Comprehensive Health Care: Providing for the full range of personal health services for diagnosis, treatment, follow-up and rehabilitation of patients.Health Care Reform: Innovation and improvement of the health care system by reappraisal, amendment of services, and removal of faults and abuses in providing and distributing health services to patients. It includes a re-alignment of health services and health insurance to maximum demographic elements (the unemployed, indigent, uninsured, elderly, inner cities, rural areas) with reference to coverage, hospitalization, pricing and cost containment, insurers' and employers' costs, pre-existing medical conditions, prescribed drugs, equipment, and services.Universal Coverage: Health insurance coverage for all persons in a state or country, rather than for some subset of the population. Factors influencing this ability include geographic, architectural, transportational, and financial considerations, among others.Insurance, Health: Insurance providing coverage of medical, surgical, or hospital care in general or for which there is no specific heading.United StatesHealth Status: The level of health of the individual, group, or population as subjectively assessed by the individual or by more objective measures.Public Health: Branch of medicine concerned with the prevention and control of disease and disability, and the promotion of physical and mental health of the population on the international, national, state, or municipal level.Cross-Sectional Studies: Studies in which the presence or absence of disease or other health-related variables are determined in each member of the study population or in a representative sample at one particular time. This contrasts with LONGITUDINAL STUDIES which are followed over a period of time.Questionnaires: Predetermined sets of questions used to collect data - clinical data, social status, occupational group, etc. The term is often applied to a self-completed survey instrument.Health Surveys: A systematic collection of factual data pertaining to health and disease in a human population within a given geographic area.Health Promotion: Encouraging consumer behaviors most likely to optimize health potentials (physical and psychosocial) through health information, preventive programs, and access to medical care.Risk Factors: An aspect of personal behavior or lifestyle, environmental exposure, or inborn or inherited characteristic, which, on the basis of epidemiologic evidence, is known to be associated with a health-related condition considered important to prevent.Mental Health: The state wherein the person is well adjusted.Health: The state of the organism when it functions optimally without evidence of disease.Attitude to Health: Public attitudes toward health, disease, and the medical care system.Health Care Surveys: Statistical measures of utilization and other aspects of the provision of health care services including hospitalization and ambulatory care.Health Planning: Planning for needed health and/or welfare services and facilities.Primary Health Care: Care which provides integrated, accessible health care services by clinicians who are accountable for addressing a large majority of personal health care needs, developing a sustained partnership with patients, and practicing in the context of family and community. (1/227) Development of a heart failure center: a medical center and cardiology practice join forces to improve care and reduce costs .

Congestive heart failure (CHF) is a rapidly growing and expensive cardiovascular disorder. Conventional care for CHF is ineffective and results in a cycle of "crisis management" that includes repeated emergency department visits, hospitalizations, and physician visits. Recently, a number of outpatient coronary care centers that provide consistent, aggressive outpatient therapies and extensive patient education have emerged and are successfully breaking this cycle of dependence on hospital services. One such effort is the Heart Institute's Heart Failure Center, the result of a partnership between a private-practice cardiology group and our tertiary-care medical center. Our program includes not only patient education and outpatient infusions of inotropic agents, but an electronic linkage to the emergency department and home healthcare services. Preliminary data show that 16 months after the program was initiated, hospital admissions decreased by 30%, hospital days by 42% and average length of stay by 17%. An effective outpatient heart failure program can alleviate the economic burden of CHF and improve the quality of patient care.  (+info)

The Alma-Ata Declaration has long been regarded as a watershed in the health policy arena. The global goal of the World Health Organization, 'Health for All by the Year 2000' through primary health care, has attracted many countries both in the developed and the developing world and commitments to this end have been made at every level. However, albeit this consensus on the paper, a common and explicit definition of the concept has not been reached yet. This paper aims at discussing various definitions of primary health care that emerged after the Declaration and also presenting a case study from Turkey, a country that advocates primary health care in her recent health policy reform attempts. After setting the conceptual framework for discussion the Turkish case is presented by using research carried out among Turkish policy-makers at different levels of the State apparatus. It has been concluded that application of primary health care principles as defined in the broad definition of the concept requires major changes or rather shake-ups in Turkey. Because persons with disabilities (PWDs) use health and social services extensively, both the United States and the United Kingdom have begun to integrate care across systems. Initiatives in these two countries are examined within the context of the reality that personal needs and use of systems differ by age and by type and severity of disability. The lessons derived from this scrutiny are presented in the form of five "laws" of integration. These laws identify three levels of integration, point to alternative roles for physicians, outline resource requirements, highlight friction from differing medical and social paradigms, and urge policy makers and administrators to consider carefully who would be most appropriately selected to design, oversee, and administer integration initiatives. Both users and caregivers must be involved in planning to ensure that all three levels of integration are attended to and that the borders between medical and other systems are clarified.  (+info)

The complex array of needs posed by older adults has frequently produced fragmentation of care in traditional fee-for-service systems. Integration of care components in newer health systems will maximize patient benefits and organizational efficiency. This article outlines the major issues involved in integration of care for older populations. A health system must integrate its care of older adults in many ways: among providers, both in primary care and specialty services; with community-based sources of care; and across sites of care (clinic, hospital, emergency department, and nursing home). Integrating reimbursement structures for various services will serve to create a client-oriented system, as opposed to a finance-centered system, thereby enhancing coordination of care. The extent to which two experimental comprehensive systems, PACE (Program of All-inclusive Care of the Elderly) and SHMO II (Social Health Maintenance Organization), have achieved clinical and financial integration are discussed in detail. Healthcare organizations are encouraged to create integrated models of care and to study the effects of integration on patient outcomes.  (+info)

OBJECTIVE: The purpose of this paper is to describe the "spectrum of prevention", a framework for developing multifaceted approaches to injury prevention. The value of the tool is that it can help practitioners develop and structure comprehensive initiatives. METHODS: The spectrum is comprised of six inter-related action levels: (1) strengthening individual knowledge and skills, (2) promoting community education, (3) educating providers, (4) fostering coalitions and networks, (5) changing organizational practices, and (6) influencing policy and legislation. Activities at each of these levels have the potential to support each other and promote overall community health and safety. CONCLUSIONS: The spectrum of prevention is a tool which can help practitioners and policy leaders move beyond a primarily educational approach to achieve broad community goals through injury prevention strategies that include policy development. This framework has been endorsed and applied in a variety of disciplines, however it has not been formally evaluated, a process that could clarify the scope of its effectiveness.  (+info)

OBJECTIVE: To describe the origin, scope, operations, funding, and outcomes of innovative healthcare programs for chronically ill older persons. STUDY DESIGN: Cross-sectional survey. METHODS: A national expert panel nominated chronic illness programs they believed to be innovative and field tested. The directors of the 31 eligible programs provided descriptive information in 60-minute semistructured telephone interviews. RESULTS: The innovative programs we surveyed tended to target their services to high-risk patients, use teams of providers to deliver care, designate providers to coordinate multiple components of complex care plans, and shift care from higher- to lower-cost environments and/or redesign the delivery of primary care. CONCLUSIONS: Recent innovations in healthcare programs hold considerable promise for improving the outcomes of chronic care, but most have yet to be rigorously evaluated.  (+info)

The setting of prenatal care has long been the focus of interventions and policies to prevent these adverse outcomes. Thus it is essential to view the female life course from a broader perspective in order to consider the full range of policy options for reducing the infant mortality and morbidity caused by maternal substance abuse. The dialysis-transplantation (D-T) program at The Hospital for Sick Children, Toronto has a mental health component directed by a psychiatrist and a social worker. As of Jan. 1, 1975, 53 kidney transplants had been carried out on 44 children. Patients and their families are counselled continuously by the psychiatrist and the social worker before, during and after transplantation. Members of the multidisciplinary team meet regularly to plan treatment for the children. Mental health issues are an integral part of team discussions and help determine D-T program policy. Psychological preparation, mental health consultation, therapeutic intervention and continuous counselling prevent many of the mental health problems that plague a D-T program.  (+info)

How does the current health care system in America work?


  • I know there is a huge debate about universal health care and such on health care reform in America. But what is it that makes sparked this reform? How does the current system work and what it wrong with it? I'm trying to find the answers online, but I can't find anything that can answer my question. Thank you for all responses!
  • The real answer will be too long to post here, but a few highlights. Health care used to be under the control of doctors. In WWII the US government unconstitutionally froze wages and prices so in order to obtain and retain the best employees, employers had to add benefits as they couldn't adjust wages. One of the benes added was health insurance which was not really needed (not anywhere as important as it is today). After WWII the link remained--in peoples' minds and in fact. Come 1966 and the feds decided to take their unconstitutional Social Security program a step farther and create Medicare--now the feds were in health care big time. Medicare has grown over time and was always supposed to ensure that no doc or hospital ever got a profit from treating Medicare patients--unfortunately docs have been taken to the cleaners on the deal which is why it's getting harder and harder to find one who takes Medicare. Also premiums are on the rise, particularly in the last decade: In the US, Medicare is going bankrupt. In 1998, Medicare premiums were $43.80 and in 2008 will be $96.40--up 120%. "Medigap" insurance is common because of the 20% co-pay required for service. Medicare HMOs are common because they reduce that burden without an extra charge in many cases. HOWEVER, many procedures which used to have no or a low co-pay NOW cost the full 20% for the HMO Medicare patient. ALSO the prescription coverage they tended to offer has been REDUCED in many cases to conform to the insane "donut hole" coverage of the feds. Doctors are leaving Medicare because of the low and slow pay AND because the crazy government wants to "balance" their Ponzi scheme on the backs of doctors. "That dark cloud lurking over the shoulder of every Massachusetts physician is Medicare. If Congress does not act, doctors' payments from Medicare will be cut by about 5 percent annually, beginning next year through 2012, creating a financial hailstorm that would wreak havoc with already strained practices. Cumulatively, the proposed cuts represent a 31 percent reduction in Medicare reimbursement. The door was open for the hijacking of medical care and the skyrocketing of prices. As you see with Medicare payments, doctors would NOT be the beneficiaries. Since that time the boldness of insurance companies to ignore their contracts as well as antitrust laws has grown wildly and the government sits and twiddles its thumbs wasting time and tax dollars on "investigating" Microsoft and baseball instead of enforcing laws. Hospitals have decided that they'll charge the uninsured about 3 times what they charge the insured and have become aggressive in going after CITIZENS who don't pay their bills in full. As always, the government has ensured problems will exist with their mandates that everyone be treated at an ER if he has a life-threatening condition, regardless of ability to pay. Sounds good on paper. Hospitals deliberately misinterpret the mandate to mean "treat all illegals no matter how little they need medical care and bill the heck out of the taxpayer." Combine this with the stupid governmental mandate of "compassionate entry" (the Border Patrol is instructed to let in everyone who is ill so they can be treated here, knowing full well in many cases the taxpayer is going to eat the bill), and you have hard-working Americans paying more and seeing portions of their hospitals shut down because of governmental meddling and their inability to do their job: secure the borders. A few things that should be of interest to any thinking man on the subject of health care in the US--in other words, the pols won't discuss this and the media give it short shrift: When 75% of the people who declare bankruptcy over medical bills ARE INSURED, then insurance is CLEARLY not the answer. "Aldrich’s situation is "asinine" but increasingly common, said Dr. Deborah Thorne of Ohio University. Thorne, co-author of a widely quoted 2005 study that found medical bills contributed to nearly half of the 1.5 million personal bankruptcies filed in the U.S. each year, said that ratio has likely worsened since the data was gathered. ... Like Aldrich, Thorne said, three-quarters of the individuals in the study who declared bankruptcy because of health problems were insured.   • Health Care professionals say it is the high cost of malpractice insurance? Is it that the insurance companies are too greedy or the patients are too litigeous? So, who is zooming who here? Would state purchase of cheap foreign doctors help? or GI bill(state sponsorship) for med students who praqctice at state hospitals.
  • Healthcare cost is rising because we the people don't have a say so. Insurance companies have taken over health care. Physicians don't have the power in what they can do or not do. It is completely controlled by the insurance companies. Here is a scenario, plastic surgery or laser eye surgery. They do not cost near as much as a basic surgery in the hospital. Why because they are not controlled by innsurance plans. Most insurance don't cover these types of surgery. so the doctors are left to provide QUALITY service and competitive prices to keep clients coming back. Therefore, they are providing quality and are affordable. The customers are able to choose and select who they want to go to because they are paying for it.

In the 50's-60's when health care was run successfully as not-for-profit entities people could more readily afford care and companies still thrived, though there were still folks who slipped through the cracks. The local doctors/nurses/hospitals that we know personally in my county are actively supporting the call for Health Reform. We also have extended family in several 1st World Democratic nations (Canada and Europe). They live well, are well educated, are free citizens in free countries, and healthy. They are astounded at the mis-information out there on their country's health care. They pay higher taxes than us but have higher incomes for the same type of jobs, which balances that fact, and so live equally as well as we do. Again, this is our personal experience which has influenced our opinions. I have no problem as a citizen of a government run by the people, for the people, seeing my tax dollars used to improve my fellow countryman's health.

  • I live in Michigan, and Molina Health care was an option for me, also, but when I called the insurance company to get enrolled, and then I called my doctor to make an appointment, and my doctor said what kind of health care to you have? And I said Molina, and my doctor said that he does not accept it, and I had to go through the whole process of getting a health plan that my doctor does accept, and it was a pain, but I finally got on Health Plan of Michigan, and it is wonderful♥ My advice would be to call your doctor or the insurance companies and find out if your doctor is one of the providers listed under their plan, and if he is not, then that you will not have to go through what I did in getting a health plan and then making a hundred phone calls to change plans and get one that my doctor will accept. What a headache that was♥
