PURPOSE: To devise an analytical framework to help identify strengths and weaknesses in the audit process as specified by existing psychiatric nursing audit systems, in order to analyse current audit practice and identify improvements for incorporation in the Newcastle Clinical Audit Toolkit for Mental Health. The properties of the human visual system (HVS) relevant to the diagnostic process are described after a brief introduction on the general problems and advantages of using soft copy for primary radiology interpretations. At various spatial and temporal frequencies the contrast sensitivity defines the spatial resolution of the eye-brain system and the sensitivity to flicker. The adaptation to the displayed radiological scene and the ambient illumination determine the dynamic range for the operation of the HVS. Although image display devices are determined mainly by state-of-the-art technology, analysis of the HVS may suggest technical characteristics for electronic displays that will help to optimize the display to the operation of the HVS. These include display size, spatial resolution, contrast resolution, luminance range, and noise, from which further consequences for the technical components of a monitor follow. It is emphasized that routine monitor quality control must be available in clinical practice. These image quality measures must be simple enough to be applied as part of the daily routine. An image processing strategy is presented that assures very similar soft-copy presentation on diagnostic workstations of a picture archiving and communication system (PACS) over the lifetime of an image file and simultaneously provides efficient work-flow. The strategy is based on rigid partitioning of image processing into application- and display-device-specific processing. Application-specific processing is optimized for a reference display system. A description of this system is attached to the file header of the application-specifically processed image which is stored in the PACS. (4/3566) A critical appraisal of the quality of the management of infective endocarditis.

OBJECTIVES: The purpose of this study was to assess the quality of the management of infective endocarditis. BACKGROUND: Although many guidelines on the management of infective endocarditis exist, the quality of this management has not been evaluated. METHODS: We collected data on all patients (116) hospitalized with infective endocarditis over 1 year in all hospitals in the Rhone-Alpes region (France). RESULTS: Prophylactic antibiotics were not given before infective endocarditis to 8/11 cardiac patients at risk and who underwent an at risk procedure. Among the 55 cardiac patients at risk and with fever and who consulted a physician, blood cultures were not performed before antibiotic therapy was initiated for 32 patients. In-hospital antibiotic therapy was incorrect for 23 patients. The portal of entry was not treated for 16/61 patients with an accessible portal of entry. Among the 19 patients who had severe heart failure or fever persisting more than 2 weeks in spite of antibiotic therapy and who could have undergone early surgery, surgery was delayed for five, and not performed for three. Overall, the average score was 15/20. CONCLUSIONS: More information on the management of infective endocarditis should be widely disseminated to the physicians' and the dentists' communities and to the patients at risk.  (+info)

(5/3566) Evaluation of the quality of an injury surveillance system.

The sensitivity, positive predictive value, and representativeness of the Canadian Hospitals Injury Reporting and Prevention Program (CHIRPP) were assessed. Sensitivity was estimated at four centers in June through August 1992, by matching independently identified injuries with those in the CHIRPP database. The positive predictive value was determined by reviewing all "injuries" in the database (at Montreal Children's Hospital) that could not be matched. Representativeness was assessed by comparing missed with captured injuries (at Montreal Children's Hospital) on demographic, social, and clinical factors. Sensitivity ranged from 30% to 91%, and the positive predictive value was 99.9% (i.e., the frequency of false-positive capture was negligible). The representativeness study compared 277 missed injuries with 2,746 captured injuries. The groups were similar on age, sex, socioeconomic status, delay before presentation, month, and day of presentation. Injuries resulting in admissions, poisonings, and those presenting overnight were, however, more likely to be missed. The adjusted odds ratio of being missed by CHIRPP for admitted injuries (compared with those treated and released) was 13.07 (95% confidence interval 7.82-21.82); for poisonings (compared with all other injuries), it was 9.91 (95% confidence interval 5.39-18.20); and for injuries presenting overnight (compared with those presenting during the day or evening), it was 4.11 (95% confidence interval 3.11-5.44). These injuries were probably missed because of inadequate education of participants in the system. The authors conclude that CHIRPP data are of relatively high quality and may be used, with caution, for research and public health policy.  (+info)

In recent years Lot quality assurance sampling (LQAS), a method derived from production-line industry, has been advocated as an efficient means to evaluate the coverage rates achieved by child immunization programmes. This paper examines the assumptions on which LQAS is based and the effect that these assumptions have on its utility as a management tool. It shows that the attractively low sample sizes used in LQAS are achieved at the expense of specificity unless unrealistic assumptions are made about the distribution of coverage rates amongst the immunization programmes to which the method is applied. Although it is a very sensitive test and its negative predictive value is probably high in most settings, its specificity and positive predictive value are likely to be low. The implications of these strengths and weaknesses with regard to management decision-making are discussed.  (+info)

Non-governmental organizations (NGOs) have increasingly been promoted as alternative health care providers to the state, furthering the same goals but less hampered by government inefficiencies and resource constraints. However, the reality of NGO health care provision is more complex. Not only is the distinction between government and NGO providers sometimes difficult to determine because of their operational integration, but NGOs may also suffer from resource constraionts and management inefficiencies similar to those of government providers. Some registered NGOs operate as for-profit providers in practice. Policy development must reflect the strengths and weaknesses of NGOs in particular settings and should be built on NGO advantages over government in terms of resource mobilization, efficiency and/or quality. Policy development will always require a strong government presence in co-ordinating and regulating health care provision, and an NGO sector responsive to the policy goals of government.  (+info)

OBJECTIVES: To establish an accurate and reliable comparative database of discharge abstracts and to appraise its value for assessments of quality of care. DESIGN: Retrospective review of case notes by trained research abstractors and comparison with matched information as routinely collected by the hospitals' own information systems. SETTING: Three district general hospitals and two major London teaching hospitals. PATIENTS: The database included 3905 medical and surgical cases and 2082 obstetric cases from 1990 and 1991. MAIN MEASURES: Accessibility of case notes; measures of reliability between reviewers and of validity of case note content; application of high level quality indicators. RESULTS: The existing hospital systems extracted insufficient detail from case notes to conduct clinical comparative analyses for medical and surgical cases. The research abstractors at least doubled the diagnostic codes extracted. Interabstractor agreement of about 70% was obtained for primary diagnosis and assignment to diagnosis related group. These data were sufficient to create a comparative database and apply high level quality indicators designed to flag topics for further study. For obstetric-specific indicators the rates were comparable for abstractors and the hospital information systems, which in each case was a departmentally based system (SMMIS) producing more detailed and accessible data. CONCLUSIONS: Current methods of extracting and coding diagnostic and procedural data from case notes in this sample of hospitals is unsatisfactory: notes were difficult to access and recording is unacceptably incomplete. IMPLICATIONS: Improvements as piloted in this project, are readily available should the NHS, hospital managers, and clinicians see the value of these data in their clinical and managerial activities.  (+info)

I generally like the health care here .. we do actually pay into it, not as much as Americans do, but we still do.  Roughly $70 a month if for medical/dental.  Enables us to go and have our prescriptions, doctors appointments, check ups surgeries, pretty well anything, for that cost.  The downside is, we do wait a long time for surgeries.  Also for exams, I am currently waiting 6 months to have an MRI done.  Depending on the surgery, you could wait anywhere from a few weeks, to a year for it.

What effect does the health care system of a country have on quality of life?

For one, take a look at the HDI website....or even the wiki version. The countries in the top four, Canada, Norway, Australia and Iceland, all have Universal Healthcare systems in place. They are all considered the countries with the highest quality of life. 

It is not a coincidence that these countries all have what Americans consider to be "socialist" medical systems in place, and rate way higher on the HDI than the US.

Ok so your employer is a health care organization...as a manager you have the pressure of improving the customer service and than there's a community of patients who wants you improve health care quality with affordable cost...BUT how can you improve customer service + quality when your health care organization demands to maximize the patient per physician ratio? Is there really a way to work around all three of these demands?

I would say yes, but only if the government stays out of it. When the government gets involved costs almost ALWAYS go up, and quality almost ALWAYS goes down. Customer service on the other hand does NOT have to be a zero sum game, it's just a matter of keeping your personnel trained up and keep their morale up. If the employees are happy, then they will be more positive around their patients. If they are well trained, then they should be able to answer questions better that patients have. Patient to doctor ratio really shouldn't play too much of a part in that!

Which version of healthcare bill?  I don't think anyone knows what the final version will look like.  In general, I would say that we're trying to do too much in one big chunk.  
There are smaller steps I'd take first and go from there.  Remove the anti-trust exemptions from insurance companies and allow for purchase of insurance across state lines.  Both of these steps, in my opinion, would stimulate competition and bring down cost of insurance.
Undocumented aliens?  Is it better to subsidize their insurance premiums with tax dollars or to continue to have them use the ER for primary care, free of charge?  I don't know....Both expensive.  Best solution would be to protect our borders.
If I sound like I'm parroting the Republican talking points, I ask one question....Where the hell were they for the 8 years preceding the Obama's ascension to office??????  I'm sick of the whole Washington mentality.  Politics is a DIRTY word.

The real answer will be too long to post here, but a few highlights. Health care used to be under the control of doctors. In WWII the US government unconstitutionally froze wages and prices so in order to obtain and retain the best employees, employers had to add benefits as they couldn't adjust wages. One of the benes added was health insurance which was not really needed (not anywhere as important as it is today). After WWII the link remained--in peoples' minds and in fact.
Come 1966 and the feds decided to take their unconstitutional Social Security program a step farther and create Medicare--now the feds were in health care big time. Medicare has grown over time and was always supposed to ensure that no doc or hospital ever got a profit from treating Medicare patients--unfortunately docs have been taken to the cleaners on the deal which is why it's getting harder and harder to find one who takes Medicare. Also premiums are on the rise, particularly in the last decade:
In the US, Medicare is going bankrupt. In 1998, Medicare premiums were $43.80 and in 2008 will be $96.40--up 120%. "Medigap" insurance is common because of the 20% co-pay required for service. Medicare HMOs are common because they reduce that burden without an extra charge in many cases. HOWEVER, many procedures which used to have no or a low co-pay NOW cost the full 20% for the HMO Medicare patient. ALSO the prescription coverage they tended to offer has been REDUCED in many cases to conform to the insane "donut hole" coverage of the feds. Doctors are leaving Medicare because of the low and slow pay AND because the crazy government wants to "balance" their Ponzi scheme on the backs of doctors. 
"That dark cloud lurking over the shoulder of every Massachusetts physician is Medicare. If Congress does not act, doctors' payments from Medicare will be cut by about 5 percent annually, beginning next year through 2012, creating a financial hailstorm that would wreak havoc with already strained practices.

Cumulatively, the proposed cuts represent a 31 percent reduction in Medicare reimbursement. If the cuts are adjusted for practice-cost inflation, the American Medical Association says Medicare payment rates to physicians in 2013 would be less than half of what they were in 1991."

In the very early 1970s, others looked at the growth of health care as we made more scientific advances (DNA was only discovered in 1953, for example) and decided that being able to have control over life and death should be pretty profitable, so the concept of "managed care" based on the LIE that doctors were making "too much money" was born and embraced by those who couldn't get into or through medical school. The door was open for the hijacking of medical care and the skyrocketing of prices. As you see with Medicare payments, doctors would NOT be the beneficiaries.
Since that time the boldness of insurance companies to ignore their contracts as well as antitrust laws has grown wildly and the government sits and twiddles its thumbs wasting time and tax dollars on "investigating" Microsoft and baseball instead of enforcing laws. Hospitals have decided that they'll charge the uninsured about 3 times what they charge the insured and have become aggressive in going after CITIZENS who don't pay their bills in full.
As always, the government has ensured problems will exist with their mandates that everyone be treated at an ER if he has a life-threatening condition, regardless of ability to pay. Sounds good on paper. Hospitals deliberately misinterpret the mandate to mean "treat all illegals no matter how little they need medical care and bill the heck out of the taxpayer." Combine this with the stupid governmental mandate of "compassionate entry" (the Border Patrol is instructed to let in everyone who is ill so they can be treated here, knowing full well in many cases the taxpayer is going to eat the bill), and you have hard-working Americans paying more and seeing portions of their hospitals shut down because of governmental meddling and their inability to do their job: secure the borders.
A few things that should be of interest to any thinking man on the subject of health care in the US--in other words, the pols won't discuss this and the media give it short shrift:
When 75% of the people who declare bankruptcy over medical bills ARE INSURED, then insurance is CLEARLY not the answer.

Healthcare cost is rising because we the people don't have a say so. Insurance companies have taken over health care. Physicians don't have the power in what they can do or not do. It is completely controlled by the insurance companies.
Here is a scenario, plastic surgery or laser eye surgery. They do not cost near as much as a basic surgery in the hospital. Why because they are not controlled by innsurance plans. Most insurance don't cover these types of surgery. so the doctors are left to provide QUALITY service and competitive prices to keep clients coming back. Therefore, they are providing quality and are affordable. The customers are able to choose and select who they want to go to because they are paying for it.
