental services were provided in medical offices reached 10,875. A total of 38,056 preventive dental visits occurred in medical offices in 2002. By the end of 2002, only sixteen of the state's one hundred counties had no pediatrician, family physician, or local health department participating. The preliminary results from this program demonstrate that nondental professionals can integrate preventive dental services into their practices. The program has increased access to preventive dental services for young Medicaid children whose access to dentists is restricted. Assessments of effectiveness and cost-effectiveness of both the provider and patient interventions are under way.  (+info)

Fluoride release by restorative materials before and after a topical application of fluoride gel. (34/364)

The release of fluoride from restorative materials (Vitremer, Ketac-Fil, Fuji II LC and Freedom) was evaluated during two 15-day periods, before and after a topical application of acidulated phosphate fluoride gel (APF). For each material, 6 specimens were made, which were immersed in 2 ml of deionized water. The fluoride concentration dosages in the solutions were read at intervals of 24 hours for 15 days. After this period, the specimens of each material received treatment with APF gel for 4 minutes and the fluoride released was analyzed at 24-hour intervals during the following 15 days. The analysis of variance and the Tukey test (p < 0.05) showed that the total mean fluoride released during the initial 15 days was greater for Vitremer and Ketac-Fil and lower for Fuji II LC and Freedom; and in the final 15 days there was a difference in release readings, with the greatest value for Vitremer, followed by Fuji II LC, Ketac-Fil and Freedom. The comparison of the results between the 1st day and the 16th day (after gel application) showed a greater fluoride release on the 16th day for Vitremer, Fuji II LC and Freedom and was equal for Ketac-Fil. Although all the materials evaluated gained fluoride with the application of APF, the data suggest that the resin-modified ionomers are more efficient in releasing fluoride to the medium than the other materials.  (+info)

The influence of albumin on corrosion resistance of titanium in fluoride solution. (35/364)

Proteins can interact with corrosion reactions in several ways. In this study, we investigated the effect of albumin on the corrosion resistance of titanium in the presence of fluoride. The effects of the NaF concentration, albumin concentration, and pH on the corrosion characteristics of commercially pure titanium (CP-Ti) were examined by means of electrochemical techniques. The corrosion resistance of titanium decreased as the NaF concentration increased and as pH decreased. The corrosion resistance of titanium in NaF solutions was improved in the presence of albumin. The natural electrode potential was elevated, and the passive current density was reduced by albumin at a concentration of 0.01%. The polarization resistance rose with increased concentrations of albumin in fluoride solution. These results showed that the albumin in saliva and dental plaque affected the corrosion resistance of CP-Ti in fluoride solution.  (+info)

Fluoridated elastomers: in vivo versus in vitro fluoride release. (36/364)

OBJECTIVES: To compare (i) the in vivo release of fluoride from fluoridated elastomers to the in vitro release, and (ii) the residual fluoride content of the elastomers after 1 week in the mouth with and without fluoride toothpaste and mouthrinse. DESIGN: A prospective, longitudinal, cross-over study. SUBJECTS AND METHOD: Six subjects were recruited by poster to take part in the study. Each subject had one premolar in each quadrant to which a bracket could be fixed and exemplary oral hygiene. Elastomers were then placed on these brackets. INTERVENTION: The study was divided into two parts: (i) subjects used oral hygiene products with fluoride and (ii) oral hygiene products with fluoride were excluded. Both groups of elastomers were left in the mouth for 1 week. After collection the elastomers were stored in distilled water. MAIN OUTCOME MEASURES: The amount of residual fluoride in the ligatures after they have been placed in the mouth for 1 week was compared with the cumulative fluoride release in vitro over 1 week and 6 months. RESULTS: Only 13 per cent of the total amount of fluoride in fluoridated elastomers was released during the first week in vitro, compared with 90 per cent in vivo. There was a significantly greater amount (P = 0.001) of residual fluoride when the elastomers were in the mouth for 1 week in the presence of fluoride toothpaste and mouthrinse, than when fluoride supplements were excluded. CONCLUSIONS: (1) Higher levels of fluoride are lost from the fluoride elastomers in vivo than in vitro during the first week. (2) A significantly greater amount of residual fluoride was released from the elastomers placed in the mouth when fluoride toothpaste and mouthrinse were used.  (+info)

Effect of fluoride exposure on cariostatic potential of orthodontic bonding agents: an in vitro evaluation. (37/364)

AIMS: The aims of this in vitro study were to compare the cariostatic potential of a resin modified glass ionomer cement (Fuji Ortho LC) to that of a resin control (Transbond) for bracket bonding and to compare the effect of extrinsic fluoride application on the cariostatic potential of each material. SETTING: Ex vivo study. MATERIALS AND METHODS: Orthodontic brackets were bonded to 40 extracted premolars, 20 with Fuji Ortho LC and 20 with Transbond. The teeth were subjected to pH cycling, pH 4.55, and pH 6.8, over a 30-day period. Ten teeth bonded with each material were immersed in a 1000 ppm fluoride solution for 2 minutes each day. Fluoride release was measured throughout the study from all teeth. After 30 days, the teeth were assessed visually for signs of enamel decalcification. RESULTS: Significant differences in decalcification existed macroscopically between all four groups of teeth, with the exception of those bonded with Fuji Ortho LC alone compared with Transbond alone (P = 0.22), and Fuji Ortho LC alone compared with Transbond with added fluoride (P = 0.3). Fluoride release from Fuji Ortho LC alone fell to minimal values, but with the addition of extrinsic fluoride the levels fell initially and then followed an upward trend. There was minimal fluoride release, from Transbond alone, but with daily addition of extrinsic fluoride, subsequent fluoride release was increased. Significant differences existed in the amount of fluoride released between all groups, except comparing Fuji Ortho LC alone and Transbond with added fluoride. CONCLUSIONS: The results of this study have indicated that with an in vitro tooth-bracket model, the creation of white spot inhibition could best be achieved by the use of a resin-modified glass ionomer cement, supplemented with fluoride exposure. The least protection was afforded by the composite control. The resin-modified glass ionomer cement alone and the composite with added fluoride demonstrated equivalent protection.  (+info)

A modified pH-cycling model to evaluate fluoride effect on enamel demineralization. (38/364)

Since in vitro pH-cycling models are widely used to study dental caries, they should allow evaluations of fluoride effect on early stages of caries development. Therefore, acid etching on enamel surface must be avoided, enabling surface microhardness (SMH) analysis. In the present study, the pH-cycling model originally described by Featherstone et al.9 (1986) was modified to preserve the enamel surface and to produce early carious lesions that could be evaluated using SMH and cross-sectional microhardness (CSMH) measurements. In order to validate this modified model, a dose-response evaluation with fluoride was made. Human enamel blocks with known SMH were submitted to such regimen with the following treatments: distilled deionized water (DDW; control) and solutions containing 70, 140 and 280 ppm F. Data from %SMH change and deltaZ (mineral loss) showed a statistically significant negative correlation between F concentration in treatment solutions and mineral loss. In conclusion, the modified pH-cycling model allowed the evaluation of changes on the outermost enamel layer during caries development, and a dose-response effect of fluoride reducing enamel demineralization was observed.  (+info)

Evaluation of the fluoride stability of dentifrices sold in Manaus, AM, Brazil. (39/364)

The minimum requirement for the anti-caries effect of a dentifrice is the presence of available and stable fluoride in the formulation. The concentration of available fluoride in the major dentifrices sold in Brazil has been reported, but few data have been published about its stability, which is temperature-dependent. Thus, the aim of this study was to evaluate the concentration and stability of fluoride in dentifrices sold in Manaus, AM, Brazil, which is a typical tropical city. The concentrations of total fluoride, total soluble fluoride, MFP, and of insoluble fluoride of six Brazilian dentifrices and an imported one were analyzed. The analyses were made when the dentifrices were purchased and during a year of storage at room temperature (28.9 1.16 C) and under refrigeration (26.3 0.88 C). The analyses were performed using an ion specific electrode Orion 96-09. At the time of purchase, all the dentifrices analyzed showed more than 1,000 ppm (mug F-/g; w/w) of soluble fluoride. However, in most of them, this form of fluoride was not shown to be stable. The highest loss of soluble fluoride was found for storage at room temperature, reaching up to 40%. Although all dentifrices comply with the Brazilian guidelines with regard to the concentration of total fluoride (maximum of 0.15%), the instability of soluble fluoride observed in some of them can impair their anti-caries effect and this condition is not contemplated in the Governmental rules.  (+info)

In vitro evaluation of human dental enamel surface roughness bleached with 35% carbamide peroxide and submitted to abrasive dentifrice brushing. (40/364)

The aim of this in vitro study was to evaluate the surface roughness of human enamel bleached with 35% carbamide peroxide at different times and submitted to different superficial cleaning treatments: G1 - not brushed; G2 - brushed with fluoride abrasive dentifrice; G3 - brushed with a non-fluoride abrasive dentifrice; G4 - brushed without dentifrice. Sixty fragments of human molar teeth with 4 x 4 mm were obtained using a diamond disc. The specimens were polished with sandpaper and abrasive pastes. A perfilometer was used to measure roughness average (Ra) values of the initial surface roughness and at each 7-day-interval after the beginning of treatment. The bleaching was performed on the surface of the fragments for 1 hour a week, and the surface cleaning treatment for 3 minutes daily. The samples were stored in individual receptacles with artificial saliva. Analysis of variance and the Tukey test revealed significant differences in surface roughness values for G2 and G3, which showed an increase in roughness over time; G1 and G4 showed no significant roughness differences. The bleaching with 35% carbamide peroxide did not alter the enamel surface roughness, but when the bleaching treatment was performed combined with brushing with abrasive dentifrices, there was a significant increase in roughness values.  (+info)
