Ventilatory function and personal breathing zone dust concentrations in Lancashire textile weavers. (1/37)

BACKGROUND: To report findings on ventilatory function and estimations of concentrations of personal breathing zone dust in Lancashire textile weavers. Weaving room dust is considered to be less harmful than that encountered in the cardroom or spinning room and weavers are generally thought to have less respiratory disability than carders or spinners. However, this occupational group has not been extensively studied. METHODS: Each person was given a respiratory symptom questionnaire (modified Medical Research Council, UK, questionnaire on respiratory diseases). Ventilatory function tests, forced expiratory volume in 1 second (FEV1) and forced vital capacity (FVC) were performed on each person. A representative sample of workers from each occupational group underwent dust sampling in their personal breathing zone. Dust concentrations and ventilatory tests were analysed statistically with the Student's t test, Pearson's correlation coefficient, and forward step regression for relations with symptoms and environmental factors. Significance was p > or = 0.05. RESULTS: The FEV1 and FVC were reduced in workers with respiratory symptoms (non-specific chest tightness, shortness of breath, persistent cough, and wheezing) as well as in preparation room workers, current and former smokers, Asians, those working with predominantly cotton fibre (> 50% cotton) and starch size. Mean total dust concentration (pd1) in the personal breathing zone was 1.98 mg/m3. The corresponding value for total dust with large fibres lifted off the filter paper (pd2) was 1.55 mg/m3. There was a strong correlation (r = 0.94, p < 0.0001) between pd1 and pd2. Non-specific chest tightness was predicted by low dust concentrations and persistent cough by high dust concentrations. On regression analysis, impairment of ventilatory function (FEV1, FVC) was predicted by smoking, male sex, age, not working in the weaving shed, not being white, and personal dust concentrations. CONCLUSIONS: The FEV1 and FVC were impaired in smokers and those exposed to high dust concentrations in the personal breathing zone. Symptoms were inconsistently related to dust concentrations in the personal breathing zone.  (+info)

In the past, human violence was associated with food shortage, but recently it has increased even in relatively well-fed societies. The reason appears from studies of monkeys under relaxed, spacious conditions and under crowding stress. Uncrowded monkeys have unaggressive leaders, rarely quarrel, and protect females and young. Crowded monkeys (even well-fed) have brutal bosses, often quarrel, and wound and kill each other, including females and young. Crowding has similar behaviour effects on other mammals, with physiological disturbances including greater susceptibility to infections. All this appears to be a regular response to overpopulation, reducing the population before it has depleted its natural resources. Human beings, like monkeys and other mammals, need ample space, and become more violent when crowded. Human history is marked by population cycles: population outgrows resources, the resulting violence, stress and disease mortality cuts down the population, leading to a relief period of social and cultural progress, till renewed population growth produces the next crisis. The modern population crisis is world-wide, and explains the increase of violence even in well-fed societies. The solution to the problem of violence is to substitute voluntary birth control for involuntary death control, and bring about relaxed conditions for a reduced world population.  (+info)

Women survivors of child sexual abuse. How can health professionals promote healing? (4/37)

OBJECTIVE: To explore how health professionals can practise in ways sensitive to adult women survivors of child sexual abuse. DESIGN: Qualitative semistructured in-depth interviews. SETTING: Small and midsize cities in Ontario and Saskatchewan. PARTICIPANTS: Twenty-seven women survivors of childhood sexual abuse. METHODS: Respondents were asked about their experiences with physical therapists and other health professionals and asked how practice could be sensitive to their needs as survivors. A grounded-theory approach was used. After independent analyses, researchers achieved consensus on the main themes. Findings were checked with participants, other survivors, and mental health professionals. MAIN FINDINGS: A crucial theme was the need to feel safe when consulting any health professional. Participants described specific ways for clinicians to facilitate the feeling of safety. Disclosure of abuse history was another key theme; analysis revealed no one "right way" to inquire about it. CONCLUSION: Women survivors of child sexual abuse want safe, accepting environments and sensitive, informed health professionals with whom to work in partnership on all their health concerns.  (+info)

Role for human posterior parietal cortex in visual processing of aversive objects in peripersonal space. (5/37)

The posterior parietal cortex of both human and non-human primates is known to play a crucial role in the early integration of visual information with somatosensory, proprioceptive and vestibular signals. However, it is not known whether in humans this region is further capable of discriminating if a stimulus poses a threat to the body. In this functional magnetic resonance imaging (fMRI) study, we tested the hypothesis that the posterior parietal cortex of humans is capable of modulating its response to the visual processing of noxious threat representation in the absence of tactile input. During fMRI, participants watched while we "stimulated" a visible rubber hand, placed over their real hand with either a sharp (painful) or a blunt (nonpainful) probe. We found that superior and inferior parietal regions (BA5/7 and BA40) increased their activity in response to observing a painful versus nonpainful stimulus. However, this effect was only evident when the rubber hand was in a spatially congruent (vs. incongruent) position with respect to the participants' own hand. In addition, areas involved in motivational-affective coding such as mid-cingulate (BA24) and anterior insula also showed such relevance-dependent modulation, whereas premotor areas known to receive multisensory information about limb position did not. We suggest these results reveal a human anatomical-functional homologue to monkey inferior parietal areas that respond to aversive stimuli by producing nocifensive muscle and limb movements.  (+info)

CME workshop in recognizing boundary limits during a medical visit. (6/37)

BACKGROUND AND OBJECTIVES: Professional boundaries help to establish and maintain the foundation of the doctor-patient relationship. Our objective was to measure physicians' level of awareness in recognizing a boundary limit, their intention to change their behavior regarding boundaries, and their self-reported changes in behavior after participation in an educational workshop, "The Right Distance." METHODS: This longitudinal study used questionnaires to measure the level of awareness of the participants in recognizing boundary limits before and after the workshop and to measure their intention to change their behavior and self-reported behavior changes immediately after the workshop and 6 months later. RESULTS: Before the workshop, the level of awareness in recognizing the boundary limits was 3.9 on a 5.0 point scale in which 5.0 represented the highest level of awareness, and immediately after the workshop it was 4.2/5.0. Female physicians were more aware of these issues. Following the workshop, 51% of participants indicated an intention to change their behavior. On the 6-month follow-up questionnaire, 138 physicians (60%) answered that they had not changed their behavior, and 92 physicians (40%) answered that they had modified their behavior by changing their attitude toward their personal and professional life and toward gifts and gratuities. When the post-workshop and the 6-month follow-up questionnaires were matched, 34 of 57 (60%) participants who stated their intention to change did in fact change their behavior regarding the "right distance." CONCLUSIONS: Boundary issues and level of awareness can be effectively taught in a continuing medical education workshop.  (+info)

Neural bases of personal and extrapersonal neglect in humans. (7/37)

Human awareness of left space may be disrupted by cerebral lesions to the right hemisphere (hemispatial neglect). Current knowledge on the anatomical bases of this complex syndrome is based on the results of group studies that investigated primarily the best known aspect of the syndrome, which is visual neglect for near extrapersonal (or peripersonal) space. However, another component-neglect for personal space-is more often associated with, than double-dissociated from, extrapersonal neglect, especially, in chronic patients. The present investigation aimed at exploring the anatomical substrate of both extrapersonal and personal neglect by using different advanced methodological approaches to lesion-function correlation. Fifty-two right ischaemic patients were submitted to neuropsychological assessment and in-depth MRI evaluation. The borders of each patient's lesion were delimited onto its own high-resolution anatomical image and then submitted to an automated spatial normalization algorithm. Besides conventional lesion density plots and subtraction analysis, region-based statistical analyses were performed on percentage values of the lesioned tissue also using a new parcellation of the white matter (WM). Data were finally submitted to voxelwise statistical analysis using a recently proposed method (voxel-based lesion-symptom mapping). Results converged in showing that awareness of extrapersonal space is based on the integrity of a circuit of right frontal (ventral premotor cortex and middle frontal gyrus) and superior temporal regions, whereas awareness of personal space is rooted in right inferior parietal regions (supramarginal gyrus, post-central gyrus and especially the WM medial to them). Common but less crucial regions for both neglect sub-types were located in the temporo-peri-Sylvian cortex. We suggest that extrapersonal space awareness critically involves a ventral circuit recently described for the exogenous allocation and reorienting of attention in space. Disruption of personal space awareness, instead, seems to be due to a functional disconnection between regions important for coding proprioceptive and somatosensory inputs, and regions coding more abstract egocentric representations of the body in space. In conclusion, present data strongly support a segregation of personal and extrapersonal spatial awareness in humans, both from a functional and an anatomical point of view.  (+info)

Analysis of role-play in medical communication training using a theatrical device the fourth wall. (8/37)

BACKGROUND: Communication training is a central part of medical education. The aim of this article is to explore the positions and didactic functions of the fourth wall in medical communication training, using a role-play model basically similar to a theatrical performance. METHOD: The empirical data stem from a communication training model demonstrated at an international workshop for medical teachers and course organizers. The model involves an actress playing a patient, students alternating in the role of the doctor, and a teacher who moderates. The workshop was videotaped and analyzed qualitatively. RESULTS: The analysis of the empirical material revealed three main locations of the fourth wall as it moved and changed qualities during the learning session: 1) A traditional theatre location, where the wall was transparent for the audience, but opaque for the participants in the fiction. 2) A "timeout/reflection" location, where the wall was doubly opaque, for the patient on the one side and the moderator, the doctor and the audience on the other side and 3) an "interviewing the character" location where the wall enclosed everybody in the room. All three locations may contribute to the learning process. CONCLUSION: The theatrical concept 'the fourth wall' may present an additional tool for new understanding of fiction based communication training. Increased understanding of such an activity may help medical teachers/course organizers in planning and evaluating communication training courses.  (+info)
