Smoking status is an important variable in epidemiological and genetic studies of disease risk. In the past, questionnaires or interviews were used to ascertain tobacco use, but current studies are increasingly using de-identified versions of electronic medical records for genetic research. William Bush , Ph.D., graduate student Laura Wiley and colleagues evaluated ICD-9 International Classification of Diseases, Ninth Revision tobacco use codes as identifiers of smoking status. The researchers compared smoking status determined by a manual review of clinical records to two automated definitions: ICD-9 and natural language processing that extracts smoking status from the narrative text of electronic medical records. ... 'Virtual rehabilitation' is a concept in psychology in which a therapeutic patient's training is based entirely on, or is augmented by, virtual reality simulation exercises. If there is no conventional therapy provided, the rehabilitation is said to be "virtual reality-based." Otherwise, if virtual rehabilitation is in addition to conventional therapy, the intervention is "virtual reality-augmented." Today, a majority of the population uses the virtual environment to navigate their daily lives and almost one fourth of the world population uses the internet. As a result, virtual rehabilitation and gaming rehabilitation, or rehabilitation through gaming consoles, have become quite common. In fact, virtual therapy has been used over regular therapeutic methods in order to treat a number of disorders. OBJECTIVE: To review published criteria for specifically evaluating health related information on the world wide web, and to identify areas of consensus. DESIGN: Search of world wide web sites and peer reviewed medical journals for explicit criteria for evaluating health related information on the web, using Medline and Lexis-Nexis databases, and the following internet search engines: Yahoo!, Excite, Altavista, Webcrawler, HotBot, Infoseek, Magellan Internet Guide, and Lycos. Criteria were extracted and grouped into categories. RESULTS: 29 published rating tools and journal articles were identified that had explicit criteria for assessing health related web sites. Of the 165 criteria extracted from these tools and articles, 132 (80%) were grouped under one of 12 specific categories and 33 (20%) were grouped as miscellaneous because they lacked specificity or were unique. The most frequently cited criteria were those dealing with content, design and aesthetics of site, disclosure of authors, sponsors, or developers, currency of information (includes frequency of update, freshness, maintenance of site), authority of source, ease of use, and accessibility and availability. CONCLUSIONS: Results suggest that many authors agree on key criteria for evaluating health related web sites, and that efforts to develop consensus criteria may be helpful. The next step is to identify and assess a clear, simple set of consensus criteria that the general public can understand and use.  (+info)

Computers are already in widespread use in medical practice throughout the world and their utility and popularity is increasing day by day. While future generations of medical professionals will be computer literate with a corresponding increase in use of computers in medical practice, the current generation finds itself in a dilemma of how best to adapt to the fast-evolving world of information technology. In addition to practice management, information technology has already had a substantial impact on diagnostic medicine, especially in imaging techniques and maintenance of medical records. This information technology is now poised to make a big impact on the way we deliver medical care in India. Ophthalmology is no exception to this, but at present very few practices are either fully or partially computerized. This article provides a practical account of the uses and advantages of computers in ophthalmic practice, as well as a step-by-step approach to the optimal utilization of available computer technology.  (+info)

(3/1277) Information exchange in an epilepsy forum on the World Wide Web.

The Partners Healthcare Epilepsy Service hosts an epilepsy 'Webforum'. In this paper, we describe our observations regarding who uses it, what kind of information is exchanged, how much misinformation is present and how we can better serve our patients. We examined a sample of 155 posts to the forum and 342 responses to those posts. The individual making the post and the type of questions were categorized. We also determined whether any information was objectively inaccurate. The principal users were care-givers (49%) and patients (34%). Eighty percent of the primary posts were questions. Answers were given largely by patients (38%) and care-givers (34%). The most commonly asked questions were about treatment options (31%) and the natural history of the illness (28%). In 20% of the questions, the user incidentally remarked that a health-care provider had not met their information needs. Six percent of the information was objectively inaccurate. The Web can serve as an effective means for the exchange of information between individuals with a common medical condition. We found that a small amount of misinformation is exchanged and that health-care providers are sometimes perceived as unable or unwilling to supply important health-related information.  (+info)

(4/1277) Informatics at the National Institutes of Health: a call to action.

Biomedical informatics, imaging, and engineering are major forces driving the knowledge revolutions that are shaping the agendas for biomedical research and clinical medicine in the 21st century. These disciplines produce the tools and techniques to advance biomedical research, and continually feed new technologies and procedures into clinical medicine. To sustain this force, an increased investment is needed in the physics, biomedical science, engineering, mathematics, information science, and computer science undergirding biomedical informatics, engineering, and imaging. This investment should be made primarily through the National Institutes of Health (NIH). However, the NIH is not structured to support such disciplines as biomedical informatics, engineering, and imaging that cross boundaries between disease- and organ-oriented institutes. The solution to this dilemma is the creation of a new institute or center at the NIH devoted to biomedical imaging, engineering, and informatics. Bills are being introduced into the 106th Congress to authorize such an entity. The pathway is long and arduous, from the introduction of bills in the House and Senate to the realization of new opportunities for biomedical informatics, engineering, and imaging at the NIH. There are many opportunities for medical informaticians to contribute to this realization.  (+info)

Informatics and information technology do not appear to be valued by the health industry to the degree that they are in other industries. The agenda for health informatics should be presented so that value to the health system is linked directly to required investment. The agenda should acknowledge the foundation provided by the current health system and the role of financial issues, system impediments, policy, and knowledge in effecting change. The desired outcomes should be compelling, such as improved public health, improved quality as perceived by consumers, and lower costs. Strategies to achieve these outcomes should derive from the differentia of health, opportunities to leverage other efforts, and lessons from successes inside and outside the health industry. Examples might include using logistics to improve quality, mass customization to adapt to individual values, and system thinking to change the game to one that can be won. The justification for the informatics infrastructure of a virtual health care data bank, a national health care knowledge base, and a personal clinical health record flows naturally from these strategies.  (+info)

Non-health-care uses of information technology (IT) provide important lessons for health care informatics that are often overlooked because of the focus on the ways in which health care is different from other domains. Eight examples of IT use outside health care provide a context in which to examine the content and potential relevance of these lessons. Drawn from personal experience, five books, and two interviews, the examples deal with the role of leadership, academia, the private sector, the government, and individuals working in large organizations. The interviews focus on the need to manage technologic change. The lessons shed light on how to manage complexity, create and deploy standards, empower individuals, and overcome the occasional "wrongness" of conventional wisdom. One conclusion is that any health care informatics self-examination should be outward-looking and focus on the role of health care IT in the larger context of the evolving uses of IT in all domains.  (+info)

(7/1277) Personalized health care and business success: can informatics bring us to the promised land?

Perrow's models of organizational technologies provide a framework for analyzing clinical work processes and identifying the management structures and informatics tools to support each model. From this perspective, health care is a mixed model in which knowledge workers require flexible management and a variety of informatics tools. A Venn diagram representing the content of clinical decisions shows that uncertainties in the components of clinical decisions largely determine which type of clinical work process is in play at a given moment. By reducing uncertainties in clinical decisions, informatics tools can support the appropriate implementation of knowledge and free clinicians to use their creativity where patients require new or unique interventions. Outside health care, information technologies have made possible breakthrough strategies for business success that would otherwise have been impossible. Can health informatics work similar magic and help health care agencies fulfill their social mission while establishing sound business practices? One way to do this would be through personalized health care. Extensive data collected from patients could be aggregated and analyzed to support better decisions for the care of individual patients as well as provide projections of the need for health services for strategic and tactical planning. By making excellent care for each patient possible, reducing the "inventory" of little-needed services, and targeting resources to population needs, informatics can offer a route to the "promised land" of adequate resources and high-quality care.  (+info)

(8/1277) Randomised trial of personalised computer based information for cancer patients.

OBJECTIVE: To compare the use and effect of a computer based information system for cancer patients that is personalised using each patient's medical record with a system providing only general information and with information provided in booklets. DESIGN: Randomised trial with three groups. Data collected at start of radiotherapy, one week later (when information provided), three weeks later, and three months later. PARTICIPANTS: 525 patients started radical radiotherapy; 438 completed follow up. INTERVENTIONS: Two groups were offered information via computer (personalised or general information, or both) with open access to computer thereafter; the third group was offered a selection of information booklets. OUTCOMES: Patients' views and preferences, use of computer and information, and psychological status; doctors' perceptions; cost of interventions. RESULTS: More patients offered the personalised information said that they had learnt something new, thought the information was relevant, used the computer again, and showed their computer printouts to others. There were no major differences in doctors' perceptions of patients. More of the general computer group were anxious at three months. With an electronic patient record system, in the long run the personalised information system would cost no more than the general system. Full access to booklets cost twice as much as the general system. CONCLUSIONS: Patients preferred computer systems that provided information from their medical records to systems that just provided general information. This has implications for the design and implementation of electronic patient record systems and reliance on general sources of patient information.  (+info)

I'm developing a health informatics program to help manage patient records. Unfortunately, I don't have any examples of medical files that I can use. Does anybody have know of a website that has an "average" medical file?

I don't know of any websites that have an example of medical files.  If you could find a medical transcription book at the library it will have all the information you are looking for.


What is a medical informatics?


Health informatics or medical informatics is the intersection of information science, computer science, and health care. It deals with the resources, devices, and methods required to optimize the acquisition, storage, retrieval, and use of information in health and biomedicine. Health informatics tools include not only computers but also clinical guidelines, formal medical terminologies, and information and communication systems.

AMIA is the professional home for biomedical and health informatics. AMIA is dedicated to promoting the effective organization, analysis, management, and use of information in health care in support of patient care, public health, teaching, research, administration, and related policy.


How is medical marijuana working out in the states that have legalized it?


I'm doing a speech for school about medical marijuana. I know it's legal in several states. What I don't know is how well these programs are working out. I'm hoping to find the most successful examples of medical marijuana use. My goal is to find the best way to implement such a program in my state, Arkansas, where it is illegal to use marijuana for any reason. I'd like to hear about problems that have come up and what was done to correct them. I am a member of a group of 5 people doing the speech, which is pro legalization of medical marijuana. My portion is 'visualization', where I must speak for about 5 minutes about what it would be like if medical marijuana use were legalized in Arkansas.

The state that is probably the most advanced in this area is California. There are several cities and other localities in California that have more or less officially sanctioned medical marijuana and allow "compassion clubs" to operate. These are dispensaries that sell or give marijuana to people who have the proper permits.

There have been some reported problems with these clubs. One is that they are sometimes not well-received by the local officials who are using any kind of excuse they can think of to shut them down. In many cases, they get the DEA to raid the places. In other places, such as the Oaksterdam area of Oakland, the facilities receive the support of the local community and were a large part of the reason that a former "war zone" area of Oakland has experienced a rebirth. See http://www.oaksterdamnews.com/ for more info on that.

Local officials in some places have lodged complaints against the places such as that they are selling to people who aren't apparently sick. The state law, of course, does not require that people be visibly sick and no one can be diagnosed with anything simply by seeing them on the street. 

There have also been complaints that the compassion clubs occasionally get robbed. This is the same as complaining about convenience stores because they get robbed -- it is not really the fault of the person getting robbed and the solution is not to punish the victims of robbery.

Another complaint is that they bring undesireable elements to the area or that they patrons take their purchased marijuana and sell it on the street. While these kinds of things may occur in some places that are not well-run, they are by no means universal. It depends largely on the management and local regulation of the individual clubs. In the places where they are properly regulated and have the support of the local officials, these problems are not significant. 

A better question is why marijuana use should be punished at all -- regardless of whether it is for medical purposes or "personal use" (non-medical).  Every major study of the subject in the past 100 years has concluded that the marijuana laws were the product of racism, ignorance, and nonsense, and that they should have been repealed long ago because they do more harm than good.

Is medical assitant a way to start in the medical field?


I was wondering if i made the right choice to start off in the medical field which is Medical Assistant.  I heard from some people who had graduated from the program that it is hard to find a job.

What exactly do you want to do or be?  To be a nurse, you would need more schooling.  But being a medical assistant alone, is a wonderful job. You could become the Head Med. Assistant, which would mean more $$.  Talk to the office manager.  She might have some ideas for you.

Be aware of working with a lot of women.  Women ten to get pretty B****y! So stay out of gossiping and back-stabbing, and just do your job!!  :)


How do I get medical coupons, if I have medical insurance?


I know the question sounds stupid but, my husband and I just found out that we are having a baby in January.  I have medical insurance, which does cover maternal care, with the exception of copay, and deductibles, all that good stuff.  Although we have been trying for out lil miracle, it has been so long, that it surprised us when the test came out positive.  My husband had some employment struggles and he is in the process of finding full-time employment.  Which means for us that we are going to have a lot of medical bills that will be difficult to pay.  My original question is can I get medical coupons if I am not making enough to support everything?  Even though I have insurance?

Check with your Department of Health.  You may be eligible for Medicaid--to cover the rest of your expenses.  You may also be eligible for WIC to help with food for you and the baby.  My husband and I struggled through the hospital bills with our first, just to find out we probably could have gotten some sort of assistance.  Your hospital probably has payment options, as well.  With our second child, we were able to pay all of our hospital bills right away--except for the BIG one.  We're making equal monthly payments for a year and not paying any interest.  

Another thing you should look into is a Flexible Spending Account.  If your company offers this as one of their benefits, sign up for it during the open enrollment period at the end of the year.  Since your baby is due in January, this may be a real help.  The way it works is:  You estimate how much your medical bills come to for the year. You tell the FSA administrator that you want to have that amount in your FSA.  You get that amount to spend UP FRONT.  Then, you repay the amount out of your paycheck for the rest of the year--and that comes out before taxes.  I have 2 kids and a husband that are on my benefits.  I estimated that we'll spend about $1040 on medical expenses.  So, $20 a week comes out of my check for that--and since it's pre-tax, I don't even miss it.  The rules of the account state that I can only use that money for medical, dental, prescriptions, and OTC medications for me and my dependents.  And I have to be able to produce receipts for these expenditures when they request them for IRS reporting purposes.  It is worth it, though.

Is a doctor belonging to just one single medical group possibly better?


We have been trying to short list a few Primary physicians (Internal medicine or Family practice) in our area. Some doctors belong to just one medical group where as other doctors belong to multiple medical groups. 

Can we make any assumptions based on this - example - the doctor belonging to multiple medical groups is possibly not getting enough patients in one medical group and hence needs to list himself in multiple medical groups. Similarly can we also assume the doctor belonging to just one medical group is able to sustain herself with enough patients in just that medical group and hence possibly slightly better?

I wouldn't assume so.
getting many patients doesn't neccesarily mean that the doctor is good. 
I would highly suggest you go with the Internist, not the family medicine doctor

 I have been doing some research on the quinoa plant. I can't seem to find enough information on the benefits medically from the quinoa. Can you help me, by telling me the medical benefits of the plant? As well as, which part of the plant helps each medical condition/problem? Links(not wikipedia) to sites about the medical benefits can help me too.

It is good for those with celiac disease. Quinoa isn't a grass, so it doesn't cause problems. It used to be grown mostly in South America and it has nigh levels of Vit B and protein. 
I have eaten it, it isn't bad. You can buy it at Whole Foods.


What to do before a medical examination?


I'm looking to pass a medical examination for pre-employment in flight catering. I was wondering if there are any tips as to what I can do before the actual medical itself that would maybe increase my chances of passing?

I don't have a medical condition, I seem healthy and it's one day away... So.. Anything?
