This project is a longitudinal study of over 300 infants at risk for autism based on their family history of having older siblings who are diagnosed with ASD. The developmental trajectories of children from 6 to 36 months will be studied, including subgroups of children who manifest varying patterns of symptoms ranging from full autism to the broader autism phenotype to non-affected siblings. Additionally, the children in this study will be followed to age 5 to describe the developmental trajectory of this cohort more fully. This project is a longitudinal study of over 300 infants at risk for autism based on their family history of having older siblings who are diagnosed with ASD. The developmental trajectories of children from 6 to 36 months will be studied, including subgroups of children who manifest varying patterns of symptoms ranging from full autism to the broader autism phenotype to non-affected siblings. Safer's older brother was the “difficult” one in her family, and she writes with poignant pain about his isolation and the debilitating pressure she felt as “the favored child.” She shares her highly dysfunctional family situation, and the disappointment of not being able to connect with her brother as an adult. Safer's story did speak to the importance of siblings, and I agreed with her that, until recently, neurotypical siblings have had few opportunities for sibling-specific support. So I was worried about him being pretty surly and unhappy, especially since both tiny TV/VCR sets recently died from exhaustion and toddler abuse. To our surprise, he has been pretty happy, even participating in outdoor activities to some extent. That primarily includes sitting on the ground outside the kiddie pool, flinching and getting splashed and finding random things to throw in there and push around, and then getting splashed some more. The professionals who might have taken control of the situation as her health and functioning deteriorated have not done so and I must presume that they believe they cannot do so. (2/1632) Haematopoietic stem cell transplantation for thalassaemia major in Hong Kong: prognostic factors and outcome.

From August 1992 to August 1999, 44 patients received allogeneic haematopoietic stem cell transplantation in a single institution. The donors were HLA-identical siblings except for one who was a phenotypically matched father. Thirty-eight patients received bone marrow stem cells and the others received peripheral blood stem cells or umbilical cord blood (UCB). The mean age at transplant was 10.7+/-5.1 years, ranging from 1.8 to 21 years. Patients received busulphan (16 mg/kg) and cyclophosphamide (150 to 200 mg/kg) as conditioning, and antithymocyte globulin was given to 42 patients to prevent graft rejection. All had engraftment except a patient who received a UCB transplant. Four patients died from early treatment-related mortality, and one died from interstitial pneumonitis 3 months after transplant. Two patients developed secondary graft rejection and both received a second transplant. Thirty-eight patients survived and all except one were transfusion independent. The 5-year overall and event-free survival rates were 86% and 82%, respectively. By multivariate stepwise Cox proportional hazard analyses, severe veno-occlusive disease (VOD) of liver and Pesaro class 3 features were the significant factors associated with survival. Patients aged more than 11 years were more inclined to develop VOD. In conclusion, haematopoietic stem cell transplantation should be performed early if an HLA identical sibling is available.  (+info)

(3/1632) Healthy sibling donor anxiety and pain during bone marrow or peripheral blood stem cell harvesting for allogeneic transplantation: results of a randomised study.

This study reports the first comparison of healthy donor subjective well-being during two alternative procedures of hematopoietic stem cells harvesting for allogeneic transplantation. Among the 105 donors included between September 1996 and October 1998 in the SFGM French randomised trial aiming to compare allogeneic bone marrow (BM) transplantation and blood cell (BC) transplantation, 64 donors (33 in BC and 31 in BM groups) were relevant for the analysis. They had received a set of self-administered questionnaires to complete during the collection process, aiming to measure anxiety (assessed using the Spielberger's State-Trait Anxiety Inventory) and pain induced by the procedure (evaluated using a visual analogical scale). Results showed that no harvest procedure is free from pain even if none was more painful than the other. Levels of anxiety before the collection procedure were high in both groups and significantly so for BC donors. Although BC collection induces at least similar levels of pain and anxiety as does BM collection, they were of a different kind, and the short-term impact of G-CSF stimulation on the well-being of BC donors has to be taken into account in improving quality of care in the allogeneic setting.  (+info)

(4/1632) Myocarditis in sibling boxer puppies associated with Citrobacter koseri infection.

Two sibling Boxer puppies presented with severe suppurative myocarditis in the absence of additional disseminated suppurative foci. The identification of gram-negative bacteria within areas of myocarditis in both puppies and the pure growth of large numbers of Citrobacter koseri from the myocardial lesions in one of the dogs were consistent with a bacterial etiology. The fact that C. koseri is an opportunist pathogen suggested intercurrent immunosuppression. The finding of a concomitant bacterial myocarditis in two canine siblings is novel. The case is also unusual in that syncope could be related to the myocardial injury.  (+info)

(5/1632) Familial risk of preeclampsia in Newfoundland: a population-based study.

This study sought to quantify the familial risk of preeclampsia (proteinuric hypertension) in Newfoundland and to identify characteristics in probands that predict increased familial risk. Reviewed were 5173 obstetric charts from 10 hospitals, representing 99% of deliveries on the island of Newfoundland for a 1-yr period from April 1996 to March 1997; pregnancy-induced hypertension was diagnosed according to strict criteria. Family obstetric histories were obtained from identified probands with preeclampsia, and sisters and mothers of probands were interviewed. In addition, the obstetric charts from sisters and mothers were reviewed to identify preeclampsia. The incidence of preeclampsia in the population was 5.6% (n = 292), and in primiparous women it was 7.9%. Factors independently associated with increased risk of preeclampsia included primiparous delivery, multiple gestation, pregestational and gestational diabetes, maternal age of more than 35 yr, and region of the province. Of 330 sisters identified, 217 had 445 pregnancies, with 331 charts located for review. The incidence of preeclampsia (based on chart review) in 163 primiparous sisters was 20.2%. The relative risk of preeclampsia in primiparous sisters of probands with preeclampsia compared with primiparous women in the population was 2.6 (95% confidence interval, 1.8 to 3.6). Factors in probands independently associated with a higher risk of preeclampsia in sisters included at least 2+ proteinuria and region of the province. This population-based study, which used unbiased ascertainment and strict diagnostic criteria, demonstrated a significantly higher risk of preeclampsia in sisters of probands with preeclampsia, particularly when probands were defined by severity of preeclampsia and by geographic region.  (+info)

(6/1632) Genetic heterogeneity in schizophrenia II: conditional analyses of affected schizophrenia sibling pairs provide evidence for an interaction between markers on chromosome 8p and 14q.

Information from multiple genome scans and collaborative efforts suggests that schizophrenia is a heterogeneous, complex disorder with polygenic and environmental antecedents. In a previous paper we demonstrated that stratification of families on the basis of co-segregating phenotypes (psychotic affective disorders (PAD) and schizophrenia spectrum personality disorders (SSPD) in first-degree relatives of schizophrenic probands increased linkage evidence in the chromosome 8p21 region (D8S1771) among families with co-segregating SSPD. We have now applied a method of conditional analysis of sib-pairs affected with schizophrenia, examining shared alleles identical-by-descent (IBD) at multiple loci. The method yields enhanced evidence for linkage to the chromosome 8p21 region conditioned upon increased allele sharing at a chromosome 14 region. The method produces a more refined estimate of the putative disease locus on chromosome 8p21, narrowing the region from 18 cM (95% confidence interval) in our previous genome scan, to approximately 9.6 cM. We have also shown that the affected siblings sharing two alleles IBD at the chromosome 8p21 region and one allele IBD at the chromosome 14 region differ significantly in clinical symptoms from non-sharing affected siblings. Thus the analysis of allele sharing at a putative schizophrenia susceptibility locus conditioned on allele sharing at other loci provides another important method for dealing with heterogeneity.  (+info)

(7/1632) Reciprocal bone marrow transplantation between brother and sister.

A child with AML underwent allogeneic BMT from an HLA-identical sister donor. Prompt and stable triline-age engraftment occurred and after few months he returned to a normal life. Eight years later a primary NHL of bone developed in his sister. A partial remission was obtained by means of standard NHL treatment, but 3 months later rapid disease progression occurred with complete bone marrow invasion (ALL-L3). She was treated with a leukemia relapse protocol, obtaining a second partial remission. Unpurged bone marrow harvested from the brother, transplanted for AML 8 years earlier, was infused after conditioning with TBI and thiothepa. No GVHD prophylaxis was given. Neutrophil engraftment occurred by 14 days and platelet engraftment by 20 days after BMT. No acute GVHD was observed, but unexpectedly she developed skin and liver GVHD-like symptoms 80 days after BMT. Since the liver biopsy was suggestive of liver GVHD and in the absence of any other evidence as a possible cause of the hepatic damage, the patient started mycophenolate. Two months later serum hepatitis B markers were detectable.  (+info)

(8/1632) Vitamin D receptor gene polymorphism associates with graft-versus-host disease and survival in HLA-matched sibling allogeneic bone marrow transplantation.

We investigated the role of polymorphism of the vitamin D receptor (VDR) gene in HLA-matched sibling BMT for polymorphisms previously associated with human disease pathology. In intron 8 of the VDR gene, the B and A alleles of the BsmI and ApaI RFLPs were found to associate with reduced aGVHD when present in the patient's genotype. Logistic regression analysis demonstrated that patient VDR genotype, along with previously identified IL-10(-1064) and IFN-gamma genotype to be risk factors for severe acute GVHD. The A allele also associates with increased likelihood of death when present in the donor genotype (AA vs Aa or aa, hazard ratio 2.03, P = 0.0232). In patients who received increased prophylaxis with multi-agent therapy, patients whose graft was from a donor with an AA genotype had a substantially worse survival than patients whose graft was from a donor with a non-AA genotype (hazard ratio 12.93, P < 0.0001). Analysis of VDR genotype in prospective BMT recipients could indicate patients at risk of severe aGVHD. Analysis of VDR genotype in prospective BMT donors may identify individuals who have greater transplant-related mortality, and also allow appropriately restricted use of increased immunosuppressive prophylaxis.  (+info)

What's the chance of biological siblings living parallel lives if they never meet each other?


  • Let's say there's 2 half-siblings 9 year age difference. Both are given up for adoption and never meet each other. In both of their adopted homes, they are the oldest child. What's the likelihood that these siblings will be extremely similar in personality and will respond to environmental stimuli in the same ways? What's the chance that they will move to the same city and meet each other?
  • This happened with my brother and I, except he is 16 years older than I am and we were the youngest in our families. While we both have similar senses of humor, medical conditions, physical appearances, ect., we lived very different lives. He liked to do cocaine, smoke pot, drink, and smoke in general...suffered with addiction, eventually got the help he needed and has been clean for a long time (besides smoking cigarrettes.) Me, I drink occasionally to relax but other than that I've never been a user. I was an over achiever in school, he was an under achiever (although you could probably blame the drugs on that one). I suppose the posibility of siblings living parallel lives is possible, although highly unlikely. It's something you only see in the movies...


What do I do with uncontrollably immature hyperactive siblings?


  • These siblings have the tendency to constantly throw tiny objects, poke people with things, roll in my bed while sleeping, bear hug or immatate combat or wrestle moves at inappropriate times causing restraint of movement and causing severe disruption of daily activities. One is 16 the other is 12 and their behavior is not age appropriate. The 16 year old sometimes makes jokingly homosexual advances during inapropriate times. The 12 year old sometimes responds combativly to any repremends or criticism of behavior. I know I cant legally respond defensivly to the 12 year olds physically combative behavior. So what are proactive ways to deal with uncontrollable behavior? On occasion they intentionaly potentuate flinchful reflexes by faking possible injurous actions that can get annoying. As they unabativly roughhouse. Treebird I DON'T SLEEP WITH THEM MORON!! I sleep alone in my bed and they barge in at the crack of dawn jumping and rolling around in it and acting stupid. The parents do discipline them, but every attempt to do so fails. We take away their video games, we deactivate their cell phones, stop them from using internet yet they continue to misbehave no matter what action is taken.
  • These are your siblings? How old are you? Where are their parents? Why do they sleep with you? Are they boys? Are you? Without more information I would say it is unlikely you can control their behavior, so you should get away from them. Behind a locked door might work.


What do you think about separating siblings?


  • What about a sibling group of 5, when people have come forward for 3 of them but not all 5? There are no issues with these children, they only need stability. The siblings have been visiting monthly for years, but likely dont even remember living together. What are your thoughts on this?
  • I think siblings should stay together unless they are a danger and threat to one another...and some are. You say they have no issues.....except of course someone wants three of them but not all five. Siblings should never be separated when they can live together. The sibling bond is the longest bond we will ever know.....by nature it is even longer than the bond of parent and child because by nature, the parent dies first before the child....siblings will stand beside each other as they bid farewell to their deceased parent(s). Once a separation occurs, the children are at the mercy of the a/parent to continue the contact and that tends to not happen as life gets in the way and goals and expectations take off and depart between families. So......to the point, siblings should not be separated to make the "wanting adults" happy. Especially if they can live together in a home that will take them all.....when that family is found.


What is a good age difference for siblings to be?

  • I have noticed some things about children and ages and siblings. Two of my friends had 2 year olds and then decided to have another baby. In each case, the 2 year old did not understand this new baby, and they acted out, sometimes terribly at the newborn. It's not their fault, obviously, but it is extremely hard on a 2 yr old to go from being the center of attention to hardly being noticed because everyone is fawning over new baby. It happens, and no matter how hard the parents try to include the 2yr old, the 2 yr old still will likely feel the need to act out. In both cases, the toddler reacted rather violently to the child. My friends with the two girls, saw the their little girl haul off and slap the baby across the face. It took lots of discipline to stop this. My other set of friends had a two year old boy and a little girl. He was throwing pillows and stuff onto the sleeping newborn while we were visiting and our friends told us that he had hit his baby sister the other day. It was obvious what was happening. He wanted attention so badly that he would be bad just to receive it. We had even gotten him a new present when we came to visit, hoping to include him, but he still acted out. The baby had received a bunch of collectors dolls, one in her namesake...the other ones the toddler named after himself and his daddy. He left out his mom, lol. It's been difficult for both of them and it has taken ALOT of discipline to control the toddlers in each case. The boy still acts out and is now the little bully of the athletic center we go to, so it's terrible twos times ten. Another of our friends is pregnant with a baby and they have a daughter who is 4. The little girl is so excited about the new baby and wants to help her mom. To a 2 year old, a baby is just a baby, but to a 4 year old, mommy's baby is OUR baby. She proceeded to tell me how she was going to have a sister and she was going to help name her. I stressed our as the 4 year old acknowledges that the new addition will be hers too. She is excited, is of an age where she understands that mommy is pregnant, and she gets to help with the baby which is incredibly important. Younger children cannot think of it this way and will likely act out. I am not going to comment on what is the best age difference. I only am adding my observation as it's whatever is best for you and your partner. If under 4, you may want to be prepared for some tough discipline on your toddler when baby arrives as from everything that I have seen, they will act out. Special attention needs to be paid to the toddlers so they don't feel left out.


Is it normal for an only child to feel jealous of those with siblings?


  • I am an only child, and though this sounds petty, I feel somewhat jealous of those with siblings. I don't act hostile towards such people, but why do I feel jealous over this?
  • Yes it's normal, and I'm not a parent. You feel it because you wish you could feel the family feeling of siblings, and also it helps to have someone your age always around for you to talk to. I'm not an only child, I have a brother, but he's 6 years older than me. I was always jealous of kids who had siblings within 3 years of age to them. My brother and I never got along (for private family reasons), and then once he moved out I felt exactly like an only child. I am jealous of kids who have that sibling bond. I never had it. I feel that if my brother and I were closer in age things would have been different.


How do children turn out depending on how many siblings they have?

  • The first poster seems to be wearing their ass hat today. I'm from a family of 7 (6 now, one of my sisters is no longer welcome as a family member) and my husband's from a family of 6. We both had our squabbles and different levels of closeness with different siblings, but it's not like we turned out radically different from only children, or children from the average 2.4 kid household. I do think we're both able to handle stress, being busy all the time, chaos, and a lot of responsibility better than kids from smaller homes. We were both higher up age wise in our family, I'm the third and he's the second born, so we both spent all our lives having responsibilities and helping our parents out. There's little room for hysterics in a big family, and you can't let little things stress you out, so you just kinda learn to go with the flow and not freak out.


How do I teach my siblings and cousins good manners and loyalty?


  • I want my siblings and cousins to be very nice people that have manners and loyalty and everything but also know how to have fun and be kids and everything. How can i help teach them to have good manners and loyalty and everything? Kids are all ages Right now they are ok but i want them to be real good at it.
  • Lead by example. That's the best way to teach kids. If they look up to you and enjoy your company they will pick up these good behaviors. If they are small and asking for something without saying please or thank you encourage it. I remember my grandmother when I was a child when I asked for something and didn't say please she would always say "what do you say??" and if I didn't say something she would follow it up with a "say please". If I didn't say thank-you she would always say "say thank-you". Overtime its just something you get used to. My kids are 2yrs and 3yrs and are both very polite. They are a little excessive with it (i.e. my daughter thanks me for changing her diaper and my son for making him meals), but at least they get the point. I never really pushed them to do it, I just thanked them when they did something for me and said please when I asked something of them. They seen how I used my words and picked it up quick. =)


Why are there so many siblings available for adoption?


  • I browsed the adoption site and noticed many groups of 5-6 siblings waiting to be adopted. Do the parents die or suddenly decide they don't like their 6 kids, or does the state take them away? I'm just interested.
