BACKGROUND: The accreditation and provision of continuing education for general practitioners (GPs) is set to change with new proposals from the General Medical Council, the Government, and the Chief Medical Officer. AIM: To review the theories, policies, strategies, and effectiveness in GP continuing education in the past 10 years. METHOD: A systematic review of the literature by computerized and manual searches of relevant journals and books. RESULTS: Educational theory suggests that continuing education (CE) should be work-based and use the learner's experiences. Audit can play an important role in determining performance and needs assessment, but at present is largely a separate activity. Educational and professional support, such as through mentors or co-tutors, has been successfully piloted but awaits larger scale evaluation. Most accredited educational events are still the postgraduate centre lecture, and GP Tutors have a variable role in CE management and provision. Controlled trials of CE strategies suggest effectiveness is enhanced by personal feedback and work prompts. Qualitative studies have demonstrated that education plays only a small part in influencing doctors' behavior. CONCLUSION: Maintaining good clinical practice is on many stakeholders' agendas. (2/1652) An audit of distribution and use of guidelines for management of head injury.

Ensuring effective distribution of guidelines is an important step towards their implementation. To examine the effectiveness of dissemination of a guidelines card on management of head injury and determine its usefulness to senior house officers (SHOs), a questionnaire survey was performed in May 1990, after distribution of the cards in induction packs for new doctors and at postgraduate lectures and displaying the guidelines in accident and emergency departments and wards. A further survey, in March 1992, assessed the impact of modifying the distribution. All (175) SHOs working in general surgery, accident and emergency medicine, orthopaedics, and neurosciences on 1 February 1990 in 19 hospitals including two neurosurgical units in Northern region were sent self completion questionnaires about awareness, receipt, use, and perceived usefulness of the guidelines. 131 of 163(80%) SHOs in post responded (median response from hospitals 83% (range 50%-100%)). Over three quarters (103, 79%) of SHOs were aware of the guidelines and 82(63%) had ever possessed a guidelines card. Only 36(44%) acquired the card in the induction pack. 92%(98/107) found them useful and 81% (89/110) referred to them to some extent. Owning and carrying the card and referring to guidelines were associated with departmental encouragement to use the guidelines. Increasing the displays of guidelines in wards and departments and the supply of cards to consultants in accident and emergency medicine as a result of this survey did not increase the number of SHOs who received cards (52/83, 63%), but more (71/83, 86%) were aware of the guidelines. The guidelines were welcomed by SHOs and used in treating patients with head injury, but their distribution requires improvement. Increased use of the guidelines may be achieved by introducing other distribution methods and as a result of encouragement by senior staff.  (+info)

(3/1652) Investigation into the attitudes of general practitioners in Staffordshire to medical audit.

OBJECTIVES: To investigate the attitudes of general practitioners to medical audit, and any associations between their attitudes and their personal characteristics. DESIGN: Postal questionnaire survey. SETTING --Staffordshire, United Kingdom. SUBJECTS: 870 Staffordshire general practitioners. MAIN MEASURES: Agreement or disagreement and associations between the attitudes to 16 statements about audit and the doctors' personal or practice characteristics--namely, sex, number of years since qualification, practice list size, number of partners, and the practices' experience of audit. RESULTS: 601 Staffordshire general practitioners (69%) responded. There was most agreement with the statements that audit is time consuming (86%), that ongoing training and education is needed (71%), that there is a compulsion applied on doctors to audit (68%), and that extra resources for audit should be provided by the medical audit advisory group (65%). There was considerable disagreement (53% of general practitioners) with the statement that inverted question markgovernment policy to expect general practitioners to do audit will enhance the population's health. inverted question mark The median response by the 601 general practitioners was four positive responses out of 14 statements about audit (two of the 16 statements could not be graded positive or negative to audit). Women doctors generally had more positive attitudes towards audit, and so had those working with smaller mean list sizes, those in larger partnerships, and those in practices that had carried out audit for a longer time. CONCLUSIONS: There was a generally negative attitude to medical audit, but it was encouraging that those doctors with the most experience of audit obtained the most job satisfaction from it. IMPLICATIONS: More effort is needed to convince general practitioners of the value of audit. Without this, attempts to involve other members of the primary care team in multidisciplinary clinical audit are unlikely to be effective. Successful audits that are shown to be cost effective as well as leading to improvements in patient care should be publicised and replicated. Healthcare reform has created a new working environment for practicing physicians, as economic issues have become inseparably intertwined with clinical practice. Although physicians have recognized this change, and some are returning to school for formal education in business and healthcare administration, formal education may not be practical or desirable for the majority of practicing physicians. Other curriculum models to meet the needs of these professionals should be considered, particularly given the growing interest in continuing education for physicians in the areas of managed care and related aspects of practice management. Currently, no theory-based models for implementing a managed care curriculum specifically for working physicians have been developed. This paper will integrate diffusion theory, instructional systems design theory, and learning theory as they apply to the implementation of a managed care curriculum for continuing medical education. Through integration of theory with practical application, a CME curriculum for practicing physicians can be both innovative as well as effective. This integration offers the benefit of educational programs within the context of realistic situations that physicians can apply to their own work settings.  (+info)

Physicians must incorporate concepts of practice management and knowledge of managed care into their practices. Managed care presents an immediate and challenging opportunity to providers of continuing medical education to offer effective educational programs for physicians on managed care issues. In this exploratory research, the problem-based learning model was used to develop a continuing medical education program that would offer an interactive and effective method for teaching physicians about managed care. Problem-based learning is a departure from the traditional lecture format of continuing medical education programs because it is designed for small groups of self-directed learners who are guided by a faculty facilitator. Although only a small number of participants participated in this program, the findings offer important considerations for providers of continuing medical education. For example, participants reported increased confidence in their knowledge about managed care issues. Participants also clearly indicated a preference for the small group, interactive format of the problem-based learning model.  (+info)

(8/1652) Identifying and managing patients with hyperlipidemia.

Cardiovascular disease related to hyperlipidemia is a significant cause of morbidity and mortality in the United States. The benefit of lowering lipid levels in patients with and without cardiovascular disease has been demonstrated in numerous clinical trials. The results of these trials prompted the National Heart, Blood, and Lung Institute to form the Nation Cholesterol Education Panel (NCEP). This panel developed guidelines for identifying and treating lipid disorders. Before starting antilipemic therapy, patients should be evaluated for secondary causes of hyperlipidemia, including disease states and medications. Risk factors for cardiovascular disease should be identified and used to determine the patient's goal low-density lipoprotein level. Regardless of the drug therapy used, the cornerstone treatment for hyperlipidemia is dietary changes. The NCEP recommendation for dietary modification follows a two-step plan to reduce intake of cholesterol and dietary fats. Other nonpharmacologic treatments for hyperlipidemia include exercise, weight reduction for obese patients, reduction of excessive alcohol use, and smoking cessation . Drug therapy should be considered in patients who do not respond to an adequate trial of dietary modifications and lifestyle changes. The principal lipid-lowering agents currently used are the bile acid sequestrants, nicotinic acid, 3-hydroxy-3-methylglutaryl coenzyme A (HMG CoA) reductase inhibitors, and fibric acid derivatives. Estrogen, fish oil, and alcohol also can decrease the risk of developing heart disease. In pharmacoeconomic studies, lipid-lowering drug therapy has been shown to decrease the number of procedures, hospitalizations, and other medical interventions required by patients with cardiovascular disease.  (+info)


How is medical marijuana working out in the states that have legalized it?


I'm doing a speech for school about medical marijuana. I know it's legal in several states. What I don't know is how well these programs are working out. I'm hoping to find the most successful examples of medical marijuana use. My goal is to find the best way to implement such a program in my state, Arkansas, where it is illegal to use marijuana for any reason. I'd like to hear about problems that have come up and what was done to correct them. I am a member of a group of 5 people doing the speech, which is pro legalization of medical marijuana. My portion is 'visualization', where I must speak for about 5 minutes about what it would be like if medical marijuana use were legalized in Arkansas.

The state that is probably the most advanced in this area is California. There are several cities and other localities in California that have more or less officially sanctioned medical marijuana and allow "compassion clubs" to operate. These are dispensaries that sell or give marijuana to people who have the proper permits.

There have been some reported problems with these clubs. One is that they are sometimes not well-received by the local officials who are using any kind of excuse they can think of to shut them down. In many cases, they get the DEA to raid the places. In other places, such as the Oaksterdam area of Oakland, the facilities receive the support of the local community and were a large part of the reason that a former "war zone" area of Oakland has experienced a rebirth. See http://www.oaksterdamnews.com/ for more info on that.

Local officials in some places have lodged complaints against the places such as that they are selling to people who aren't apparently sick. The state law, of course, does not require that people be visibly sick and no one can be diagnosed with anything simply by seeing them on the street. 

There have also been complaints that the compassion clubs occasionally get robbed. This is the same as complaining about convenience stores because they get robbed -- it is not really the fault of the person getting robbed and the solution is not to punish the victims of robbery.

Another complaint is that they bring undesireable elements to the area or that they patrons take their purchased marijuana and sell it on the street. While these kinds of things may occur in some places that are not well-run, they are by no means universal. It depends largely on the management and local regulation of the individual clubs. In the places where they are properly regulated and have the support of the local officials, these problems are not significant. 

A better question is why marijuana use should be punished at all -- regardless of whether it is for medical purposes or "personal use" (non-medical).  Every major study of the subject in the past 100 years has concluded that the marijuana laws were the product of racism, ignorance, and nonsense, and that they should have been repealed long ago because they do more harm than good.

How do i obtain my medical records for when i was in a psychiatric hospital?


Im interested in reviewing my medical records for when i was in the hospital for reasons of my diagnoses, notes they kept of me, symptoms i showed my reactions to things, just everything from when i was in. I know i would have to call the hospital but would i have to go in person to sign for them? What exactly do i ask for?
Someone told me in my other post about the same question, that psychiatric records are way different to try getting than a regular medical record. Is this true and if so, why?

You just need to call them and request a copy of your complete medical file (or records) while you were under the services of their facility.  You might ask if you need to individually contact each physician that worked with you... Sometimes they require the request in writing, or they require that you come personally (with I.D.) to sign for your records, and don't be surprised if they ask you to pay for a copy of your records.  Unfortunately, a lot of facilities and private practices request a fee for the release of your records.  Good luck!


How soon after a medical abortion did you have your next period?


I had a medical abortion 6 weeks ago today. I still haven't had a normal period and there are no signs of it coming. I'm just curious about when it came back for most other women. I don't think I'm pregnant, so I'm not worried about that. I'd just like for my cycles to get back to normal so I can get on with my life. Thanks.

Wow, since no one has actually bothered to answer this question, I'm going to be the first (I really don't understand why people have to use this as a forum for their pro-life nonsense.)
After my abortion, I had my first "period" about 2 weeks afterwards (although the nurses after the operation said it would be about 3-4 weeks before this would happen so I guess it's different for every woman.) I was later told that this wasn't a period but instead it was normal bleeding that occurs after the procedure. It hurts a LOT.
Then about 5-6 weeks after that, my nomal period resumed. 
So you should be fine but if you're still worried, see a doctor. It's very unlikely that you could still be pregnant after a medical abortion as there is so little room for error but you never can be too careful. Or maybe it's the stress of the whole situation you've just been through that's preventing your period coming back.

Medical transcription is becoming a big trend in todays medical field.  Especially with smaller MD offices.  They do not have the staff to do this task.  So usually, one would have several MD offices on contract to do their tanscriptions.  Good money too.  Local community colleges offer a course.  In just a short time you could be certified.  Check your local community college or look in the free ADs paper, like "thirty nickel", or "green sheet" there is usually ads in them that could help you.  Good Luck

In order to get a medical marijuana card you must have a debilitating condition. Now if you have a debilitating medical condition, how can you possibly  be in good enough shape to handle what is one of the most physically demanding jobs in the world? If you put this on your record you will exempt yourself from many employment opportunities.  I use marijuana as a medicine. I don't need it to survive, but since I'm over 50, broke up from years in the Army and the oilfield, it's undeniable that I can receive "Palliative Relief" (as our state law defines it) with the use of marijuana, and I enjoy is effects. Look man, my heart goes out to you, but if you want a career in public service, do not do this. If you want to use marijuana socially, you must be discreet-I spent many years in the Army indulging from time to time. We're only a few years away from total legalization. Until then you've got to play the game. Be smart and KISS.


What is wrong with the medical community that continues to prescribe fluoroquinolone antibiotics?


Cipro, levaquin and all fluoroquinolone antibiotics should have a black box label to prevent MD's from handing them out like candy.  I personally know four people that have had horrible adverse effects from the drug.  Some of these effects don't start for weeks or months and can be permanent. Check out the fluoroquinolone toxicity research forum.  The denial of the medical profession to address this is a tragedy for victims.  Even Bayer (the manufacturer admits these horrid effects are from these drugs.  Your tendons can rupture, peripheal nerve damage, cardiac damage, and severe anxiety to name a few.  If this is brought to the attention of your doctor, you will probably be referred to a shrink.  There is a class action lawsuit from the postal workers given cipro for the anthrax scare.  They have experienced horrible side effects.  How can patients get their doctors out of denial?

It is naive to think that doctors are swayed by the drug reps.  It would not be worth it to prescribe a contraindicated medication just so I could continue to get pedicures.  From experience, many docs hate to see drug reps b/c they slow you down.

Chemotheraphy has cured up to 70% of leukemia, I'm sure no alternative medicine can do that.


What does a medical license entitle a doctor to practice in Michigan?

In theory, a medical license gives someone legal permission to do just about anything he/she wants, medically. Kinda stupid, but true.

In reality, this is, of course, is not the case. There are restraints, such as the terms of malpractice insurance, threats of a lawsuit, peer pressure, hospital privilege limitations, personal integrity and commons sense.

Occasionally the restraints fail, and patients suffer the consequences. Usually, the only damage to the doctor is higher malpractice costs.
